Objective To construct the sequential management model of networked drug-cognitive behavior in patients with chronic insomnia, and to explore the effects of this model on treatment and management of patients with chronic insomnia. Methods A total of 160 patients with chronic insomnia treated from January 2014 to June 2015 were randomly divided into the experimental group and the control group. The experimental group was treated with networked drug-cognitive behavior sequential therapy and management. Through the establishment of patient management files in the network management system, the disease was assessed, treatment programs were developed, remote implementation of 8 weeks of drug-cognitive behavior sequential treatment was conducted,12 months of net-work remote dynamic management and efficacy evaluation was performed. The control group received 8 weeks of traditional medical care with face-to-face drug-cognitive behavior sequential treatment and 12 months outpatient fol-low-up management. Results Comparison of management core indicators:there were significant differences between two groups in number of visiting hospital, exit status, treatment completion and documentation, sleep diary comple-tion,sleep scale completion and patient satisfaction. Comparison of sleep quality:after 2 months of treatment, there was no difference in quality of sleep between two groups; after 12 months of treatment, there were significant dif-ferences in sleep latency, awakening time after sleep, total sleep time and sleep efficiency between two groups. Scale score:after 12 months of treatment, there were significant differences in Pittsburgh sleep quality index, sleep personal beliefs and attitude scale score between two groups. Conclusion Network-based management improves the compliance of patients with chronic insomnia, reduces the loss of follow-up rate, improves sleep cognition, increases sleep quality, saves patients' time and cost, increases patient satisfaction, which is worth promoting in clinical application.

Objective To evaluate the reliability and validity of hepatocellular carcinoma (HCC)using the liver imaging reporting and data system (LI-RADS).Methods By using the LI-RADS v2013.1,two radiologists evaluated 1 62 patients with cirrhosis or with a risk of HCC according to an inclusion criteria. The Kappa value was used to evaluate the consistency between two different diagnoses and was compared with pathological and follow-up results. The accuracy of the LI-RADS was assessed including sensitivi-ty,specificity,positive predictive value, negative predictive value, positive likelihood ratio, negative likelihood ratio and accuracy rating. Results In all 1 62 patients, there were HCCs in 97 patients including 7 patients belonging to the LR3,benign diseases in 50,and other kinds of malignancy in 1 5.The Kappa value was 0.882 (P =0.000)between two observers on LI-RADS grading. The sensitivity, specificity and accuracy rating of LI-RADS grading in diagnosing HCC was 100.00%,91.30%,and 97.06%,re-spectively.Conclusion The LI-RADS has high consistency and stability in evaluation and diagnosis of HCC by enhanced CT.LR3 di-agnosis should be cautious because of a susceptible development to HCC,which can be improved through the combination of clinic and laboratory examination.

Aminoglycoside antibiotics, due to their strong antibacterial effects and broad antimicrobial spectra, have been very commonly used in clinical practice in the past half century. However, aminoglycoside antibiotics manifest severe ototoxicity and nephrotoxicity, and are one of top factors in hearing loss. In this study, three members of the aminoglycoside antibiotics family, gentamycin, neomycin and streptomycin, were chosen as the representatives to be investigated for their toxicity to the embryonic development and the larva hair cells in zebrafish, and also to their target genes associated with hearing-related genes. The results showed that: (1) the lethal effect of all three drugs demonstrated a significant dependence on concentration, and the severity order of the lethal effect was streptomycin > neomycin > gentamycin; (2) all the three drugs caused the larva trunk bending in resting state at 5 dpf (day past fertilization), probably due to their ototoxicity in the physical imbalance and postural abnormalities; (3) impairment and reducing of the hair cells were observed in all three cases of drug treatment; (4) four genes, eya1, val, otx2 and dlx6a, which play an important role in the development of hearing organs, showed differential and significant decrease of gene expression in a drug concentration-dependent manner. This study for the first time reports the relevance between the expression of hearing genes and the three ototoxic antibiotics and also proved the feasibility of establishing a simple, accurate, intuitive and fast model with zebrafish for the detection of drug ototoxicity.

From 2006 to 2008, the susceptibility of different species of animal hosts to Campyebacter jejubni infection was observed in various areas of Jiangsu province, in which the API Campy System was used to perform the biochemical identification and the multiple PCR assay was employed to analyse the C.jejuni isolates from 3010 specimens of fouls. Cattle, pigs and monkeys, and in addition, the susceptibility of isolates to various antibiotics was also determined. In these specimens investigated 402 samples were found to be positive in the detection of C.jejuni with a positive detection rate of 13.36%. The positive detection rates in chicken, water fouls, milk cows, pigs, monkey, red crowned crane and wapiti were 15.83% (258/1630), 10.4% (52/100), 8.24% (42/510), 15.63% (25/160), 15% (15/100), 12.5% (15/80) and 0% (0/30) respectively. Meanwhile, the antibiotics to which the isolates from different hosts showed high rate of sensitivity to 27 antibiotics of 10 varieties included: chloromycetin (100%), Almocylin (99.7%), amicarcin (92.59%), cefprozil (91.67%), alchimycin (90.74%); while the antibiotics to which these isolates showed high rate of resistance were compound neoromin (99.7%), cefoperazone (99.07%), trimethoprim (97.22%), cepronatin (91.67%), cepromondo (99,07%) respectively. It is evident that the susceptibility of different hosts to C.jejuni infection and the status of drug-resistance of the isolates appear to be quite different and more complicated in Jiangsu province.
