2.The Effects of Carotid Arteriosclerosis on Cognitive Functions and Recent Prognosis in Patients with Cerebral Infarction

Objective: To investigate the effects of carotid arteriosclerosis on cognitive functions and current prognosis in patients with cerebral infarction. Methods: Extracranial carotid arteries were assessed using dopple ultrasongraphy in 112 patients with cerebral infarction. Cognitive functions were evaluated with the Mini-Mental state Examination (MMSE) and five neuropsychological tests assessing memory, attention, calculation psychomotor rapidity and visuospatial perception. Deficits in neurological functions were assessed on admission and 3 to 4 weeks. Results:All neuropsychological measures were found to be poorer in patients with carotid arteriosclerosis than those with no carotid arteriosclerosis, especially in cases with severe carotid arteriosclerosis and severe carotid stenosis. There was a positive relationship between severity of carotid arteriosclerosis and change in cognitive functions. The scores of SSS were higher on admission in cerebral infarction patients with carotid arteriosclerosis. Recent prognosis was also poorer in patients with carotid arteriosclerosis.Conclusions: Significant effect of carotid arteriosclerosis was shown on cognitive functions of patients with cerebral infarction. As cerebral ischemic injury is severe, prognosis in cerebral infarction patients with carotid arteriosclerosis is poor.
3.The relationship between carotid atherosclerosis and cerebral infarction recurrence

Objective To study the clinical significance of carotid atherosclerosis and the relationship between carotid atherosclerosis and cerebral infarction recurrence.Methods Carotid atherosclerosis was assessed using doppler ultrasongraphy in 312 patients with cerebral infarction, the duration of following up was 12 to 18 months,and the characteristics of carotid atherosclersis was compared to patients with or without recurrence cerebral infarction.Results Of the 312 patients with cerebral infarction, 61 patients suffered from new cerebral infarction during following up period, the recurrence rate in patients with atherothrombotic brain infarction was higher than those with lacunar infarction, and the cerebral infarction recurrence usually occurred in the same side of initial stroke of the 57 patients with severe carotid atherosclerosis, 26 patients had a sufference of new infarction, of the 48 patients with high grade stenosis, 25 had a sufference of new cerebral infarction, of the 42 patients with ulcerated plaque, 23 had sufference of new cerebral infarction,showing a recurrence rate significantly higher than those patients with non carotid atherosclerosis or those with mild carotid atherosclerosis. The logistic regression analysis showed that the levels of carotid atheroslerosis , stenosis and ulcerated plaque were positively related to the cerebral infarction recurrence.Conclusions There is a positively relationship between carotid atherosclerosis and cerebral infarction recuurrence, and the level of carotid atherosclerosis is a risk factor of cerebral infarction recurrence.It serves as a risk marker of cerebral infarction recurrence.
4.Ischemic postconditioning alleviates lung ischemic/reperfusion injury induced by cardiopulmonary bypass

ObjectiveTo investigate the role of ischemic postconditioning on lung ischemic/reperfusion injury induced by cardiopulmonary bypass (CPB) in combined mitral and aortic valve replacement,and evaluate the effects of different protocol of ischemic postconditioning,and analyze its possible mechanisms.Methods24 patients diagnosed as rheumatic mitral and aortic valve disease with or without tricuspid valve disease were randomly divided into 3 groups (8 in each):Control group( group A),patients undergoing routine cardiac surgery; Postconditioning group Ⅰ (group B ),patients undergoing routine cardiac surgery and ischemic postconditioning by occlusion of the pulmonary artery of five cycles of 15 s ischemia and 15 s repeffusion before the pulmonary artery totally restoring peffusion; Postconditioning group Ⅱ (group C),patients undergoing routine cardiac surgery and ischemic postconditioning by occlusion of the pulmonary artery of five cycles of 30 s ischemia and 30 s repeffusion before the pulmonary artery totally restoring perfusion.The Oxygenation Index (OI) was measured at pre -operation and 1 h,2 h,3 h,6 h and 12 h after terminating CPB.The plasma contents of MDA were detected by Enzyme-linked Immunosorbent Assay at pre -operation,1 h,3 h,6 h and 12 h after terminating CPB.ResultsCompared with group A and C,OI in group B significantly increased at 1 h,2 h,3 h,6 h after terminating CPB (283.25 ±56.47 vs 384.76 ±29.17 vs 310.50 ±65.71,265.75 ±58.78 vs 381.75 ±29.67 vs 310.50 ±48.17,283.75 ±73.15 vs389.74±39.34 vs 317.87 ±78.41,310.37 ±52.00 vs 401.62 ±4 2.89 vs 337.25 ± 64.06,all P <0.05),and the plasma contents of MDA in group B reduced at 1 h,3 h,6 h,12 h after terminating CPB (4.64±0.63 vs 3.88 ±0.20 vs 4.38 ±0.41,5.75±0.49 vs 4.44 ±0.34 vs 5.28 ±0.76,4.42±0.31vs 3.77 ± 0.40 vs 4.35 ± 0.54,3.74 ± 0.31 vs 3.19 ± 0.17 vs 3.64 ± 0.24,all P < 0.05 ), However,there were no significant differences in OI and MDA between group C and A (all P >0.05 ).Conclusions Ischemic postconditioning by occlusion of the pulmonary artery of 5 cycles of 15 s ischemia and 15 s reperfusion attenuates lung oxygenation function injury induced by CPB in double valves replacement,and it is possibly mediated by reducing MDA produced by lipid peroxidation.However,ischemic postconditioning by occlusion of the pulmonary artery of 5 cycles of 30 s ischemia and 30 s repeffusion may not have significant protective effect for lung oxygenation function injury induced by CPB in double valves replacement.
5.Extraction of total RNA and cloning of sgDHAR gene from Siraitia grosvenorii.

Abstract: Total RNA was isolated from Siraitia grosvenorii fruit by the method of modified Trizol, according to S. grosvenorii fruit characteristics of rich phenols, polysaccharide, oil and proteins. The OD260/280, OD260/230, RNA integrity (RIN) and yield of the total RNA with this method were 2.01, 2.02, 9.50 and 260 mirog.g-1, respectively. The open reading frame (ORF) of dehydroascorbate reductase (DHAR), named as SgDHAR, was cloned by rapid amplification of cDNA ends (RACE) and RT-PCR method from S. grosvenorii. The GenBank accession number for this gene is KC907731. The SgDHAR gene contains a full-length cDNA of 1,252 bp including ORF of 819 bp and encodes a predicted protein of 272 amino acids. The molecular mass is 30.217 7 kD and the isoelectric point is 8.76. Homology comparison showed that it shared 87% nucleotide sequence homology with Cucumis sativus. Expression patterns using qRT-PCR analysis showed that SgDHAR was mainly expressed in fruit and stem, followed by flower, and was lowest in root, while the expression level was 6.83 times in triploid. T than that in diploid. Therefore, SgDHAR gene may be involved in abortion of triploid seedless S. grosvenorii.

CONCLUSION: Cryopreserved allogenic osteochondral pillars transplantation can repair small full-thickness articular cartilage defects. The chondrocytes are alive in short time, and they can secret cartilage matrix without obvious rejection. It has similar efficacy in histology with autogenic osteochondral pillar transplantation.

OBJECTIVETo retrospectively analyze the clinicopathology of patients with gastric gastrointestinal stromal tumor(gGIST) who underwent radical excision within 18 years in 10 domestic medical centers in order to understand the status of domestic surgical treatment of gGIST.
METHODSClinicopathological data of gGIST patients undergoing radical excision in 10 medical centers from January 1998 to January 2016 were collected, and their operational conditions, postoperative adjuvant therapy, gene detection and survival were analyzed retrospectively.
RESULTSA total of 1 846 cases were recruited in this study, including 246 cases from Guangdong General Hospital, 331 cases from Sun Yat-sen University Cancer Center, 374 cases from Union Hospital, Tongji Medical College of Huazhong University of Science and Technology, 342 cases from Nanfang Hospital of Southern Medical University, 265 cases from Fujian Medical University Union Hospital, 148 cases from Fudan University Shanghai Cancer Center, 49 cases from West China Hospital of Sichuan University, 43 cases from Peking University Cancer Hospital and Institute, 28 cases from the 81st Hospital of Pepole's Liberation Army(PLA), 20 cases from Cancer Hospital of China Medical University, Liaoning Cancer Hospital and Institute. There were 918 male (49.7%) and 928 female patients (50.3%) with median onset age of 59(18 to 95) years old. Fundus(735 cases, 39.8%) and body (781 cases, 42.3%) of stomach were the common sites of lesions. The average size of tumor was (5.3±4.6) cm. There were 1 421 cases with mitotic count ≤5(77.0%). According to the operation procedure, 924 cases (50.1%) underwent laparoscopic surgery, 759 cases (41.1%) laparotomy, 120 cases (6.5%) endoscopic surgery, and 20 cases (1.1%) laparoscopic combined with endoscopic surgery, 6 cases (0.3%) laparoscopic excision surgery through gastric wall and cavity, and 17 cases (0.9%) laparoscopy and then were transferred to laparotomy. Wedge excision were performed in 1 308 cases (70.9%), proximal gastric excision in 226 cases(12.2%), distal gastric excision in 92 cases (5.0%), total gastrectomy in 94 cases (5.1%), and local gastrectomy in 126 cases(6.8%). Multi-visceral excision was performed in 138 cases, and the splenectomy was performed in 83 cases(60.1%)with the highest ratio. According to modified NIH classification, 399 cases(21.6%) were extreme low risk, 580 cases(31.4%) were low risk, 424 cases(23.0%) were moderate risk, 443 cases (24.0%) were high risk. A total of 461 cases received postoperative imatinib adjuvant therapy, accounting for 53.2%(461/867) of patients with moderate and high risk. Among 1 846 cases, 1 402 cases (75.9%) had complete follow-up data and the median follow-up time was 33.6 (0.1 to 158) months. The 5-year survival rates of extreme low risk, low risk, moderate risk and high risk were 100%, 98.5%, 92.5%, and 79.2% with significant difference(P=0.000).
CONCLUSIONSGastric GIST occurs mostly in fundus and body of stomach in China. Wedge excision is the main operational procedure and laparoscopic operation is over 50%. General prognosis of gastric GIST is quite good.
