1.Discussion on toxic pathogen

This paper mainly discusses that the theory of toxic pathogen is a significant part in nosazontology of TCM. The paper explains the meaning of toxic evil and argues toxic pathogen as an independent factor in TCM pathogen. Meanwhile, it discusses the characteristics of disease induced toxic pathogen from aspects of onset and symptoms and signs. Toxic evil includes biologic toxic evil, physical and chemical toxic evil and endogenous toxic evil. Toxic pathogen with some infectivity can cause disease intermixing with other six evils, and it can cause disease rapidly or become latent evil. The symptoms and signs are characterized by nontypical symptoms and inapparent yin-yang and cold-heat attribute in the initial stage. Toxic pathogen can cause patho-products as blood stasis, lump, and sputum. It can affect different zangfu according to different kinds of toxin. Diseases induced by toxic pathogen are obstinate and can cause extensive inner damage.
2.Expression of matrix metalloproteinases and tissue inhibitor of metalloproteinases in Mooren's ulcer cornea

Objective To investigate the expression of matrix metalloproteinases and tissue inhibitor of metalloproteinases in active Mooren's ulcer corneas.Methods An indirect immunofluorescent technique was used to visualize the presence of matrix metalloproteinase-1,2,3,9 and tissue inhibitor of metalloproteinase-1,2 in active Mooren's ulcer corneas from 6 patients and normal human corneas from 4 corpses.The results were further analyzed by an image-processing system.Results Only TIMP1,2 were detected in normal corneal epithelium while MMP-1,2,3,9,and TIMP-1,2 all were found in Mooren's ulcer corneas.MMP-2,TIMP-2 expressed in epithelium,MMP-3 expressed in stroma,MMP-1,9,and TIMP-1 expressed in both epithelium and stroma.Conclusion In active Mooren's ulcer cases,corneal lesion and corneal wound healing coexist,but the destructive reaction is greater than wound healing.Over-expression of MMP-9 and unbalanced regulation of MMPs/TIMPs may play an important role in the Mooren's ulcer of this stage.
3.Treatment of SIRT for unresectable hepatic metastasis from colorectal cancer

Colorectal cancer is the third high incidence of tumor in the world,and liver is one of the most common sites of distant metastasis.Liver metastasis accounts for 50%-75% in distant metastasis.Only about 10% -15% of patients are candidates for resection,and the recurrence is easy.Selective internal radiation therapy is a new type of radiotherapy.It is partial and liver-targeted.For unresectable colorectal liver metastasis,selective internal radiation therapy alone or in combination with other therapies can reduce cancer focus,prolong the survival,and maybe improve overall survival.
4.Mycophenolate mofetil (MMF) used for treating type Ⅳ lupus nephritis and literature review

Objective To observe about curative effect of MMF on type Ⅳ lupus nephritis and to review the literature about MMF used to treat LN.Method Ten patients with type Ⅳ LN were treated with MMF 1~2 g/d plus prednisone at middle or little dose.Curative duration of these patients was between 6 and 12 months.After 3,6 and 12 months curative effect was observed.Results After treatment proteinurie and systemic lupus activity measurement were decreased gradually.Before and after treatment ANA positive rates were 80% and 30%,anti dsDNA antibody positive rates were 50% and 10%,respectively,and low C3 and C4 was rectified.After treatment renal failure in 3 patients was recovered.During treatment major adverse reactions included gastrointestinal symptoms and herpes zoster.Conclusion MMF has exacttly effect on type Ⅳ LN and low side effect.
5.Clinical and pathologic features of lupus nephritis patients with positive MPO-ANCA: a report of 18 cases

Objective:To study the clinical and pathologic features of patients with lupus nephritis (LN) whose myeloperoxidase anti-neutrophil cytoplasmic antibody (MPO-ANCA) were positive. Methods:The clinical and pathological features were analyzed in 18 patients with LN whose MPO-ANCA were positive. And the data of patients with different clinical outcomes were compared. Results:(1)The hematological abnormalities, hypertension and serositis in these patients were more common than general ones with LN. (2)Proteinuria and hematuria were common, the morbidities of gross hematuria and renal failure in these patients were higher than general ones with LN.(3)Various autoantibodies were positive in these patients.(4)Segmental necrosis crescentic nephritis accompanied by density of immunocomplex in glomeruli and vasculitis in intestitium were common.(5)The morbidity of ESRF and mortality of these patients were similar to general ones with LN. The morbidity of tubular atrophy in those with poor prognosis was significantly higher than those survived. Conclusion:The patients with LN whose MPO-ANCA are positive have some difference from those with negative MPO-ANCA, but positive MPO-ANCA is not directly related to the prognosis.
6.Advance in neoadjuvant chemotherapy for colorectal liver metastasis

Neoadjuvant therapy has been gradually applied to the treatment of colorectal liver metastasis in recent years. Many clinic researches indicate neoadjuvent chemotherapy, neoadjuvent radiotherapy and neoadjuvent chemoradiotherapy can improve surgical reaction rate and radical rate of colorectal liver metastasis,and can shrink the primary tumor and metastasis, improve the resection rate and sphincter preservation rate, and reduce the recurrence rate of locally advanced lower rectal cancer. Neoadjuvent therapy has great clinical practical value in the multi-modality treatment for colorectal metastasis.
7.Expression of collagen type Ⅲ, laminin and fibronectin in Mooren's ulcer

Objective To compare the locations and levels of collagen type Ⅲ, laminin, and fibronectin in Mooren's ulcer and normal human corneas. Methods An indirect immunofluorescent technique and immunohistochemical staining were used to determine the distribution of collagen type Ⅲ, laminin, and fibronectin. The positive results were quantitatively analyzed with an image-processing system. Results Collagen type Ⅲ was not detected in the normal cornea. However, the staining could be seen in the corneal stroma close to Mooren's ulcer focus. Laminin was expressed faintly in the basement membrane of the normal cornea and the positive expression increased in the basement membrane and Bowman's membrane of Mooren's ulcer. In the normal cornea basement membrane, fibronectin was located continuously. In Mooren's ulcer basement membrane, however, fibronectin could not be found in all sections in which one expressed in epithelium and the others in stoma close to the ulcer focus. Conclusion Epithelial basement membrane may play a potential role in the pathogenesis of Mooren's ulcer.
8.Observations of the curative effect with various liquid for post operative irrigation of ESS of treating chronic sinusitis and nasal polyps

Objective:To explore the influence of liquid for post-operative irrigation on the effect of ESS.Method:192 cases with chronic sinusitis and/or nasal polyps of groupIIstageII who had undergone the ESS from September,1998 to June,1999 and completed a3-month follow-up were divided randomly into 3 groups to accept 3 different kinds of irrigation liquid.The evaluation of curative effect was according to the index of HaiKou ESS-97.Result:The curative effect of Herba Houttuyniae group was better than the other two.Conclusion:The post-operative care is important and the irrigation liquid can affect the effect of ESS.
9.Caspase-3 in cellular apoptosis of bladder carcinoma induced by mitomycin C

Objective To investigate caspase 3 in the apoptosis of EJ cells in bladder carcinoma induced by mitomycin C(MMC). Methods The apoptosis and changes in cell cycle were examined by means of TUNEL and flow cytometry. The ability of caspase 3 antibody to resist apoptosis induced by a low dose of mitomycin was also studied. 10.Nonmyeloablative allogeneic peripheral blood stem cell transplantation for chronic leukemia:a report of seven cases

Objective To summarize the experimence of nonmyeloablative allogeneic peripheral blood cell transplantation in the treatment of chronic leukemia. Methods Seven patients, including 6 cases of chronic myeloid leukemia (in chronic phase), one of chronic lymphoid leukemia (in third stage), with HLA-identical siblings donor received allogeneic peripheral blood stem cell transplantation after a nonmyeloablative conditioning. Results All of them were engrafted with donor cells (4 with full of donor cells grafted, 3 with mixed chimerism) and recovered hematopoiesis (WBC recovered to more than 0.5 ?10 9/L during postoperative 9 day to 21 day and platelet recovered to more than 30?10 9/L during postoperative 11 day to 28 day). One of them developed a GVHD of degree IV. One of them developed aGVHD of degree I. Conclusion This procedure is much safe, effective and of less complications than the myeloablative condioning regimens and may represent another new approach in the management of patients with chronic leukemia.
