1.Risk factors of multiple-drug-resistant bacterial infection in lower respiratory tract of patients with acute stroke

Objective To investigate the acute stroke patients with lower respiratory tract of multi drug resistant bacteria(MDR) infection risk factors.Methods The clinical data of 170 patients with acute stroke were retrospectively analyzed from January 2013 to December 2015.The MDR infection was divided into MDR infection group and non-MDR infection group.example.Univariate analysis and multivariate logistic regression analysis were used to analyze the risk factors of MDR infection in patients with acute stroke.Results Univariate analysis: The relationship between COPD, coma, cerebral hemorrhage, cough, cough reflex, tracheal intubation, indwelling catheter, indwelling gastric tube, and combination of two or more antibiotics was more than 5 days and MDR infection(P<0.05).Multivariate logistic regression analysis: COPD, coma, tracheal intubation, cerebral hemorrhage, two or more antibiotics prophylactic use is an independent risk factor for MDR infection.Conclusion The acute stroke patients with lower respiratory tract infection in patients with MDR factors and iatrogenic factors, combined with COPD, coma, tracheal intubation, cerebral hemorrhage, irrational use of antibiotics are independent risk factors for its occurrence.
2.Determination of Compound Proglumide Tablets by HPLC

Objective:To establish an HPLC method for the determination of compound proglumide tablets. Methods:A Diamon-sil C18(250 mm ×4.6 mm,5 μm)column was used. The mobile phase was 2% ammonium acetate-methanol (40∶60), the detection wavelength was 225nm, and an external standard method was employed. Results:The linear range of proglumide was 2. 60-208. 10 μg (r=0. 999 9), and the average recovery was 99. 8%(n=6,RSD=0. 6%). Conclusion: The HPLC determination method for com-pound proglumide tablets is accurate, simple and reproducible.

4.Differential Expression of Sox9 in Conventional Chondrosarcoma and Dedifferentiated Chondrosarcoma

Objective: To investigate the differential expression of Sox9 in conventional chondrosarcoma,dedifferentiated chondrosarcoma and normal cartilage. Methods: We reported 12 cases of chondrosarcomas,which were initially diagnosed as chondrosarcomas(6 cases of conventional chondrosarcoma and 6 cases of dedifferentiated chondrosarcoma)at Peking University People's Hospital between January 2003 and January 2007.We used genechip method to identify difierentially expressed genes involved in conventional chondrosarcoma,dedifferentiated chondrosarcoma and in normal cartilage(6 cases)and found thousands of differentially expressed genes after extensive statistical analysis.With Sox9 which played crucial roles in the process of both differentiation and maturation of chondrocyte as a candidate,we used Real-time PCR,Westem blot and immunohistochemistry to confirm the results found by gene chip. Results: DNA microarray results showed that Sox9 was up-regulated about 1.6 times in conventional chondrosarcoma compared with that in normal cartilage.But in dedifferentiated chondrosarcoma,the expression level of Sox9 was significantly down-regulated,0.082 times of that in normal cartilage.Real-time PCR results showed that the expression levels of Sox9 mRNA in conventional chondrosarcomas and dedifferentiated chondrosarcomas were 1.68±0.119 and 0.088±0.017,respectively.Sox9 protein level was significantly higher in humen conventional chondrosarcomas than that in normal cartilage.Sox9 protein level in dedifferentiated chondrosarcomas was significantly lower than that in normal cartilage tissue.All of the 6 cases of conventional chondrosarcomas showed diffuse and strong staining of Sox9.However,Only scattered staining was observed in dedifferentiated chondrosarcomas. Conclusion: Compared with that in normal cartilage,Sox9 expression is up-regulated in conventional chondrosarcomas and down-regulated in dedifferentiated chondrosarcomas.Decrease of Sox9 expression in dedifferentiated chondrosarcoma is correlated with poor survival,indicating that Sox9 may serve as a molecular prognostic marker for chondrosarcomas and disease progression.
5.CT findings of benign lymphoepithelial lesion of major salivary glands

Objective To analyze the CT findings of benign lymphoepithelial lesion (BLEL) of major salivary glands.Methods The CT findings of 11 cases with BLEL which were clinically and pathologically confirmed were analyzed.The relevant literatures were reviewed.All the patients received plain CT.Among them,5 patients had contrast CT as well.The location,number,shape,margin,density,and enhancement characteristics of the lesions were observed.Results Of 11 BLEL,8 lesions were located in the parotid gland,three in the submandibular gland.There were 7 cases with bilateral multiple lesions,and 4 cases with unilateral solitary lesion.BLEL of major salivary gland could be classified into three types according to the CT findings,including multinodular type,solitary mass type,and diffusely infiltrating type.Multinodular type was the most common type.The typical CT findings of the lesion included diffusely enlargement of bilateral parotids and (or) submandibular glands,heterogeneous increase of density of the parenchyma with multiple soft tissue density nodules,and multiple cystiform hypodense.On contrast-enhanced CT,the lesions mostly manifested as heterogeneous enhancement.The solid part of the nodules showed moderate or marked enhancement.However,the cystiform hypodense showed no enhancement.Conclusion Although the CT findings of BLEL of major salivary glands are variable,they still share some common characteristics,which are useful in the clinical diagnosis.

Objective To analyze the activity of the vastus medialis (VM) and vastus lateralis (VL) muscles in the dynamic stabilization of the patella in patients with patellofemoral pain (PFPS) using standard surface electromyographic (sEMG) tests.Methods This comparative study involved 7 patients with PFPS (13 knees) and 10 healthy subjects (20 knees).They performed weight-bearing squat movements,and sEMG was employed to measure the mean amplitude and the time before peak (TBP) of their VM and VL muscles.Results There was no significant difference between the two groups in the ratio of the mean amplitudes of the VM and VL muscles.However,the ratio in both groups in squatting was significantly higher than when simply standing.In the PFPS group the average TBP delay of the VM relative to the TBP of the VL was significantly longer than in the control group during squat movements.Conclusions The delay in the TBP of the VM relative to the VL during squat movements could serve as a standardized test for assessing treatment effect in patients with local factor PFPS.
9.Clinical significance of the expression levels of ERCC1, BRCA1 and TS in advanced colorectal cancer

Objective To explore the relationship between the expression levels of excision repair cross complementation group 1(ERCC1), breast cancer susceptibility gene 1(BRCA1), thymidylate synthase (TS) mRNA and clinicopathological features, prognosis in advanced colorectal cancer, and the correlation between the expression levels of ERCC1 and BRCA1. Methods The expression levels of ERCC1, BRCA1 and TS mRNA of postoperative paraffin embedded tissue were tested by real-time quantitative reverse transcription polymerase chain reaction (RT-PCR) in 49 advanced colorectal cancer cases. The results were analyzed by χ2 test of the correlation between the expression levels and clinicopathological characteristics. Patients were followed up by clinic or telephone. The prognosis was analyzed by small sample Kaplan-Meier survival analysis and Log-rank time series analysis, and P<0.05 was statistically significant. Results The expression level of ERCC1 mRNA in patients with colorectal carcinoma had no obvious correlation with the clinical and pathological characteristics such as gender, age, tumor location, lymph node metastasis, distant metastasis, CEA and differentiation degree (P>0.05). The expression level of BRCA1 mRNA had no significant correlation with the above clinical and pathological features (P>0.05) except distant metastasis (P=0.030) and differentiation degree (P=0.002). The expression level of TS mRNA had no significant correlation with the above clinical and pathological features (P>0.05) except distant metastasis (P=0.003). The expression level of ERCC1 and BRCA1 mRNA obviously correlated (P=0.002). The 1 year overall survival rate was 95.92%(47/49);the 2 year overall survival rate was 83.67%(41/49);and the 3 year overall survival rate was 73.47%(36/49). Overall survival and progression-free survival time in ERCC1 mRNA low expression group (47.8, 41.0 months) was higher than that in ERCC1 mRNA low expression group (27.3, 20.0 months) respectively (P=0.001, P=0.001). Overall survival and progression-free survival time in BRCA1 mRNA low expression group (43.7, 42.7 months) was higher than that in BRCA1 mRNA high expression group (29.3, 25.1 months) respectively (P=0.009, 0.006). Overall survival time in TS mRNA low expression group (39.8 months) was higher than that in BRCA1 mRNA high expression group (25.2 months). Conclusions The expression level of ERCC1 mRNA is not correlated with its clinical and pathological characteristics, but with its biological characteristics. BRCA1 and TS levels are correlated with invasion and metastasis. Low levels of ERCC1 and BRCA1 expression have a better prognostic effect on platinum based first-line chemotherapy for advanced colorectal cancer, and they are correlated. Low level of TS also has longer disease-free survival. Three joint detection could be used as a prognostic factor for colorectal cancer chemotherapy.
10.Secondary lymphoid-tissue chemokine and CCR7 in it's relation with lymphangiogenesis of pancreatic adenocarcionoma

Objective To study the expression of secondary lymphoid-tissue chemokine (SLC)、 CCR7 and its correlation with clinical pathology and lymphangiogenesis in pancreatic adenocarcinoma (PAC). Methods The tissue specimens including PAC, the cancerous peripheral tissues, the normal pancreatic tissues and peripheral lymph nodes were obtained from 30 patients with PAC. The expressions of SLC and CCR7 in these tissues were assayed by immunohistochemical staining and reverse transcription polymerase chain reaction (RT-PCR). MIND marked by VEGFR-3 was detected by morphometric analysis, and the relationship between MLND and clinical pathology of PAC was analyzed. Results In all the specimens, the positive rates of SLC protein in PAC, the cancerous peripheral tissues, the normal pancreatic tissues and peripheral lymph nodes were respectively 16. 7%, 43. 3%, 76. 7% and 46. 6%. The positive rates of CCR7 protein in PAC, the cancerous peripheral tissues, the normal pancreatic tissues and peripheral lymph nodes were respectively 76. 7%, 66. 7%, 30. 0% and 70. 0%. The results of RT-PCR and fluorescence quantitative real-time PCR indicated that the expression levels of CCR7 mRNA in PAC tissues, the cancerous peripheral tissues and peripheral lymph nodes were higher than that in the normal pancreatic tissues ( P <0. 01 ). There was no significant correlation between the expression of SLC protein with MLVD of PAC ( P > 0. 05 ). There was 23 specimens that the CCR7 protein was positive, and among these specimens the MIND was higher than that in negative group of CCR7 protein (P = 0.004). Conclusions The expression of SLC was not related to lymphatic metastasis and TNM stages of PAC. The expression of CCR7 was significantly associated with lymphatic metastasis and TNM stages of PAC, and the high expression of CCR7 in PAC tissues was significantly associated with lymphangiogenesis of PAC.
