1.THE EFFECTS OF GEKKO GECKO L.ON THE METABOLISM OF FREE RADICAL IN THE RAT GUT

The effects of alcoholic extract of Gekko gecko L. on the activities of SOL), GSH-Px, CAT and the level of GSH and LPO in the rat gut were investigated. The data presented in this paper indicated that the clcrance of free radical was accelerated, LPO was decreased, the activities of SOD, GSH-Px and CAT was elevated, and, the content of GSH was increased. The extract from the tail part of the Gekko gecko L. was more active than the extract from the body. These results and our other results together indicate that Gekko gecko L. has an obvious antiaging effect
2.The CT feature in the cerebral ischemia apoplexy of type 2 diabetes mellitus

Objective To study the CT features and clinical value of cerebral ischemia infarction. Methods the CT feature of 106 diabetes ischemia infarction were analyzed. Results The patients with is-chemia brain white matter change (35 cases),small area cerebral ischemia infarction (85 cases),great area cerebral ischemia infarction ( 12 cases ),cerebral hemorrhage (6 cases). Conclusions Cerebral ischemia infarction of diabetes mellitus mainly as small area multiple cerebral ischemia infarction,ischemia cerebral white matter lesion located at the area of base ganglion thalamencephalon and cerebellum,termly CT cerebral examination can diagnosis and instruct treatment to the complication of cerebral ischemia apoplexy of type 2 diabetes mellitus.
3.Analysis on the allergens in allergic rhinitis patients in Changzhou district of Jiangsu.

OBJECTIVE:
To investigate the distribution of positive allergens in patients with allergic rhinitis in Changzhou.
METHOD:
A total of 4242 cases with allergic symptoms and signs underwent skin prick test with 16 kinds of standardized allergens. The common allergens were found out. The samples were stratified on seven age groups to analysis the distributive characteristics of allergens responsible for each group. The samples were also stratified on children group and adults group to analysis their response degree to dust mite.
RESULT:
2638 cases (62.19%) had the positive reaction, including male 1380 (52.3%) cases, female 1258 (47.7%) cases. Dermatophagoides farinae and dermatophagoides pteronyssinus had the highest positive ratio (85.86%, 83.55%) in inhalation group, followed cockroach (11.49%) and Humulus L. (9.82%), fungus (2.92%) in the lower positive ratio. Shrimp (5.76%) was the most common allergen in food group. The distribution of common allergens in different age groups was variable. There was significantly higher sensitivity to dust mite between children group compared to adults group (P < 0.01).
CONCLUSION
The study shows that Dust mite was the mostly responsible common allergen in Changzhou district. The distribution of common allergens in children group and adults group was variable.

Objective:To compare the efficacy and safety of Changsulin ? with Lantus ? in treating patients with type 2 diabetes mellitus (T2DM). Methods:This was a phase Ⅲ, multicenter, randomized, open-label, parallel-group, active-controlled clinical trial. A total of 578 participants with T2DM inadequately controlled on oral hypoglycemic agents were randomized 3∶1 to Changsulin ? or Lantus ? treatment for 24 weeks. The efficacy measures included changes in glycosylated hemoglobin (HbA1c), fasting plasma glucose (FPG), 2h postprandial plasma glucose (2hPG), 8-point self-monitoring of blood glucose (SMBG) profiles from baseline, and proportions of subjects achieving targets of HbA1c and FPG. The safety outcomes included rates of hypoglycemia, adverse events (AEs) and anti-insulin glargine antibody. Results:After 24 weeks of treatment, mean HbAlc decreased 1.16% and 1.25%, FPG decreased 3.05 mmol/L and 2.90 mmol/L, 2hPG decreased 2.49 mmol/L and 2.38 mmol/L in Changsulin ? and in Lantus ?, respectively. No significant differences could be viewed in above parameters between the two groups (all P>0.05). There were also no significant differences between Changsulin ? and Lantus ? in 8-point SMBG profiles from baseline and proportions of subjects achieving the targets of HbA1c and FPG (all P>0.05). The rates of total hypoglycemia (38.00% and 39.01% for Changsulin ? and Lantus ?, respectively) and nocturnal hypoglycemia (17.25% and 16.31% for Changsulin ? and Lantus ?, respectively) were similar between the two groups (all P>0.05). Most of the hypoglycemia events were asymptomatic, and no severe hypoglycemia were found in both groups. No differences were observed in rates of AEs (61.77% vs.52.48%) and anti-insulin glargine antibody (after 24 weeks of treatment, 6.91% vs.3.65%) between the two groups (all P>0.05). Conclusions:Changsulin ? shows similar efficacy and safety profiles compared with Lantus ? and Changsulin ? treatment was well tolerated in patients with T2DM.
5.Value of modified gastroscopic laryngeal mask airway for airway management under general anesthesia in patients with liver cirrhosis undergoing endoscopic esophageal variceal ligation

Objective:To evaluate the value of modified gastroscopic laryngeal mask airway (LMA) for airway management under general anesthesia in the patients with liver cirrhosis undergoing endoscopic esophageal variceal ligation (EVL).Methods:Sixty-two American Society of Anesthesiologists physical status Ⅱor Ⅲ patients with liver cirrhosis of either sex, aged 25-64 yr, with body mass index of 18-30 kg/m 2, undergoing endoscopic EVL with general anesthesia, were divided into 2 groups ( n=31 each) using a random number table method: tracheal tube group (group T) and modified gastroscopic LMA group (group L). After induction of anesthesia, a tracheal tube was inserted in group T, and a modified gastroscopic LMA was inserted and the patients were mechanically ventilated to maintain P ETCO 2 at 30-40 mmHg in group L. Successful tracheal intubation or insertion of modified gastroscopic LMA, successful ligator insertion and duration of ligator insertion were recorded.The occurrence of intraoperative hypotension, bradycardia and hypoxemia and consumption of propofol and remifentanil were recorded.At 1 min after tracheal intubation or right placement of LMA (T 1), immediately after the ligator insertion (T 2) and at the end of operation (T 3), Peak (P peak) and mean (P mean) airway pressure were monitored, and airway leak pressure in group L was measured.The extubation time, duration of post-anesthesia care unit stay and bucking during extubation were recorded.The occurrence of sore throat and nausea and vomiting was recorded within 6 h after operation.Postoperative satisfaction scores of endoscopists and patients were also recorded. Results:Sixty-one patients were finally enrolled in the study, with 31 in group T and 30 in group L. Compared with group T, no significant changes were found in the success rate of tracheal intubation or LMA placement, success rate of ligator insertion, P mean at each time point, incidence of postoperative nausea and vomiting, and postoperative satisfaction score of endoscopists ( P>0.05), duration of the ligator insertion was significantly shortened, P peak at each time point was decreased, the incidence of intraoperative hypotension and bradycardia was decreased, the consumption of propofol and remifentanil was reduced, the incidence of bucking during extubation and postoperative sore throat was reduced, and extubation time and duration of post-anesthesia care unit stay was shortened, and satisfaction scores of patients were increased in group L ( P<0.05). No hypoxemia was found in two groups.Airway leak pressure was maintained at 24-26 cmH 2O at each time point in group L. Conclusions:Modified gastroscopic LMA can be safely and effectively used for airway management under general anesthesia in the patients with liver cirrhosis undergoing EVL.
6. Comparison of two imaging diagnosis methods in the diagnosis of pulmonary thesaurosis induced by dust of iron and its compounds

OBJECTIVE: To compare the diagnostic value of high-k V X-ray chest photography and chest high-resolution computed tomography( HRCT) in the diagnosis of pulmonary thesaurosis induced by dust of iron and its compounds(hereinafter called the iron pulmonary thesaurosis). METHODS: A total of 80 workers exposed to iron oxide dust in a magnetic material factory were collected as the study subjects by convenience sampling method,and they were examined with high-k V X-ray chest photography and HRCT. Differences between the two methods in the diagnosis of iron pulmonary thesaurosis were compared. RESULTS: Of the 80 workers,only 5 of them(6. 2%) showed no abnormal changes in chest HRCT,others showed varying degrees of diffuse distribution of air-cavity nodules and ground-glass lesions. High-k V X-ray chest photography and chest HRCT diagnosis of iron pulmonary thesaurosis accounted for 8. 8%( 7/80) and 37. 5%(30/80) with extreme mild degree,and 21. 3%(17/80) and 26. 3%(21/80) with mild degree respectively. The diagnostic rates for iron pulmonary thesaurosis were 30. 0%( 24/80) and 63. 8%( 51/80),respectively. There was consistency between the two methods for the diagnosis of iron pneumoconiosis( Kappa = 0. 411,P < 0. 01). The chest HRCT has a higher diagnostic classification and diagnosis rate compared with the high-k V X-ray chest photography( P <0. 01). CONCLUSION:s The chest HRCT has a higher diagnostic grade and higher diagnostic rate for lung siderosis compared with the high-k V X-ray chest photography,which is helpful for the early diagnosis of the disease.

Since December 2019, novel coronavirus pneumonia (NCP) has been reported in Wuhan, Hubei Province, and spreads rapidly to all through Hubei Province and even to the whole country. The virus is 2019 novel coronavirus (2019-nCoV), never been seen previously in human, but all the population is generally susceptible. The virus spreads through many ways and is highly infectious, which brings great difficulties to the prevention and control of NCP. Based on the needs of orthopedic trauma patients for emergency surgery and review of the latest NCP diagnosis and treatment strategy and the latest principles and principles of evidence-based medicine in traumatic orthopedics, the authors put forward this expert consensus to systematically standardize the clinical pathway and protective measures of emergency surgery for orthopedic trauma patients during prevention and control of NCP and provide reference for the emergency surgical treatment of orthopedic trauma patients in hospitals at all levels.
