1.The effect of rehabilitation training on muscle strength and exercise tolerance in hemodialysis patients with muscle atrophy

Objective To evaluate the effects of rehabilitation training on muscle strength and exercise toleranee in hemodialysis patients with muscle atrophy.Methods Nine hemodialysis patients with muscle atrophy because of end renal failure were recruited in this study. A structured exercise program(90 minutes a sedssion.3 sessions a week)was administered to all the subjects for 6 month.Immediately before and at the end of the exercise programme,the muscle strength of the lower limbs,the motor conduction velocity of the peroneal nerve and maximal oxygen consumption of the patients were examined. Results It was shown that all the patients had impaired exercise capacity,weakend muscle strength and slowed nerve conduction velocity before rehabilitation training.After the exercise programme,the patients' exercise capacity as reflected by the maximal oxygen consumption and exercise time was significantly increased.The muscle strength and the motor nerve conduction velocity were significantly increased.Conclusions Muscle atrophy in hemodialysis patients results in poor exercise tolerance, but rehabilitation exercise programme improves amyotrophy and therefore has beneficial effects on the patient's overall work performance.
2.Risk factors for infection in patients with permanent dual hemodialysis catheters

Clinical data of 116 patients with implanted permanent dual catheters for hemodialysis,including 18 with infection and 98 non-infection, during January 2006 and July 2009 were retrospectively analyzed to study risk factors for catheter-related bacteremia (CRB). Duration of catheter implantation,primary disease, routine blood examinations and blood biochemical examination of the patients were analyzed between the two groups. COX proportional hazard regression analysis was performed for all predictor variables. Results showed that overall incidence of bacteremic episodes was 0. 314 per 1000 catheter-day.Compared to that in infection group, levels of hemoglobin, plasma albumin, peripheral lymphocyte count and ratio of CD4/CD8 in non-infection group were significantly higher ( all P < 0. 05 ), and OR of CRB were 4. 011 (P =0. 0213) for diabetes mellitus and 7. 181 for hemoglobin level less than 80g/L (P = 0. 0020),respectively. It is suggested that improving nutrition status and correcting anemia for patients with hemodialysis are necessary to reduce CRB.
3.Effect of curcumin on expression of p-ERK and p-CREB in spinal dorsal horn and dorsal root ganglion in type 2 diabetic neuropathic pain in rats

Objective To evaluate the effects of curcumin on the expression of phosphorylated extracellular signal-related kinase (p-ERK) and phosphorylated cAMP response element binding protein (p-CREB) in the spinal dorsal horn and dorsal root ganglion (DRG) in type 2 diabetic neuropathic pain (DNP) in rats.Methods Type 2 diabetes mellitus was induced by high-fat and high-sucrose diet and intraperitoneal streptozotocin (STZ) 35mg/kg,and confirmed by fasting blood glucose level≥ 16.7 mmol/L in male Sprague-Dawley rats.Type 2 DNP was confirmed by the mechanical withdrawal threshold (MWT) and thermal withdraw latency (TWL) measured on day 14 after STZ administration < 80% of the baseline value,and the rats with type 2 DNP were randomly divided into 3 groups (n =27 each):type 2 DNP group (group DNP),curcumin group (group Cur) and solvent control group (group SC).Curcumin and corn oil 100 mg/kg (25 mg/ml) were injected intraperitoneally once a day for 14consecutive days starting from 14 days after administration of streptozocin in Cur and SC groups,respectively.Another 27 normal rats were served as control group (group C) and were fed with common forage.MWT and TWL were measured at 3,7 and 14 days after curcumin injection (T1 3),and the lumbar segment 4-6 of the spinal cord and DRGs were removed at the same time for determination of the expression of p-ERK and p-CREB (by Western blot).Results Compared with group C,MWT was significantly decreased,TWL was shortened,and the expression of p-ERK and p-CREB in spinal dorsal horn and DRGs was up-regulated at T1-3 in DNP and SC groups,and at T1 in Cur group (P < 0.05).Compared with group DNP,MWT was significantly increased,TWL was prolonged,and the expression of p-ERK and p-CREB in spinal dorsal horn and DRGs was down-regulated at T2,3 in Cur group (P <0.05).There was no significant difference in the MWT,TWL and expression of p-ERK and p-CREB between DNP and SC groups (P > 0.05).Conclusion Curcumin can attenuate type 2 diabetic DNP by inhibiting up-regulation of the expression of p-ERK and p-CREB in the spinal dorsal horn and DRG in rats.
4.Expression and clinical significance of C-erbB-2 in colorectal carcinoma

Objective To investigate the expression of oncogene C-erbB-2 and its significance in colorectal carcinoma. Methods Immunohistological EnVision staining was used to detect the expression of C-erbB-2 in 171 colorectal carcinoma, and 15 normal colorectal tissues were taken as controls. Results The positive percentages of C-erbB-2 in colorectal carcinoma and normal colorectal tissues were 23.4 % (40/171)and 0, which was significantly different (P <0.05). Only 5 samples showed strong positive expression (++ and +++) on epicyte, no relationship was found between C-erbB-2 expression and patients' gender, tumor differentiation, tumor stage, vascular and lymph node involvement, etc(P >0.05). The five year survival rate were 69.5 % (91/131) and 65.0 % (26/40) of C-erbB-2 negative and positive expression, the difference was not statistical (P >0.05). Conclusion There was on significant association between C-erbB-2 expression and clinicopathologic characteristics of colorectal carcinoma. C-erbB-2 did not present prognostic value in colorectal carcinoma.

6.Endovascular treatment for filter-related long segment obstruction of IVC and iliac vein: preliminary results in 8 patients

Objective To evaluate the feasibility and safety of endovascular intervention for the treatment of filter-related long segment obstruction of IVC and iliac vein.Methods From January 2014 to October 2016,a total of 8 patients with filter-related chronic long segment obstruction of IVC and iliac vein were treated with percutaneous transcatheter angioplasty (PTA) and stent implantation.The clinical data and imaging materials were collected.The endovascular interventional treatment methods were discussed,and the technical success rate,procedure-related complications and follow-up results were analyzed.Results Successful recanalization of the obstructed IVC-iliac vein segment was achieved in all 8 patients.In 7 patients,angiography performed immediately after PTA and stent implantation showed that the blood flow in the stent was smooth and the collateral vessels were decreased.In one patient,acute in-stent thrombosis occurred after PTA and stent implantation,then catheter-directed thrombolysis had to be adopted,and angiography performed 2 days later indicated that in-stent thrombosis was completely dissolved and the blood flow resumed smoothly.The patients were followed up for 2-13 months,with a mean of (4.7±3.9) months.Reexamination with both CT and venography of lower limb showed that the stent was kept in good position to the last visit in all 8 patients.Slight intimal hyperplasia was observed in 3 patients,but the blood flow was unobstructed.Conclusion Long term implantation of filter may cause filter-related chronic long segment obstruction of IVC and iliac vein.PTA with stent implantation is a safe and feasible method to effectively open the occluded vessels.
7.Laparoscopic simple prostatectomy for large volume benign prostate hyperplasia: a report of 30 cases

【Objective】 To investigate the feasibility of laparoscopic simple prostatectomy (LSP) in the treatment of large volume benign prostate hyperplasia (BPH). 【Methods】 Clinical and follow-up data of 30 patients with large volume BPH treated with LSP in our hospital during Feb.2019 and Dec.2021 were retrospectively analyzed. All patients underwent extraperitoneal LSP operation. The perioperative and 1-12 month postoperative follow-up data were analyzed. 【Results】 The average prostate volume was (92.4±38.9) mL, operation time (125±45) min, and weight of resected prostate (60.25±16.90) g. The hemoglobin decreased by (12.21±7.25) g/d after operation. No blood transfusion was needed. There was no need for bladder irrigation after operation in 21 cases (70%), and 9 cases (30%) had bladder irrigation time of (0.95±0.49) d. The postoperative catheter indwelling time was (6.92±2.51) d, and hospital stay (5.36±1.63) d. During the follow-up of (9.25±5.4) months, there was 1 case of postoperative intestinal obstruction (Clavien-Dindo grade II), 1 case of transient urinary incontinence (Clavien-Dindo grade I), and 1 case of delayed hematuria (Clavien-Dindo grade I). No urethral stricture occurred. The maximum urinary flow rate (Qmax), post-void residual urine volume (PVR), International Prostate Symptom Score (IPSS) and quality of life (QoL) 3 months after operation were significantly improved compared with those before operation (P<0.05). There was no significant difference in sexual function before and after operation (P>0.05). 【Conclusion】 LSP is safe and effective in the treatment of large volume BPH. It has advantages of complete resection of glands, minor bleeding and short postoperative bladder irrigation time. However, it still needs to be confirmed by a prospective control study of large samples.
