1.Low-density extracorporeal shock wave and low-dose intermittent recombinant human parathyroid hormone 1-34 influence proliferation and differentiation of osteoblasts

BACKGROUND:Suitable stress signals such as extracorporeal shock wave (ESW) and parathyroid hormone can be involved in regulation of bone formation and metabolism.
OBJECTIVE:To investigate the effects of low-density ESW and low-dose intermittent recombinant human parathyroid hormone 1-34 (rhPTH1-34) on the proliferation and differentiation of osteoblasts from rats cultured in vitro.
METHODS:Osteoblasts from the skul of neonatal rats were cultured using col agenase digestion method. After 60-150 times of stimulations of 0.18 mJ/mm2 ESW, different patterns of rhPTH1-34 at 10-12-10-10 mol/L and ESW+intermittent rhPTH1-34 (10-11 mol/L), osteoblasts were col ected to observe cel proliferation by cel counting, MTT and flow cytometry analysis and to observe cel differentiation by measuring alkaline phosphatase and type I col agen expression.
RESULTS AND CONCLUSION:0.18 mJ/mm 2 ESW (60-150 times), intermittent rhPTH1-34 (10-11 and 10-10 mol/L) and ESW+intermittent rhPTH1-34 (10-11 mol/L) stimulations could al improve proliferation and differentiation of rat osteoblasts cultured in vitro (P<0.05). ESW (60-150 times)+intermittent rhPTH1-34 stimulation showed better effects than ESW and intermittent rhPTH1-34 alone (P<0.05). These findings indicate that the combination of suitable ESW stimulation and low-dose intermittent rhPTH1-34 stimulation can improve the proliferation and differentiation of rat osteoblasts significantly.
2.The Renoprotective Mechanism of Mobic in Experimental Type 2 Diabetic Rat

Objective To observe the renal expression of cyclooxygenase-2 (COX-2) in rats with type 2 diabetes, and explore the effect of selective COX-2 inhibitor Mobic on the expression of renal COX-2, matrix metalloproteinase-9(MMP-9), tissue inhibitor of matrix metalloproteinase-1(TIMP-1), TXB 2 and 6-Ket-PGF1?, as well as renal structure and function. Methods All rats were divided into control group, diabetes mellitus group and treatment group. Type 2 diabetic rats were treated with Mobic and vehicle respectively. Immunohistochemistry was used to detect the expression of COX-2,MMP-9 and TIMP-1 in renal tissues. The urinary TXB 2 and 6-Ket-PGF1? concentration was determined by radioimmunoassay at 6th week. Results There were an increasing expression of COX-2, TIMP-1 and decreasing MMP-9 expression in the renal tissue of type 2 diabetic rats. Mobic could increase MMP-9 expression and depress TIMP-1 expression througth inhibiting the expression of COX-2 in the renal tissues of type 2 diabetic rats. Conclusion COX-2 was involved in the pathogenesis of type 2 diabetic nephropathy. Selective COX-2 inhibitor Mobic might exert its renoprotective effects through inhibiting COX-2 activity, decreasing prostagladins systhesis, and modulating MMP-9 and TIMP-1 expression.

6.Median effective concentration of remifentanil inhibiting responses to tracheal extubation in patients of Uygur nationality

Objective To determine the median effective concentration (EC50) of remifentanil inhibiting responses to tracheal extubation in patients of Uygur nationality.Methods The patients of Uygur nationality,aged 18-60 yr,of ASA physical status Ⅰ or [Ⅱ,scheduled for elective surgery,were enrolled in this study.After the end of surgery,remifentanil was given by target-controlled infusion until tracheal extubation.The initial target plasma concentration (Cp) of remifentanil was 2.0 ng/ml.The response was defined as positive when MAP and/or HR increased by > 15% of the level at the end of operation and the duration > 15 s during extubation.Each time the target Cp increased/decreased if the cardiovascular response was positive or negative.The ratio between the two successive concentrations was 1.1.The EC50 and 95 % confidence interval of remifentanil blunting responses to extubation was calculated by Probit method.Results Twenty-eight patients completed the study.The EC50 and 95 % confidence interval of remifentanil required for inhibiting the responses to extubation was 1.75 ng/ml and 1.45-2.01 ng/ml.Conclusion The EC50 of remifentanil inhibiting the responses to tracheal extubation is 1.75 ng/ ml in patients of Uygur nationality.

Objective To determine the median effective target effect-site concentration (EC50) of sufentanil inhibiting cardiovascular response to tracheal intubation when combined with propofol in patients of Uygur nationality.Methods Thirty-one ASA Ⅰ or Ⅱ Uighurs of both sexes,aged 21-59 yr,with body mass index 18-28 kg/m2,undergoing elective surgery,were enrolled in this study.Anesthesia was induced and maintained with propofol and sufentanil target-controlled infusion and iv injection of cisatracurium 0.2 mg/kg.The target effect-site concentration (Ce) of propofol was set at 3.0 μg/ml.Tracheal intubation was performed after the target Ce and plasma concentrations were balanced.The target Ce was set at 0.8 ng/ml in the first patient.Each time Ce increased/decreased by 10% in the next patient depending on whether or not the cardiovascular response to tracheal intubation occurred.The positive cardiovascular response was defined as increase in systolic blood pressure by 15% and/or HR> 90 bpm lasting for > 15 s.The EC50(95% confidence interval) of sufentanil blunting cardiovascular responses to trancheal intubation was calculated by Probit analysis.Results EC50 (95 % confidence interval) of sufentanil inhibiting cardiovascular response to tracheal intubation when combined with 3.0 μg/ml propofol was 0.46 (0.43-0.49) ng/ml.Conclusion EC50 of sufentanil inhibiting cardiovascular response to tracheal intubation is 0.46 ng/ml in patients of Uygur nationality when combined with propofol.
8.Expression and clinical significance of C-erbB-2 in colorectal carcinoma

Objective To investigate the expression of oncogene C-erbB-2 and its significance in colorectal carcinoma. Methods Immunohistological EnVision staining was used to detect the expression of C-erbB-2 in 171 colorectal carcinoma, and 15 normal colorectal tissues were taken as controls. Results The positive percentages of C-erbB-2 in colorectal carcinoma and normal colorectal tissues were 23.4 % (40/171)and 0, which was significantly different (P <0.05). Only 5 samples showed strong positive expression (++ and +++) on epicyte, no relationship was found between C-erbB-2 expression and patients' gender, tumor differentiation, tumor stage, vascular and lymph node involvement, etc(P >0.05). The five year survival rate were 69.5 % (91/131) and 65.0 % (26/40) of C-erbB-2 negative and positive expression, the difference was not statistical (P >0.05). Conclusion There was on significant association between C-erbB-2 expression and clinicopathologic characteristics of colorectal carcinoma. C-erbB-2 did not present prognostic value in colorectal carcinoma.
9.Whole brain diffusion tensor imaging in diagnosing social anxiety disorder based on support vector machine

Objective To evaluate the clinical value of whole-brain diffusion tensor imaging(DTI) in diagnosing patients with social anxiety disorder(SAD) using an automated method based on support vector machine(SVM) classification.Methods Whole brain DTI data were collected from 19 patients with SAD and 19 age-,gender-and education-matched healthy control(HC) subjects.Fractional anisotropy(FA) of whole brain was obtained by input all tensor images into Diffusion Toolkit software.Based upon the characteristics of brain FA,the pattern recognition of brain image data(PROBID) toolbox on the grounds of SVM algorithm was employed to classify the subjects,evaluate the diagnostic value of whole-brain FA data based SVM in diagnosing SAD patients and verify the robustness of the diagnostic results using permutation test with the threshold at P≤0.001.The weight vector score of each voxel was calculated according to the ratio between this voxel and whole brain in FA differences of the two groups.The white matter regions identified by setting the threshold to the top 30% of the weight vector scores with at least 10 contiguous voxels were demonstrated by MRIcro software.Results Diagnostic accuracy of whole-brain FA based SVM in diagnosing SAD was 92.11% (35/38) in which the specificity was 94.44% (17/18),the sensitivity was 90.00%(18/20),the positive likelihood ratio was 17.01,the negative likelihood ratio was 0.11 and the diagnostic index was 184.22%.Permutation test suggested that the diagnostic results were significantly reliable.White matter regions showing major contributions favoring SAD over HC were located in the genu and splenium of the corpus callosum,the left uncinate fasciculus,the left inferior longitudinal fasciculus,the left inferior fronto-occipital fasciculus,bilateral frontal gyri and the left occipital lobe.Whereas,white matter in bilateral anterior cingula,the left middle cerebellar peduncle and the left inferior parietal lobule showed more contributions to diagnose HC than to diagnose SAD.Conclusions As whole brain FA data based on SVM showing a high accuracy in diagnosing SAD,brain DTI characteristics have the potential to be the specific indicators in the diagnosis of SAD.SVM might be used as a tool to verify the reliability of white matter abnormalities and provide regions of interest in DTI study of neurological and psychiatric diseases.
10.The effects of total hip replacement and dynamic hip screw to intertrochanteric fracture in the geriatric with serious osteoporosis

Objective To investigate the effects of total hip replacement (THR) and dynamic hip screw (DHS) to intertrochanteric fracture in the geriatric with serious osteoporosis. Methods The clinical data of 51 senile cases (age over 60 years) with serious osteoporosis (Singh index less than grade Ⅲ ) cured from January 2003 to March 2008 were retrospectively reviewed. Two different groups were randomly selected from all cases treated with THR and DHS. Nineteen cases of THR were selected in THR group, modified Gibson approach was taken to complete hip joint replacement Thirty-two cases of DHS were selected in DHS group,lateral hip approach was employed to complete DHS fixation. Mobilization by CPM from the 2nd day and weight burdening from 3rd to 7th day according to general physical condition after surgery were carried out in THR group, mobilization and weight bearing start according to different conditions from the 2nd to 10th week in DHS group. Results Among 51 cases, 47 cases had got follow-up for 5.0 -40.5 (28.1 ± 11.9) months. Nineteen cases in THR group had got follow-up, Harris hip score of 18 cases (94.7% ) were good or excellent, and the other 1 case was bad. Twenty-eight cases in DHS group had got follow-up. Harris hip score of 23 cases (82.1% ) were good or excellent, and the other 5 cases were bad. There was statistical significance between the two groups (P < 0.05 ) . The average periods to start partly weight bearing of THR group and DHS group were( 8.3 ±4.2) days and (43.6 ± 15.3) days respectively, there was statistical significance between the two groups (P< 0.05). Conclusion To the geriatric intertrochanteric fracture with serious osteoporosis, THR is a better choice than DHS, because THR has shorter recovery period before full weight bearing, higher hip functional outcome evaluated by Harris hip score, earlier recovery of mobilization and less complications related with long term bed-bound .
