1.Effect of dexmedetomidine on tracheal extubation response in patients undergoing thyroid surgery after general anesthesia

ObjectiveTo investigate the effect of dexmedetomidine on tracheal extubation response in patients undergoing thyroid surgery after general anesthesia.Methods Forty-two patients who ASA classification Ⅰ - Ⅱ,were performed thyroid surgery under total intravenous anesthesia.They were divided into dexmedetomidine group and control group by random number table with 21 cases each.Both groups patients anesthesia induction and maintenance with the same methods,dexmedetomidine group patients at 15 min before anesthesia induction with venous infusion dexmedetomidine 0.6 μ g/kg(finished 10 min infusion),followed by 0.4 μ g/(kg · h) continuous infusion,control group patients with pump equivalent in 0.9% sodium chloride injection in the same way.Systolic blood pressure (SBP),diastolic blood pressure (DBP) and heart rate (HR),surgery time,recovery time,extubation time,orientation recovery time,modified Aldrete score ≥9 scores time,rate of recovery from restlessness,tolerance score and recovery from any adverse effects were recorded.ResultsThere were no significant differences between two groups in surgery time,recovery time,extubation time,orientation recovery time,modified Aldrete score ≥ 9 scores time (P >0.05).SBP,DBP,HR at immediately and after extubation with 1,3,5 min of dexmedetomidine group were lower than those of control group(P < 0.05 or < 0.01 ).Tolerance score excellent rate of dexmedetomidine group was higher than that of control group[95.2% (20/21) vs.28.6% (6/21)] (P < 0.05),the rate of recovery from restlessness of dexmedetomidine group was lower than that of control group[0 vs.28.6%(6/21 ) ] (P < 0.05).ConclusionBefore anesthesia induction in patients with venous infusion dexmedetomidine 0.6 p g/kg(finished 10 min infusion),followed by 0.4 p g/(kg·h) continuous infusion can effectively reduce the patients' cardiovascular response to extubation,strengthen the patients' tolerance to endotracheal tube,reduce the patients' agitation and recover consciousness comfortable.
2.Clinical analysis of 3D printed total hip arthroplasty in elderly patients with acetabular fracture

Objective:To study the clinical efficacy of 3D total hip arthroplasty in elderly patients with acetabular fracture.Methods:A total of 43 patients with non-obsolete acetabular fractures admitted to Xining First People's Hospital from May 2016 to December 2018 were randomly divided into the control group (22 cases) and the observation group (21 cases) for a perspective study. The control group received routine surgery and the observation group was treated with total hip arthroplasty combined with 3D printing. The patient's operation time, intraoperative blood loss, and hospitalization time were recorded. The hip function was assessed by Harris Hip Scale (HHS) after 6 months of follow-up. The patient's quality of life was assessed using the short form 36 item health survey questionnaire (SF-36) before and 6 months after surgery. The complications of the patients during operation and follow-up were recorded.Results:The operation time, hospital stay and intraoperative blood loss were significantly lower in the observation group than those in the control group ( P<0.05). At 6 months after operation, the hip joint function of the observation group was significantly better than that of the control group ( P<0.05); the quality of life scores were significantly higher than the control group ( P<0.05). There was no significant difference in the incidence of adverse reactions between the two groups ( P<0.05). Conclusions:3D printed total hip arthroplasty can effectively improve the operation and hip function of elderly patients with non-obsolete acetabular fracture and the quality of life of patients without increasing adverse reactions.
3.Clinical Observation of Levosimendan in the Treatment of Acute Left Heart Failure

OBJECTIVE:To investigate therapeutic efficacy and safety of leosimendan in the treatment of acute left heart failure (ALHF) and its effects on left ventricular function indexes and serum markers.METHODS:A total of 110 patients with acute left ventricular function selected as research objects from No.251 Hospital of PLA during Jan.2014-Dec.2015,and then were divided into control group (53 cases) and observation group (57 cases) according to random number table.Both groups received routine therapy.Control group was additionally given Dopamine hydrochloride injection or Dobutamine hydrochloride injection as cardiotonic on the basis of routine therapy.Observation group was additionally given Levosimendan injection with initial dose of 10 μg/kg+0.9% Sodium chloride injection 50 mL,ivgtt (10 min),and then with micro pump infusion at the rate of 0.1 g/(kg· min) for continuous 24 h.Both groups were treated for continuous 7 d.Clinical efficacies of 2 groups were observed,and the levels of left ventricular function indexes (PER,PFR,LVEF,LVSF) level,serum marker (NT-proBNP) and galectin-3 (Gal-3) before and after treatment,the occurrence of ADR was recorded.RESULTS:Two cases were withdrawn from the study due to death (one case in each group).Finally,a total of 108 cases were included,involving 52 cases in control group and 56 cases in observation group.Clinical total response rate of observation group (94.64%) was higher than that of control group (86.54%),but without statistical significance (P>0.05).Before treatment,there was no statistical significance in left ventricular function indexes or serum markers levels between 2 groups (P> 0.05).After treatment,the levels of left ventricular function indexes were improved significantly in 2 groups,and LVEF and LVSF of observation group were significantly higher than those of control group,with statistical significance (P<0.05).NT-proBNP and Gal-3 of 2 groups were decreased significantly,and the observation group was significantly lower than the control group,with statistical significance (P<0.05).No obvious ADR was found in 2 groups during treatment.CONCLUSIONS:Leosimendan in the treatment of ALHF have the similor clinical efficacy with dopamine,but helps to strengthen the left heart pump function,reduce heart failure markers levels with good safety.
4.A new natural saponin from fruits of Siraitia grosvenorii.

OBJECTIVETo study the chemical constituents in the fruits of Siraitia grosvenorii.
METHODIsolation and purification of the constituents were carried out on column chromatography. Their structures were identified by NMR and MS spectral analysis.
RESULTSix compounds were isolated and elucidated as mogroside IIIA1 (1), siamenoside I (2), mogroside IVa (3), mogroside IVe (4), mogroside V (5) and 11-oxo-mogroside V(6), respectively.
CONCLUSIONCompound 1, mogrol-24-O-beta-D-glucopyranosyl (1 --> 2)-[beta-D-glucopyranosyl(1 --> 6) ]-beta-D-glucopyranoside, was identified as a new natural product from the fruits of S. grosvenorii.
