Objective:
To preliminarily investigate the difference of position fixation accuracy between polyurethane styrofoam and vacuum negative pressure pad in intensity-modulated radiation therapy (IMRT) after radical mastectomy for breast cancer.
Methods:
Forty breast cancer patients, who received breast-conserving surgery followed by hyper-fractionated IMRT of the whole breast (42.56 Gy for 16 times) in our hospital between 2017 and 2018 were recruited and randomly divided into the polyurethane styrofoam group and vacuum negative pressure pad group. Before IMRT treatment, the anterior and lateral films of patients were taken with kilovoltage digital radiographs (KVDRs) by Varian Trilogy machine-borne OBI KV image verification system. The KVDRs images were matched with the DRR images reconstructed by the planned system to obtain the setup errors in the left and right, head and foot, and ventral and back directions between two groups. Objective:To explore the effects of the hierarchical management based on comprehensive quantitative risk assessment strategy in psychiatric inpatients.Methods:Totally 1 356 inpatients admitted at the Department of Psychiatric Rehabilitation in Huaian No.3 People's Hospital between January and December 2019 were included by convenient sampling into a control group, which received routine risk management and nursing intervention; 1 282 inpatients admitted between January and December 2020 were included into an observation group, which underwent hierarchical management based on comprehensive quantitative risk assessment on this basis. The incidence of accidental events was compared between the two groups of patients.Results:The incidence of accidental events in the observation group was 1.87%, lower than 3.54% in the control group, and the difference was statistically significant ( P<0.05) . Conclusions:The hierarchical management based on a comprehensive quantitative risk assessment can reduce the incidence of accidental risk events in psychiatric inpatients.

4.Clinical study of 63 cases of achalasia treated by peroral endoscopic myotomy

In order to evaluate the efficacy of peroral endoscopic myotomy (POEM) for achalasia, data of 63 patients with achalasia who were treated with POEM in China-Japan Union Hospital of Jilin University from 2017 to 2021 were collected. Postoperative Eckardt score, high-resolution manometry and upper gastrointestinal radiography were compared with preoperative data. The mean age of the 63 patients was 49.0 years, and there were 31 famales. The preoperative Eckardt score was 9 (3), and the postoperative Eckardt score was 2 (2), with significant difference ( V=1 953, P<0.001). The lower esophageal sphincter pressure decreased significantly after operation compared with that before operation [9.90 (3.35) mmHg VS 26.80(13.85)mmHg, V=2 016, P<0.001]. Fifty-three patients (84.1%) had satisfactory curative effects. The incidence of adverse events was 3.2% (2/63). POEM is safe, effective and minimally invasive for the treatment of achalasia.
5.Effects of society skill training on frustration and self-esteem of schizophrenics

Objective:To explore the effect of social skill training on the frustration and self-esteem of schizophrenics.Methods:From December 2020 to May 2021, 100 schizophrenics hospitalized in the same ward of the Huai'an Third People's Hospital of Jiangsu Province were selected as the research object by convenience sampling. The patients were randomly divided into the control group and the observation group with 50 cases each. The control group received routine nursing, while the observation group carried out social skills training on the basis of the control group. Before and 8 weeks after the intervention, the frustration, self-esteem and social function of the patients in the two groups were evaluated with the Defeat Scale (DS), Rosenberg Self-esteem Scale (SES) and the Scale of Social Function in Psychosis Inpatients (SSPI) .Results:Before intervention, there was no statistical difference in scores of frustration, self-esteem and social function between the two groups ( P>0.05). After intervention, the scores of frustration in the two groups were lower than those before intervention, and the scores in the observation group were lower than those in the control group, with statistical differences ( P<0.01). The scores of self-esteem and social function in the two groups were higher than those before intervention, and the scores in the observation group were higher than those in the control group, with statistical differences ( P<0.05) . Conclusions:Social skill training can improve the mental state of schizophrenics, reduce their frustration, adjust their self-esteem, and improve their social functions.

Objective:To evaluate the safety and efficacy of the domestic robotic surgical system for general surgery.Methods:A prospective single-center, single-arm exploratory study was conducted at Gansu Provincial People's Hospital from Jun 2022 to Oct 2023, enrolling 54 patients undergoing general surgery using domestically produced Toumai? Endoscopic Surgical Robotic System. The primary study endpoint was surgical success rate, and the secondary study endpoints were intraoperative bleeding, operative time, complications, system performance, hospitalization days.Results:In this study, robotic surgery was successfully completed in 52 patients, and in 2 patients undergoing thyroid operation it was converted to open surgery due to bleeding, with a success rate of 96%, no organ injury or death during surgery, and no system failure. The types of surgery included cholecystectomy, radical gastric cancer resection, radical colorectal cancer resection, inguinal hernia repair, partial hepatectomy, total thyroidectomy and choledocho-jejunal anastomosis.Conclusion:The study provides preliminary evidence of the safety and efficacy of the Toumai? Endoscopic Surgical Robotic System for the treatment of general surgical diseases.
7.Efficacy of laparoscopic choledochoscopy combined with holmium laser lithotripsy versus traditional laparotomy in treatment of bile duct stones: A Meta-analysis

ObjectiveTo systematically review the efficacy and safety of laparoscopic choledochoscopy combined with holmium laser lithotripsy through a meta-analysis. MethodsThis study was conducted based on PRISMA guidelines, with a PROSPERO registration number of CRD42023406221. Chinese databases including CNKI, Wanfang Data, and VIP and foreign language databases such as PubMed, Embase, the Cochrane Library, and Web of Science were searched for original articles on traditional laparotomy versus laparoscopic choledochoscopy combined with holmium laser lithotripsy in the treatment of bile duct stones. Dichotomous variables were assessed by odds ratio (OR) and 95% confidence interval (CI), while continuous variables were assessed by weighted mean difference (WMD) and 95%CI, and a sensitivity analysis was performed for outcome measures with relatively high heterogeneity. The Begg test and Egger test were used to evaluate publication bias. Stata 15.0 and Review Manager 5.3 were used to perform the statistical analysis. ResultsA total of 26 retrospective studies from China were included in this study, with 2 238 patients in total. The meta-analysis showed that compared with traditional laparotomy for the treatment of bile duct stones, laparoscopic choledochoscopy combined with holmium laser lithotripsy had significantly shorter time of operation (WMD=-1.26, 95%CI: -1.36 to -1.16, P<0.001), length of hospital stay (WMD=-1.93, 95%CI: -2.64 to -1.12, P <0.001), and time to bowel function recovery (WMD=-1.52, 95%CI: -1.68 to -1.35, P<0.001), significantly less intraoperative blood loss (WMD=-1.79, 95%CI: -1.93 to -1.66, P<0.001), a significantly lower rate of intraoperative residual stone (OR=0.15, 95%CI: 0.11-0.20, P<0.001), and significantly fewer complications (OR=0.17, 95%CI: 0.13-0.23, P<0.001). ConclusionCompared with traditional laparotomy, laparoscopic choledochoscopy combined with holmium laser lithotripsy shows better efficacy in the treatment of bile duct stones.
