2.Research and prospect of tissue engineered blood vessels

The tissue engineering technology that developed in the 1980s brings a hope for the regeneration of human organs. It is feasible to construct by-pass vessels, which have no immune resistance but are capable of anticoagulation. At present, a huge number of researches have provided the necessary basis for the construction. However, there are still some difficulties on the points of induction and proliferation of endothelial cells, development of new materials for scaffold and co-culture of seed cells in scaffold. These difficulties have become the bottle neck of tissue engineered blood vessels. The tissue engineering vessels are nowadays absent in clinical practice. With the progress in researches of tissue engineering, it is strongly believed that human organs and tissues can be constructed using tissue engineering technology, thus satisfying the donors of organ transplantation.
3.Research Progress in Effects of Herbal Medicines on PXR and CAR Nuclear Receptor Pathways

Pregnane X receptor ( PXR) , a member of the nuclear receptor subfamily, plays an important role in the regulation of drug metabolic enzymes and transporters. PXR can regulate the expression of downstream target genes through transforming a large number of exogenous and endogenous chemical substances, and can be activated by a variety of Chinese herbal medicines. The same as PXR, constitutive androstane receptor ( CAR) can participate in the regulation of drug metabolic enzyme CYP450 and become the targets of drug action through combining with exogenous ligands to regulate the expression of CYP2B6, CYP3A4, CYP2C19 and UGT1A1.
4.Changes in the pathological diagnosis rate of Graves'disease and Hashimoto's thyroiditis——analysis on the clinicopathologic data from 1978 to 2005 in General Hospital of Tianjin Medical University

Objective To observe the changes in the pathological diagnosis rate of Graves'disease(GD) and Hashimoto's thyroiditis(HT)in the past 28 years and to probe its relationship to the environmental factors. Methods Statistic analysis was performed on data of thyroid disease,GD,HT and the complications of HT in Department of Pathology(1978-2005)in General Hospital of Tianjin Medical University with Run test,Cox-stuart test,ANOVA,t or t'test and Chi-square test.Results In 28 years there were 243 403 biopsies(average 8 693/ year)including 6 771 cases of thyroid disease(27.8‰),216 cases of GD(0.89‰),352 cases of HT (1.45‰).The biopsy rate of thyroid disease and GD showed a descending tendency since 1982 but an ascending tendency from 1995,furthermore,with GD it showed a descending tendency from 2001 to 2005 once more.While the biopsy rate of HT and GD+HT showed a descending tendency from 1986 and an ascending tendency after 1996,being 1-4 years later than thyroid disease and GD.The biopsy rate of GD+HT and the proportion of HT showed a descending tendency in 2002-2005 and 2003-2005 respecitvely.The average age of total and female patients with GD showed a descending tendency from 1997.The average age of GD was 13.7 years younger than HT,the patients of GDⅠ-type andⅡ-type being younger thanⅢ-type.The average age of lymphoid type(L), oxyphilie epithelium type(O),pronounced epithelium destruction type(P)in HT increased sequentially.In 28 years,there were more female patients with GD and HT,especially with HT.There existed 7.95% hyperthyroidism,6.25% nodular goiter,2.27% carcinoma in HT(more in L-type)and 2.56% hypothyroidism in HT(more in P-type).Conclusion Within 28 years,according to the pathological diagnosis rate,GD,HT,and GD+HT showed phasic tendencies related to drinking low iodine water and the implement of universal salt iodization policies in 1994 and 2000.Therefore,on the levels of genetics and environment,the factors that influence autoimmunity of thyroid gland could not exclude the variations of iodine intake,infection,affection or emotional stress.The mechanism of the changes in the pathological diagnosis rate needs further study.
5.Digital artery bilobed flap for the treatment of skin degloving injury of thumb.

OBJECTIVETo explore clinical effects of digital artery bilobed flap for the treatment of skin degloving injury of thumb.
METHODSFrom January 2007 to December 2012, 45 patients with skin degloving injury of thumb were treated with grafting of digital artery bilobed flap. There were 39 males and 6 females, ranging in age from 19 to 46 years, with an average of 32 years. The disease course ranged from 0.5 to 15 h. Eighteen patients suffered from defect of palmar skin above nail root and nail bed, 19 patients suffered from skin degloving injury of thumb phalangette, and 8 patients had whole skin degloving injury of thumb. The double lobe flaps were designed at the ulnar side of middle finger and the radial side of ring finger according to the defect of thumb skin. The arteria digitalis communis between the middle and ring fingers and its two branches of arteriae digitales propriae supplying the two fingers were used as a vessel pedicle. The flap with digitales proprii nervi was transposed and used to cover the exposed phalanx of thumb. Full thickness graft was used for the donor site. Observation of the appearance, texture, color and wear resistance of flap, appearance, color and depression of grafting area, skin feeling, and finger activities was conducted.
RESULTSAll the flaps and grafts were alive. Forty-three patients were followed up with an average duration of 25 months, and two patients lost follow-up. The color and texture of the flaps were similar to that of the contralateral thumb pulp. The average two point discrimination was 4.2 +/- 0.3 mm. The color of graft skin was slightly deeper than that of the surroundings skin.
CONCLUSIONDigital artery bilobed flap graft is an effective and ideal operation, which is of low risk and high success rates for skin degloving injury of thumb.
