1.A retrospective survey on diagnosis and treatment of the patients with lung cancer in the affiliated hospital of Hainan Medical College and its revelation

Objective To investigate the diagnosis and treatment of the patients with lung cancer hospital-ized in affiliated hospital of Hainan Medical College.Methods A retrospective study was performed from January 1st 2005 to December 31th 2009 in the Hospital.201 patients were enrolled in this study.Results The average onset age of patients with lung cancer was 66 years old.No difference in different gender.The incidence of male was 72.1%, which was higher than of female(27.9%).Male to female ratio was 2.6:1.70.3% of male patients with lung cancer had a history of smoking.146 cases(72.6%)were diagnosed by clinical data,only 55 cases(27.4%)were diagnosed by histopathology.134 cases(66.7%)hadn′t indications for surgery.only 17 cases(25.4%)agreed to have surgery among 67 cases (33.3%)with indications for surgery,50 cases(74.6%)didn't agree to have surgery.34 cases (16.9%)had no indications for chemotherapy.40 cases(24.0%)agreed to have chemotherapy among 167 cases (83.1%)with indications for chemotherapy,but only 14 cases(35.0%)had completed 4 to 6 cycles of chemotherapy, and 26 cases(65.0%)hadn't completed 4 to 6 cycles.8 cases(4.0%)agreed to have radiotherapy.3 cases(1.5%) agreed to have targeted therapy.3 cases (1.5%)transfered to other hospital.Conclusion The ratio of lung cancer patients in male is higher than female in the hospital.smoking is closely related to male patients.Clinical diagnosis rate of patients is high,whereas histopathologic diagnosis rate is low.The rate of the intervention that patients accepted is low and many patients give up available management after the diagnosis is made.
2.Correlation of Isokinetic Parameter and Modified Ashworth Scale Applied in Evaluation of Ankle Spasticity

Objective To study the correlation between isokinetic parameters and modified Ashworth Scale (MAS) in ankle spasticity as-sessment. Methods 18 stroke patients (spasticity group) and 16 healthy subjects (control group) in our hospitals from August 2014 to March 2015 were included. MAS was used to assess the ankle muscular tension. The ankle passive movement of both groups was measured with BIODEX isokinetic motor assessment system under 10 °/s, 60 °/s, 120 °/s, 180 °/s, 240 °/s. The peak torque (PT), peak torque/body weight (PT/BW), average torque (AT), and slopes of the linear regression curve of torque-velocity (SLOPE) were recorded. The correlation of the isokinetic paramenters and the MAS were tested with Spearman correlation analysis. Results The PT, PT/BW and AT were higher in the spasticity group than in the control group (P<0.05). And they increased as the angular velocity increased, and slowed after 120 °/s. The corre-lation coefficient of MAS and PT, PT/BW, AT, SLOPE were from 0.3043 to 0.7632 (P<0.01). Conclusion The isokinetic parameters were speed-dependent and closely related to MAS. 120 °/s was of the highest sensitivity, and the SLOPE under this anglular velocity was highly correlated with MAS.
3.The characteristics of affective intensity and cognitive emotion regulation in borderline personality disorder

Objective To examine the characteristics of affective intensity and cognitive emotion regulation in subjects with borderline personality disorder ( BPD) . Methods The BPD subscale of personal-ity disorder questionnaire( PDQ-4+) was used to assess the BPD symptoms,the short affective intensity scale (SAIS) and cognitive emotion regulation questionnaire (CERQ) were used to measure affect intensity and cognitive emotion regulation strategy,respectively. 765 subjects with BPD and 776 healthy controls were se-lected. The independent-samples t test was used to analyze the differences between BPD group and controls and logistic regression analysis was used to examine the related factors affecting the BPD. Results Com-pared with the control group(negative intensity:(3.08±0.66),negative coping dimension:(37.20±5.94), BPD group got higher scores in negative intensity((3.88±0.74), t=22.29, P<0.01,Cohen''s d>0.8)and negative coping dimension((44.77±6.36), t=24.16, P<0.01,Cohen''s d>0.8). The logistic regression anal-ysis showed that negative intensity(B=1.38,Exp(B)=3.97,95%CI for EXP(B):3.15~5.00, P<0.01) and negative cognitive regulations strategy(B=0.19,Exp(B)=1.21,95% CI:1.18-1.25, P<0.01) could affect the prevalence of BPD. Conclusion Subjects with BPD traits have more significant negative affective inten-sity and tend to use negative cognitive regulations strategy.
4.Ideas Exploration and Analysis on the Prescription Comment of TCM Injections

OBJECTIVE:To explore the ideas for the prescription comment of TCM injections,and provide reference for clini-cal rational use. METHODS:Totally 9 457 prescriptions in PIVAS and 400 hospitalized medical records in our hospital in 2015 were collected. The prescriptions and hospitalized medical records which contained TCM injections were retrospectively commented in aspects of indications,usage and dosage,drug combination,solvent selection and repeat medication,and analysis of typical case. RESULTS:In 2015,there were 2 544 prescriptions(26.90%)in PIVAS and 195 medical records(48.75%)that used TCM injections;the irrational use was mainly inappropriate solvent selection(50.70%),inappropriate usage and dosage(27.00%),inap-propriate indications (10.33%),inappropriate combination therapy (7.51%) and repeat medication (4.46%). CONCLUSIONS:Prescription of TCM injections should be commented with the medical records and individual analyzed;in terms of unreasonable prescriptions and medical records TCM clinical pharmacists should actively intervene unreasonable prescriptions by the bed.
5.Effect of realgar on the gene expression profile of multiple myeloma cell line RPMI 8226.

OBJECTIVE:
To explore the effect of realgar on the gene expression profiles of multiple myeloma cell line RPMI 8226 by apply cDNA microarray.
METHODS:
The gene expression of RPMI 8226 cells before and after 48 hours of realgar treatment was determined with a cDNA microarray representing 4096 human genes.
RESULTS:
At the mRNA level, 164 genes were differentially altered; 53 genes were up-regulated; and 111 genes were down-regulated.
CONCLUSION
The realgar treatment to RPMI 8226 cell line may induce a number of gene changes. Many genes may be involved in the pathogenesis of multiple myeloma. BTG1, ALK1, and TXNIP genes may play an important role in the apoptosis and differentiation of RPMI 8226 cells.

6. Intestinal flora as the potential target for nutritional support treatment in abdominal infection and its clinical significance

Intra-abdominal infections (IAIs) are common in the clinical practice, which include a variety of patholo-gical conditions. Severe IAIs can lead to sepsis, secondary organ dysfunction, and threaten the lives of patients. Patients with IAIs are under a high metabolic reaction, and often have gastrointestinal dysfunction, manifesting as impaired intestinal mucosal barrier function, out of control in intestinal flora regulation, and continuous loss of nutrients. The body is in a malnutrition condition, and body resistance severely declines, which further aggravates disease progression. Intestinal micro-ecology is the largest and most complex ecosystem in the human body. In the case of coexistence of many bacteria, the synergy and antagonism between different strains maintain the balance of digestive tract microecology. Intestinal flora and nutritional status under IAIs have their particularity. Understanding the mechanism of intestinal flora abnormalities under IAIs, reasonable and effective nutritional support treatment and management is essential for improving the prognosis of patients with IAIs.

8.Foot inversion during walking among hemiplegic stroke survivors

Objective To find the plantar pressure readings which best indicate foot inversion during the stance phase of walking among hemiplegic stroke survivors.Methods Twenty-two hemiplegic stroke survivors who were able to walk without extra aid were recruited as the experimental group, while 17 healthy elderly men of similar age and body weight were selected as the control group.Those in both groups were asked to walk at their preferred speed over a Footscan device which measured medio-lateral pressure ratios, maximum plantar pressures and the contact areas of both feet.The Clinical Spasticity Index (CSI) was used to evaluate the affected feet.Results The average medial forefoot pressure of the affected side in the experimental group was significantly lower than that of the control group, but their average mesopodium and heel pressure was significantly higher.The average pressure applied by the great toe on the uninjured side in the experimental group was significantly smaller than that of the control group, but the average mesopodium and heel pressure of both feet among the hemiplegics were significantly higher than those of the control group.Among the experimental group, the average medial forefoot pressure of the affected foot was significantly greater than that of the healthy foot.The average contact area of the great toe on the affected side was significantly bigger than was observed in the control group.That of the medial forefoot was, however, significantly smaller than in the control group.There was no significant difference in the contact area between the healthy and affected feet in the experimental group, though the maxmium medio-lateral pressure ratios of their full feet and forefeet on the affected side were significantly lower than those in the healthy group.No significant differences in the maxmium medio-lateral pressure ratios of the heel were observed between the two groups, nor of the full feet, forefeet and heels of the affected and unaffected sides in the experimental group.The patients demonstrated consistently reduced joint mobility on both sides during the stance phase, coinciding with increased inversion.A significant negative correlation was found between the maxmium medio-lateral pressure ratios of the full foot and the maximum pressure of the lateral part of forefoot in the experimental group, but there was no significant correlation with contact area or CSI.Conclusions Plantar pressure data can be used to describe the amount of foot inversion in the stance phase of walking with hemiplegic patients after stroke.The maxmium medio-lateral pressure ratios can effectively reflect their foot inversion.
9.The evaluation of peri-implant bone defects with bioelectrical impedance in canine

Objective:To measure the bioelectrical impedance around the dental implants with bone defects.Methods:Bilateral maxillar second premolar were extracted in 6 native mongrel dogs,implants were placed immediately with or without bone defect of corresponding alveolar bene.Impedance value(IV)between the implant and contralateral mouth corner was measured by LCR TEST-ER.IVs were compared and analyzed by SPSS.Results:IVs of bone defect group were bigger than that of control group(P <0.05), in largger defect group were smaller than that in smaller group(P <0.05).Conclusion:Bioelectrical impedance can be used as an evaluation of peri-implant bone defects and the size of bone defect.
10.Analysis of clinical phenotypes and ATP7B gene variants in 75 children patients with Wilson' s disease.

OBJECTIVE:
To analyze the clinical phenotypes and ATP7B gene variants among children patients with Wilson' s disease from Northwestern China.
METHODS:
The clinical features and variants of the ATP7B gene among 75 children with hepatic Wilson' s disease were retrospectively analyzed.
RESULTS:
Among the 75 cases, 4 were presymptomatic, 59 had isolated transaminase elevation, 12 had acute and/or chronic liver diseases. Nine children were found to harbor homozygous variants, 64 harbored compound heterozygous variants, and two only had heterozygous variants of the ATP7B gene. In total 49 variants were detected, with common variants including c.2333G>T (p.Arg778Leu), c.2621C>T (p.Ala874Val) and c.2975C>T (Pro992Leu), which yielded allelic frequencies of 28.7%, 12.7% and 9.3%, respectively. Six novel variants were detected, which included c.1908dupC (p.Asn637Glnfs*118), c.4179_4180insC (p.Pro1394Profs*15), c.1604A>G (p.Glu535Gly), c.2278C>T (p.Pro760Ser), c.3008C>A (p.Ala1003Glu) and c.3532A>C (p.Thr1178Pro). Except for c.1604A>G (p.Glu535Gly), the remainder five were all predicted to be likely pathogenic. No significant correlation was found between genotype and phenotype among the patients.
CONCLUSION
The common mutation types of the ATP7B gene among patients with hepatic Wilson disease in Northwestern China are c.2333G>T (p.Arg778Leu), c.2621C>T (p.Ala874Val) and c.2975C>T (p.Pro992Leu), there is no significant correlation between their genotypes and phenotypes.
