1.Changes of biochemical indexes in the brain and spinal cord after shock and vibration damage

BACKGROUND: High-energy vibration is easy to damage non-cavity organs of the body and the damage effect is remarkable, but few studies concern the process of high-energy vibration-induced injury.
OBJECTIVE: To understand physiological, biochemical and pathological changes of animal bodies after high-energy vibration-induced injury.
METHODS: A total of 32 dogs were randomly divided into four groups. Dogs in the mild vibration injury group, moderate vibration injury group and severe vibration injury group received 700, 1 000, and 2 100 m/s2 vibration. Dogs in the control group were considered as normal controls. Within 14 days after vibration, serum K+, Ca2+, Zn2+, S100β, and neuron specific enolase concentrations were detected. Immunohistochemical staining was observed in the spinal cord and the brain.
RESULTS AND CONCLUSION: Serum K+, Ca2+, Zn2+concentrations showed a regular change in the three vibration injury group, and no evident change was found immediately after vibration injury. K+ concentrations reached a minimum at 0.5 day after vibration injury. Ca2+ concentrations reached a minimum at 1 day after vibration injury. Zn2+concentrations reached a minimum at 0.5 or 1 day. Above concentrations gradual y increased and became normal at 14 days. Serum neuron specific enolase and S100β expression increased at 0.5 day after vibration in the three vibration injury groups, peaked at 1 day, gradual y diminished, and recovered to a normal level or higher level at 14 days. In the three vibration groups, bleeding point of contact position and hedge position could be found in the spinal cord and brain. The degree of bleeding was more significant when kil ed instantly after vibration compared with that at 14 days. S100β, glial fibril ary acidic protein and neuron specific enolase expression increased in the spinal cord and brain.
2.Observations on the Efficacy of Combined Acupuncture and Medicine in Treating Polycystic Ovarian Syndrome Infertility

Objective To investigate the clinical efficacy and safety of combined acupuncture and medicine in treating polycystic ovarian syndrome infertility.Methods Ninety patients with polycystic ovarian syndrome infertility were randomly allocated to treatment and control groups, 45 cases each. The control group received oral administration of clomifene citrate tablets plus muscular injection of chorionic gonadotropin and the treatment group, electroacupuncture in addition. The ovulation rates, pregnancy rates, incidences of ovarian hyperstimulation syndrome (OHSS), ectopic pregnancy rates and early miscarriage rates were compared between the two groups of patients. Results The ovulation rate and the pregnancy rate were 97.8 % and 37.8%, respectively, in the treatment group and 75.6% and 17.8%, respectively, in the control group; there was a statistically significant difference between the two groups (P<0.05). Of 17 pregnant patients in the treatment group, 2 had early miscarriage, accounting for 11.8% and none had ectopic pregnancy, accounting for 0.0%. Of 8 pregnant patients in the control group, 4 had early miscarriage, accounting for 50.0% and none had ectopic pregnancy, accounting for 0.0%. The incidence of OHSS was 4.4% in the treatment group and 6.7% in the control group; there was no statistically significant difference between the two groups (P>0.05).Conclusions Combined use of acupuncture and medicine can increase the ovulation rate and the pregnancy rate in patients with polycystic ovarian syndrome infertility. It does not significantly cause ovarian hyperstimulation and ectopic pregnancy and may decrease the early miscarriage rate.
3.Effect of mechanical thrombectomy with Solitaire stent in the treatment of acute intracranial large artery occlusion

Objective To evaluate the safety and effectiveness of mechanical thrombectomy with Solitaire AB stent in the treatment of large intracranial artery occlusions.Methods The calinical data of 15 patients with acute stroke(more than 3.5 h intravenous thrombolysis time window) who were carried out arterial embolectomy with Solitaire AB stent in the No.264th Hospital of the Chinese People's Liberation Army were retrospective analyzed.There were 11 cases of middle cerebral artery(MCA),2 cases of internal carotid artery (ICA),1 case of vertebral artery(VA) pluse basilar artery(BA),and 1 case of anterior artery(AA).The recanalization occluded situation and surgical complications were analyzed,NIHSS score of preoperatively and at discharge were compared.Results In 15 cases,there were 14 cases of totally recanalization,1 case of partial recanalization.Two case with MCA stenosis and 1 case with VA stenosis after the recanalization were accepted stent angioplasty,2 csase died.NIHSS score of 13 cases survival patients increased from (22.85±4.75) scores on admission to (4.39 ±3.67) scores out of hospital,and the difference was statistically sinificant(t=2.752,P <0.01).Conclusion The mechanical thrombectomy with Solitaire AB stent can get high recanalization rate,fewer complications and good clinical outcome on patients with large intracranial artery occlusions.For more than venous or arterial thrombolysis time window,the mechanical thrombectomy can be considered within the 8 h after comprehensive evaluation.
4.Effectiveness comparison of channel-assisted mini-incision and open Achilles shortening for treatment of healed Achilles tendon rupture

Objective To compare the clinical effectiveness between the channel-assisted mini-invasion and open Achilles shortening for treatment of the elongated Achilles tendon following previous rupture.Methods The clinical data of 19 patients admitted from Dec. 2013 to Dec. 2015 and met the inclusion criteria were analyzed retrospectively. Eight patients were treated with shortening operation by channel-assisted minimally invasive repair system, while 11 patients received dissection of Krackow Achilles tendon shortening. There was no significant difference between the two groups in gender, age, injury to operation time, preoperative calf circumference and preoperative AOFAS (American Orthopaedic Foot & Ankle Society) score (P>0.05).Results The operation time, incision length and postoperative hospital days were significantly less in min-invasion group than in incision group (P<0.05). After operation, wound healing by first intention was obtained in incision group except 1 case which got infection and healed by treatment 2 weeks later. No sural nerve and vascular injury and lower extremity deep vein thrombosis existed. MRI showed that the Achilles tendon recovered in both groups at the 8th week follow-up. The patients were followed up for 12-17 months in the mini-invasion group and 12-20 months in the incision group. The calf circumference and AOFAS score of both groups were higher in the last follow-up than that in pre-operation (P<0.05), but no significant difference existed between the two groups at the last follow-up (P>0.05).ConclusionChannel-assisted minimally invasive Achilles tendon shortening operation has not only similar effectiveness to the incision shorting operation for the treatment of elongated Achilles tendon following previous rupture, but also has the advantages of shortening operation time and stay in hospital and avoidance of sural nerve injury.
5.Effects of Sangju Qingjie Decoction on ventilator-induced lung injury in rats

Objective To investigate the effects of Sangju Qingjie Decoction on the ventilator-induced lung injury in rats.Methods Thirty-six healthy male SD rats of both sexes,weighing 300-350 g,were randomly divided into 3 groups(n =12 each):control group(group C); mechanical ventilation group(group V)and Sangju Qingjie Decoction group(group SJ).The animals were anesthetized with intraperitoneal 3% urethane 1.4 g/kg and tracheostomized.The femoral artery and vein were cannulated for blood sampling and drug administration.The animals were mechanically ventilated for 2.5 h(VT =40 ml/kg,RR 40 bpm,I:E =1:1,FiO2 21%).Ingroup SJ,Sangju Qingjie Decoction 300 g was injected into the stomach through a gastric tube once a day for 10 consecutive days,and mechanical ventilation was performed 2 h after the last injection.While in groups V and C,the equal volume of normal saline was given instead of Sangju Qingjie Decoction.Arterial blood samples were collected before ventilation,at the end of ventilation,and at 30 min after ventilation(T0-2)for blood gas analysis.Respiratory index(RI)and oxygenation index(OI)were calculated.The animals were then sacrificed and the lungs were removed for microscopic examination and determination of the content of TNF-α,IL-6 and IL-10 and W/D lung weight ratio.Results Compared with group C,RI at T1,2,the content of TNF-o,IL-6 and IL-10 and W/D ratio were significantly increased,and OI was significantly decreased at T1.2 in groups V and SJ(P < 0.05).Compared with group V,RI at T1,2,the content of TNF-α and IL-6 and W/D ratio were significantly decreased,and OI at T1,2 and the content of IL-10 were significantly increased in group SJ(P < 0.05).The pathological damage was attenuated in group SJ compared with group V.Conclusion Sangju Qingjie Decoction can attenuate the ventilator-induced lung injury through inhibition of inflammatory response in rats.

7.Efficacy and safety of multiple target therapy for lupus nephritis in children

Objective To investigate the clinical efficacy of combined use of calcineurin inhibitor in the treatment of lupus nephritis in induction or maintenance,which is resistant to mycophenolate mofetil.Methods Sixty-six cases of children with lupus nephritis were selected from February 2014 to September 2016 in Huazhong University of Science and Technology,Tongji Medical College Affiliated Wuhan Children's Hospital.The randomized method was used to divide them into the control group and the observation group randomly.Among them,31 cases in the control group were given glucocorticoid,cyclophosphamide combined with traditional medicine for treatment;35 cases in observation group were given glucocorticoid,mycophenolate mofetil,tacrolimus (calcine phosphatase inhibitor) multi-target therapy for treatment.The clinical effect of 2 groups before and after treatment were compared,and the incidence of adverse reactions in the treatment of 2 groups of children were compared.Results After treatment,the levels of systemic lupus erythematosus disease activity index (SLEDAI),serum creatinine and 24 h urine protein [(6.05 ± 1.04) scores,(45.08 ± 18.52) μmol/L,(0.96 ±0.30) g/L] in the observation group were lower than those in the control group [(11.09 ±2.33) scores,(95.33 ±36.74) μmol/L,(2.05 ±0.74) g/L],and the differences were statistically significant (t =3.097,3.356,3.871,all P < 0.05).Serum complement C3,plasma albumin levels [(1.05 ± 0.28) g/L,(63.24 ± 12.98) g/L] were higher than those in the control group [(0.34 ±0.10) g/L,(35.45 ±6.74) g/L],and the differences were statistically significant (t =4.124,3.567,all P < 0.05).After treatment,the levels of serum complement C3 and plasma albumin were significantly higher between 2 groups than those before treatment,the differences were statistically significant (all P < 0.05).The incidence of adverse reaction (14.29%,5/35 cases) in the observation group was lower than that in the control group (38.71%,12/31 cases),and the difference was statistically significant (x2 =5.128,P < 0.05).Conclusion Multi-target combination therapy and traditional cyclophosphamide therapy can effectively control lupus nephritis in children,but the clinical effect of multi-target combination therapy is better and the adverse reaction is less.
8.Research on correlation between lung and large intestine based on meridian and acupoint palpation in patients with bronchial asthma.

OBJECTIVETo explore correlation between lung and large intestine and the two meridians under pathological condition in the view of meridian theory.
METHODSNinety-six cases of bronchial asthma were applied palpation at the running course of 12 regular meridians under the elbow and knees and back-shu points. And abnormal reactions were recorded, the affected meridians and back-shu points were discovered.
RESULTSThe abnormal reactions most frequently appeared on the Lung Meridian, followed by the Large Intestine Meridian, the Spleen Meridian, the Liver Meridian, the Stomach Meridian and the Triple Energizer Meridian. And the unusual reaction of the back-shu points most frequently appeared on Feishu (BL 13), and Dachangshu (BL 25) and Pishu (BL 21) followed as the next two.
CONCLUSIONThe existence of correlation between the Lung Meridian and the Large Intestine Meridians under pathological condition can be proved through meridian and acupoint palpation on bronchial asthma patients.

9.Report of 5 cases of pediatric plastic bronchitis and review of related literature

To report the clinical course of 5 cases of pediatric plastic bronchitis and review the related literature.A total of 113 cases of pediatric plastic bronchitis occurring in China from 2000 to 2012 were retrieved through the databases of CNKI and Wanfang Med Online.Retrospective analyses were performed for the main symptoms,courses,etiologies,imaging findings,histopathological classifications,therapies and prognosis of 118 cases.Pediatric plastic bronchitis was one of critical diseases.Most of them had a rapid onset and a mortality rate.Bronchoscopy examination was essential for definite diagnosis and effective treatment.Airway management and chest physiotherapy were primary adjuvant tools.Early diagnosis and bronchoscopic treatment might result in improved prognosis.
10.Application of Problem Based Learning in the Course of Assessment & Evaluation of Physical Therapy

Objective To investigate the effect of problem based learning (PBL) applied in the course of Assessment & Evaluation of Physical Therapy. Methods PBL and traditional teaching were applied in different classes. Their scores of the final examination were compared, and the students were investigated with the questionnaire. Results The scores of final examination were higher in the PBL class than that in the traditional teaching (P<0.001). A lot of the students accepted the PBL in the course. Conclusion PBL can improved the teaching quality of the course of Assessment & Evaluation of Physical Therapy.
