1.Epidemiological characteristics of pulmonary tuberculosis in Taizhou City from 2016 to 202

Objective:
To investigate the epidemiological characteristics of pulmonary tuberculosis in Taizhou City from 2016 to 2021, so as to provide the evidence for formulating the pulmonary tuberculosis control measures.
Methods:
The epidemiological data of pulmonary tuberculosis cases in Taizhou City from 2016 to 2021 were retrieved from the Tuberculosis Management Information System and Infectious Disease Surveillance System of Chinese Disease Prevention and Control Information System, and the trends in the incidence of pulmonary tuberculosis and regional, population and temporal distributions of pulmonary tuberculosis were analyzed using a descriptive epidemiological method in Taizhou City from 2016 to 2021.
Results:
A total of 15 621 pulmonary tuberculosis cases were reported in Taizhou City from 2016 to 2021, with annual mean reported incidence of 42.12/105. The incidence of reported pulmonary tuberculosis reduced from 43.74/105 in 2016 to 36.31/105 in 2021, with an annual decline rate of 3.65% (χ2trend=34.172, P<0.001). There were 7 606 cases positive for pathogenic tests (48.69%), and the positive rate of pathogenic tests appeared a tendency towards a rise from 2016 to 2021 (χ2trend =470.906, P<0.001). The reported incidence rates of pulmonary tuberculosis were 57.83/105 and 25.46/105 in men and women, and a high incidence rate was reported in patients at ages of 20 to <25 years (72.80/105) and 65 years and older (84.75/105). Farmer was the predominant type of occupation (8 365 cases, 53.55%), and the cases peaked from March to August, with the greatest seasonal index in May (119.76%). A high annual incidence rate of pulmonary tuberculosis was reported in Xianju County (52.51/105) and Wenling City (49.98/105) from 2016 to 2021, and the a high constituent ratio of patients positive for pathogenic tests was seen in Sanmen County (58.71%) and Luqiao District (53.24%).
Conclusions
The reported incidence of pulmonary tuberculosis showed a tendency towards a decline, and the proportion of pulmonary tuberculosis patients positive for pathogenic tests showed a remarkable increase in Taizhou City from 2016 to 2021. The number of patients peaked in spring and summer. In addition, high incidence of pulmonary tuberculosis was seen in Xianju County and Wenling City.
2.Lean management in informationalized health check-up

Objective To discuss the utility of lean management in the informationalization of health check-up.Methods A total of 2125 matched adults who received health check-up from April to July 2008 (n =1001) or 2012 (n =1124) were enrolled by using random sampling.Time of health check-up registration and laboratory sheet filling,preparation of final report,consumption of labor force,and integrity of data collection were compared before and after the conduction of lean management.Results Since lean management startup,time of registration was decreased from (250.0 ± 7.7) to (7.8 ± 0.9) min/100persons,time of sheet filling was reduced from (137.0 ± 10.6) to (26.0 ± 2.9) min/100 persons,preparation of final report was less time-consuming ((702.0 ± 11.7) vs.(300.0 ± 12.6) min/100persons).The integrity and accuracy of health check-up reports were significantly improved (67.0% vs.99.2%).Newer health check-up receivers were increased from 17.8% to 29.4% (x2 =461.944,P <0.05).Conclusion Lean management may be feasible and effective in the informationalization of health check-up.
3.Analysis of the first measurement of CD4+T lymphocyte cell counts of newly reported HIV/AIDS cases in Taizhou City from 2012 to 2021

Objective:
To investigate the level and timeliness of CD4+T lymphocyte cell (CD4 cell) counts at first measurement among newly reported HIV/AIDS cases in Taizhou City, Zhejiang Province from 2012 to 2021, so as to provide insights into improving the management of HIV/AIDS cases.
Methods:
Demographic data and first measurement of CD4 cell counts of newly reported HIV/AIDS cases in Taizhou City from 2012 to 2021 were collected from the HIV/AIDS Comprehensive Control System of Chinese Disease Prevention and Control Information System. The level and time of CD4 cell counts at first measurement were descriptively analyzed, and factors affecting CD4 cell counts at first measurement were identified using a multivariable logistic regression model.
Results:
A total of 4 834 newly reported HIV/AIDS cases were recorded in Taizhou City from 2012 to 2021, including 3 889 men (80.45%), 2 005 cases at ages of 20 to 39 years (41.48%), and 2 130 farmers (44.06%). There were 1 664 cases (34.42%) with CD4 cell counts of <200/mm3 at first detection, 2 576 (53.29%) with CD4 cell counts of 200/mm3 to 499/mm3, and 594 (12.29%) with CD4 cell counts of ≥500/mm3. The proportion of CD4 cell counts of <200/mm3 showed a tendency towards a rise from 2012 to 2021 (χ2trend =4.250, P<0.001). There were 3 465 cases (71.68%) that had an interval of ≤14 days between the first detection of CD4 cell counts and confirmatory HIV test, 740 (15.31%) that had an interval of >14 to 30 days, 315 (6.52%) that had an interval of >30 to 90 days, 135 (2.79%) that had an interval of >90 to 180 days, and 179 (3.70%) that had an interval of >180 days. The proportion of an interval of ≤14 days appeared a tendency towards a rise from 2012 to 2021 (χ2trend=6.874, P<0.001). Multivariable logistic regression analysis identified women (OR=0.630, 95%CI: 0.529-0.751), age of ≥20 years (OR: 1.958 to 3.218, 95%CI: 1.216-5.412), other or unknown routes of HIV infection (OR=1.785, 95%CI: 1.100-2.896), and identification by medical institutions (OR=1.779, 95%CI: 1.497-2.114) as factors affecting CD4 cell counts of <200/mm3 at first measurement.
Conclusions
The timely detection of CD4 cell counts at first measurement gradually increased with year among newly reported HIV/AIDS cases in Taizhou City from 2012 to 2021; however, there were still 34.42% of cases with CD4 cell counts of <200/mm3. Gender, age, route of HIV infection, and sample source were factors affecting CD4 cell counts of <200/mm3 at first measurement.
4.Studying the establishment of rat model of asphyxial cardiac arrest and the efficacy of CPR

Objective To study the establishment of rat model of asphyxia-cardiac arrest and efficacy of CPR in order to find the length of optimum time of asphyxia to cause injury.Methods One hundred and twenty-six male Sprague-Dawley rats were randomly (random number) divided into sham operation group and experimental groups.Cardiac arrest was induced by asphyxiation after intravenous injection of vecuronium bromide.The experimental groups were assigned into AP4 (four-minute asphyxia period),AP6 and AP8 subgroups in accordance with different lengths of time of asphyxia subjected to.In these groups,CPR,including pre-cordial compression and synchronized mechanical ventilation,was initiated 4,6 and 8 min after asphyxia-induced cardiac arrest,respectively.The successful ratio of resuscitation and hemodynamic variables were recorded.Brain water content,neural deficit scores (NDS),imaging changes on MR,pathological changes of brain tissue and neuronal apoptosis were evaluated at 1 d,3 d and 7 days after ROSC.All the data were analyzed by single-factor analysis of variance or Chi-square test.P < 0.05 was considered statistically significant.Result The lowest NDS occurred at 1 d after ROSC,brain water content and imaging changes on MR were most obvious at 3 d after ROSC,while pathological changes of brain tissue and neuronal apoptosis increased and reached the peak at 7d after ROSC.The survival rates after 24 hours of AP4,AP6 and AP8 groups were 85%,75% and 45%,respectively.The rate of ROSC and survival rate of AP8 group were significantly lower than those of other groups (P <0.01).The longer time of asphyxia the severer pathological changes of brain tissue,brain edema,neural deficit,and magnetic resonance imaging changes in all experimental groups.As compared to other groups,the brain damage index of AP8 group was most serious,while that of AP6 group was moderate.Conclusions The rat model following asphyxia-induced cardiac arrest and cardiopulmonary resuscitation was established successfully.From the evidence of survival rate and damage grade of brain tissue,asphyxia for 6 min may be the rational length of ischemic time in this model.
5.Clinical application of self-made distal femur alignment cutting template in total knee arthroplasty

Objective To evaluate the accuracy and feasibility of primary total knee arthroplasty (TKA) assisted by the self-made extramedullary osteotomy template on distal femur alignment.Methods We retrospectively analyzed the clinical data of 40 patients with osteoarthritis or rheumatoid undergoing primary TKA from September 2015 to March 2016.All patients were divided into two groups,extramedullary group and intramedullary group.Extramedullary group had 20 patients (4 males and 16 females) aged 65.1±6.9 years (range 51-75).Intramedullary group had 20 patients (3 males and 17 females) aged 65.7±7.9 years (range 48-75).The blood loss volume,drainage in operation,postoperative range of motion (ROM) at 1 week and 2 weeks,deviation between the ideal value and preoperative hip-knee-ankle (HKA) angle,coronal and sagittal angle of the femoral prosthesis were compared within the two groups.Results The age,gender,body mass index,deviation between the ideal value and preoperative HKA angle,Hospital for Special Surgery (HSS) scoring system,ROM of knee were not significantly different between the two groups.The operation time was 97.4± 14.0 min in extramedullary group and 111.7±9.9 min in intramedullary group (P < 0.05).The blood loss was 71.8± 14.0 ml in extramedullary group and 111.7± 10.0 ml in intramedullary group (P < 0.05),while the drainage volume was 167.0±74.5 ml in extramedullary group and 243.2±91.6 ml in intramedullary group (P< 0.05).The postoperative mean deviation of the HKA angle in extramedullary group (2.37°±0.73°) was smaller than that in intramedullary group (3.26°±1.42°).Furthermore,the coronal alignment of the femoral prosthesis in extramedullary group (89.70°±1.91°) was larger than that in intramedullary group (87.69°±2.39°).The sagittal alignment of the femoral prosthesis in extramedullary group (90.03°±2.62°) was also significantly larger than that in intramedullary group (88.07°±2.43°,P < 0.05).The postoperative ROM of knee at 1 and 2 week in the extramedullary group were 101.75°±7.48° and 114.00°±7.88° respectively,while those in the intramedullary group were 101.25°±8.41° and 114.25°±8.78°.There was no statistically different between the two groups.There was no intraoperative and postoperative early complications reported in the two groups.Conclusion Although the primary TKA assisted by the selfmade extramedullary osteotomy template on distal femur alignment can shorten operation time,decrease the blood loss and increase the accuracy of coronal and sagittal alignment,the HSS score and ROM of knee are similar as intramedullary method.

Objective To build the suitable index system of the clinical research program acceptance evaluation.Methods The Delphi method and the weighted average method were adopted to determine the weight of each index.Results Followed by two rounds of Delphi method, twenty five evaluation indices were selected, including three first-class indices, seven second-class indices and fifteen third-class indices, and the weight of each index was calculated.Conclusions The index system,simple and reasonable, reflects the characteristics of clinical applied researches to a certain extent with a certain degree of operability, laid the foundation for the further supporting to carry out scientific research projects.
9.Percutaneous permeability of lidocaine hydrochloride loaded destran-based niosomes

Objective To study the percutaneous permeability through mouse skin of lidocaine hydrochloride-loaded destran-based niosomes(LID-HLD-BNs)in vitro and in vivo. Methods HPLC was employed to exam lidocaine hydrochlo?ride. Lidocaine hydro-chloride-loaded conventional liposomes (LID-CLs) and lidocaine hydrochloride injection (LID-IJ) were used as control. Isolated mouse skin was added into Franz diffusion cell to evaluate the permeability of LID-HLD-BNs in vitro. Confocal Laser Scanning Microscopy(CLSM)was used to observe the permeation depth of mouse skin in vivo. Re?sults The permeation rate and cumulative permeation amount were significantly higher in LID-HLD-BNs group than those of LID-CLs and LID-IJ groups (P<0.05). CLSM studies also confirmed that HLD-BNs reached deeper layers of the skin. Conclusion LID-HLD-BNs has good transdermal ability.
10.Quality classification criteria of Paeonia suffruticosa seeds.

In order to establish the quality classification criteria of Paeonia suffruticosa seeds, thirty-one batches of P. suffruticosa seeds from different provenances were selected. The seed rooting rate, seed germination rate, seed purity, seed viability, 1,000-seed weight and moisture content were determined and analyzed through SPSS 20.0 software. Seed rooting rate, seed germination rate and seed purity were selected as the main index for classification, while 1,000-seed weight, seed viability and moisture content could be used as important references. The seed quality grading of P. suffruticosa was set as three grades. The seed quality of each grade should meet following requirements: For the first grade seeds, seed rooting rate ≥ 80%, seed germination rate ≥ 80%, seed purity ≥ 90%, seed viability ≥ 80%, 1,000-seed weight ≥ 250 g, moisture content, ≤ 10. For the second grade seeds, seed rooting rate ≥ 50%, seed germination rate ≥ 60%, seed purity ≥ 70%, seed viability ≥ 75%, 1,000-seed weight ≥ 225 g, moisture content ≤ 10. For the third grade seeds, seed rooting rate ≥ 20%, seed germination rate ≥ 45%, seed purity ≥ 60%, seed viability ≥ 45%, 1,000-seed weight ≥ 205 g, moisture content ≤ 10. The quality classification criteria of P. suffruticosa seeds have been initially established.
