ObjectiveTo evaluate the clinical and imaging features and infarct pattern in patients with middle cerebral artery(MCA) stenosis and occlusion,to clarify its infarct pathogenesis. Methods35 MCA moderate and severe stenosis or occlusion patients diagnosed by TCD were studied;the clinical and imaging features and infarct pattern were retrospectively analyzed. Results( 1 ) Clinical features :The clinical manifestations of 35 patients was hemiplegia symptoms,in which 16 cases(45.7% ) involved to the face, upper and lower limb,11 cases(31.4% ) involved to the upper and lower limb,3 cases(8.6% ) involved to the facial and upper limb,2 cases(5.7% ) involved only to the facial,2 cases(5.7% ) involved to the facial and upper limb,1 cases(2.9% ) involved only to the upper limb. (2) MRA imaging characteristics and infarct pattern: 29 cases ( 82.9% ) of patients with stenosis or occlusion showed moderate severe stenosis,6 cases( 17.1% ) showed MCA occlusion. 30 cases( 85.7% ) showed ischemic infarction,in which perforating artery infarction was the most common infarct pattern of middle cerebral artery stenosis or occlusive patients( 53.3% ) ,followed by water-shed infarct ( 26. 7) %, pial infarct was ielatively small (20. 0% ). ConclusionThe hemiplegia was the most common clinical symptoms in patients with MCA stenosis and occlusion, and mainly involves to the face, upper and lower limb; The perforating artery infarction was the most common infarct pattern, and its infarct mechanism was an artery-to-artery embolism or hemodynamic abnormalities.

Objective To evaluate the clinical features and efficiency of surgical treatment in elderly patients with parathyroid tumor. Methods Clinical data of 12 cases (aged 60-83 years) was retrospectively analyzed.Their main complaints varies individually, including bone and muscle trouble in 9 patients, abdominal problem in 7 ones, and kidney stone in 6 ones.PTH level ranged from 78 to 2850 μg/L,and calcium level 2.6-4.5 mmol/L.Parathyroidectomy was carried out in 8 patients, while extended excision of suspected tissues in 6 ones.Intraoperative PTH determination was routinely performed. Results A drop by at least 50% in PTH level was achieved in all cases after 10 minutes of tumor-bearing tissues excision.A much improvement of main complaints was shown in all cases about 1 week postoperatively.During the follow-up period from 18 to 48 months, both PTH and calcium levels were still in normal range in 9 patients, relapse was noticed in two cases 1 or 2 years after operation respectively. One patient died even after successful excision of parathyroid tumor. Conclusions PTH and calcium level determination is advised in elderly patients with suspected parathyroid tumor, and surgical treatment is generally safe and effective.

Objective To study the MRI features and its ultrastructural pathology basis in different myocardial injury areas after acute infarction. Methods 10 rabbits were given ligation of left anterior descending branch 24 hours to make pathologic models. All animals underwent conventional MRI,contrast media dynamic enhancement MRI,cine-MRI and dobutamine stress test.Myocardial blood flow was measured with radioactive micropheres to define risk regions.Ischemic and infarcted regions were defined as Even's blue and 2,3,5triphenyltetrazolium chloride(TTC)negative regions respectively, and made electron microscopy specimens to observe the changes of mitochondria and myofibril. The myocardial ultrastructural injury was scored and graded. Results There were no significant change on T 1WI,but the signal intensity on T 2WI increased significantly after AMI.The diseased myocardium became thinning were seen in 40% cases, and the abnormal flowing high signal were seen in 60% cases. The dynamic enhancement time-signal intensity curves were different in normal, ischemic and infarcted regions.Cine-MRI showed the cardiac wall movement impairing and the wall thick thinning, but the wall movement reinforcing and the wall thick thickening after dobutamine stress. The water content of ischemic and infracted myocardium were marked higher than normal myocardium. The myocardial blood flow in diseased regions were decreased significantly than that in normal regions. The volume density and numerical density of mitochondria in different injury regions showed significant differences. Conclusion MRI appearances of AMI were associated with the injury degree of myocardial ultrastructure and the residual myocardial blood flow.

Purpose To assess the clinical and pathological features of small gastrointestinal stromal tumours (sGIST).Methods To reevaluated the clinical, histological and immunohistochemical parameters of 21 sGISTs.The standard immunohistochemical panel antibodies were studied on the tumor sections. All data were compared with clinical sGIST.Results There were a total of 7 females and 14 males of sGISTs.The median age was 63 years old.The tumors were predominantly located in the stomach showing a spindle cell morphology and the tumor sizes ranged from 0.5 cm to 1.5 cm.9 sGISTs combined with malignant tumors, which were gastric cancer have been incidentally detected during surgery. As the lesions were small in size, with infrequent bleeding, necrosis, mucosal invasion, ulceration and less mitotic index,sGISTs reoccurred less compared with clinical sGIST.p53,Ki-67 labeling index and microvascular density (MVD) in sGIST were significantly lower than clinical sGIST (P < 0.05).Conclusion sGIST may occure with digestive tract cancer synchronously.p53,Ki-67 labeling index and MVD were lower than clinical GIST,which means better prognosis.

OBJECTIVE:To provide reference for improving the education of rational drug use and skills for clinical medical students in the internship period. METHODS:Teachers in the respiratory department of our hospital took part in training interns and clinical pharmacists took part in teaching rounds to train about medication education and skills, including linking up the theoret-ical knowledge of rational drug use, training of skills about issuing rational prescriptions and judging drug information, guiding to participate in the education of rational use of drugs to patients and evaluating the knowledge and skill of rational drug use. RE-SULTS & CONCLUSIONS:Carrying out rational drug use education in practice will improve the mastery of knowledge of rational drug use, enhance the awareness of rational drug use and the idea, which is helpful for achieving the irrational drug use in clinical practice, and improve the overall quality of medical services. but there still remain some problems to be explored.

Lack of overall evaluation on the occupational ability of students existed in the national rehabilitation therapy education. And the students faced the problems such as difficulty in relating the theory with the practice and poor practical ability during the clinical internships. Objective structured clinical examination (OSCE), an objective evaluation method, which could be used as reference, could be adopted to simulate the clinical scene to test students' clinical skills.
