1.Investigation of psychological states among the caregivers of alimentary canal cancer patients and their coping styles

Objective To evaluate psychological states t among the caregivers of alimentary canal cancer patients and their coping styles. Methods By using SAS, SDS and SCSQ, 60 caregivers of the alimentary canal cancer patients were investigated and analyzed. Results The SAS scores in the observation group were significantly higher than the norm (P < 0. 001). 42 (70%) cases had depression feeling. Their anxiety and depression feeling were significantly negatively related to relation to patients and to ways of medical treatment expense. There were no significance in the scores of coping styles. The active coping styles scores of the caregivers of the alimentary canal cancer patients were significantly higher than the norm (P < 0. 001). Conclusions The caregivers of the alimentary canal cancer patients were mostly associated with the anxiety and depression feeling which were affected by a wide range of psychological factors. They mostly took the active coping styles to relieved their negative mood.
2.Comparison of effects of two intravenous infusion methods on elderly patients

Objective To explore effects of two intravenous infusion methods on elderly patients.Methods Self-control method was used on 568 cases of elderly patients.Traditional method was used on Day 1:The adjustable clamp was fully opened when a free flow of blood appeared during venipuncture.The experimental method was used on Day 2:The adjustable clamp was opened 1/3 when a free flow of blood appeared during venipuncture.Success rate of venipuncture and value of pain were compared between the two groups.Results The experimental method had a higher success rate of venipuncture than the tranditional method (97.89% vs 93.31%,x2 =13.098,P < 0.01 ).There was no significant difference in the mean value of pain during a successful venipuncture ( P > 0.05 ).During an unsuccessful venipuncture,mean value of pain in the experimental group was lower than that in the traditional method group,and the difference was statistically significant [ ( 1.97 ±0.52) vs (2.35 ± 0.66 ),t =4.987,P < 0.01 ].Conclusions Opening the adjustable clamp for 1/3 when a free flow of blood appeared during venipuncture on elderly patients could improve the success rate of venipuncture and decrease pain during an unsuccessful venipuncture.
3.Effect of calcium antagonist verapamil on tolbutamide-induced insulin release from islet?-cells of rats

Objective To investigate the effect of calcium antagonist verapamil on the function of rat?- cells and tolbutamide (D860)-induced insulin release.Methods Insulin released from isolated islets were measured in control,verapamil,D860,and verapamil+D860 groups.Furthermore,intravenous glucose tolerance test (IVGTT) was conducted in acute experiments treated with verapamil and D860 respectively to assess?-cell function in rats with the same allocation as in vitro.Another IVGTT was performed in the end of 4 weeks' treatment.The insulin contents in pancreas were assayed and pancreas islets morphology were observed with immunohistochemistry.Results Verapamil could inhibit insulin release from isolated islets.Verapamil group was [(1.244?0.082)ng?ml~(-1)?islet~(-1)]and control group (2.623?0.226) ng?ml~(-1)?islet~(-1)(P0.05).Also,similar results were obtained in normal rats during acute experiments and verapamil reduce the hypoglycemic effect promoted by D860. However,above results were not observed in the end of 4 weeks experiments,and no difference for insulin content and morphological change in islets was found among four groups.Conclusion Treatment of verapamil chronically does not impair islet function and interfere with the hypoglycemic effect of D860 in rats .
4.Construction of recombinant plasmid human imprinted gene PEG10 and the primary functional identification in transfected cell lines.

OBJECTIVETo construct a plasmid expressing human imprinted gene PEG10 and to study the effect of overexpression of PEG10 in a stable transfected human normal liver cell line L02 and in non-liver derived cell line 293.
METHODSFull length cDNA of PEG10 open reading frame 1 was amplified and subcloned into a mammalian expression vector pcDNA3.1hisC. Recombinant plasmid was stably transfected into L02 cells and control cells via Lipofectamine 2000. The expression and the function of PEG10 in L02 cells and control group cells were examined using RT-PCR, Western blot, MTT and TUNEL.
RESULTSRecombinant plasmid was successfully constructed and confirmed through DNA sequencing and restriction digesting. PEG10 gene accelerated the growth of L02 cells and inhibited their apoptosis but it had no conspicuous effect on the non-liver derived cells.
CONCLUSIONThe constructed expressing vector pcDNA3.1hisC-PEG10 provides a useful tool for further study on the effects and mechanisms of PEG10. Over-expression of PEG10 may promote L02 cells' proliferation and inhibit their apoptosis, but not in the non-liver derived cell line 293.

5.Clinical study on combination of acupuncture, cupping and medicine for treatment of fibromyalgia syndrome.

OBJECTIVETo observe the therapeutic effect of acupuncture at five mental points and moving cupping on the Hechelu of the back on fibromyalgia syndrome (FS).
METHODSSixty-six cases who conformed to the criteria were randomly divided into the treatment group treated with acupuncture at five mental points, moving cupping on the Hechelu of the back and amitriptyline, and the control group treated with amitriptyline. Clinical therapeutic effects were assessed with McGill Pain Questionnaire (MPQ) and HAMD depression scale.
RESULTSThe therapeutic effect of the treatment group was better than that of the control group with a significant difference between the two groups (P < 0.01).
CONCLUSIONCombination of acupuncture with cupping therapy is an effective therapy for fibromyalgia syndrome.

6.Rosai-Dorfman disease: clinicopathologic, immunohistochemical and etiologic study of 16 cases.

OBJECTIVESTo study the clinicopathologic features of Rosai-Dorfman disease (RDD), expression of various antigens, human herpes virus type 8 (HHV8), human papillomavirus (HPV)-DNA and Epstein-Barr virus (EBV)-mRNA, and compare the findings with those in the literature.
METHODSThe clinicopathologic findings of 16 Rosai-Dorfman disease cases were retrospectively reviewed. Immunohistochemical study for S-100 protein, CD68 (PG-M1), CD163, CD21, CD1a, CD20, CD45RO, CD4, CD8, M-CSF and HHV8 was carried out in 9 of the 16 cases. In-situ hybridization for EBV-mRNA and HPV-DNA was also performed.
RESULTSThe male-to-female ratio of the patients was 4.33:1. Amongst the 16 cases studied, 62.5% (10/16) presented nodal RDD, with cervical lymph node predominantly involved. Half of these cases had affected lymph nodes in more than one anatomic site. Extranodal RDD represented 37.5% (6/16) of the cases. The relapse rate of extranodal RDD was higher than that of nodal RDD. Histologically, nodal RDD was characterized by dilated sinuses filled with large polygonal histiocytes which contained lymphocytes and plasma cells. For extranodal lesions, various degrees of stromal fibrosis were seen in association with mixed inflammatory cells (especially plasma cells). The large polygonal histiocytes varied in number and were distributed in clusters or patches. Immunohistochemical study showed that the abnormal histiocytes were strongly positive for S-100 protein. They also expressed CD68, CD163 and M-CSF, but were negative for CD1a, CD21 and HHV8. The lymphocytes in cytoplasm of these histiocytes were positive for both T and B cell markers (with T cell predominance, including a mixture of CD4- and CD8-positive cells). HPV-DNA and EBV-mRNA were not detected by in-situ hybridization. To date, 62 cases of RDD have been reported in mainland China, including 34 cases of nodal RDD and 18 cases of extranodal RDD. The remaining 10 cases involved both lymph nodes and extranodal sites. Compared with overseas reports, RDD occurring in China tended to affect older patients and with slight male predilection.
CONCLUSIONSRosai-Dorfman disease is relatively rare in China. Pathologic diagnosis of extranodal RDD may be difficult. The demographic data of RDD in China, including age and sex of patients, are different from those in the literature.

7.Famitinib in metastatic renal cell carcinoma: a single center study.

BACKGROUNDFamitinib is a novel and potent multitargeting receptor tyrosine kinase inhibitor. The phase I clinical study showed that famitinib was well tolerated and had a broad anti-tumor spectrum. The purpose of this study was to examine the efficacy and safety of famitinib for the treatment of metastatic renal cell carcinoma (mRCC).
METHODSThe data of famitinib in treating patients with mRCC from the single-center phases I and II clinical trials were analyzed. Famitinib was administered orally at the dose of 13-30 mg once daily until tumor progression, occurrence of intolerable adverse reactions or withdrawal of the informed consent.
RESULTSA total of 24 patients with mRCC were treated including 17 patients at a dose of 25 mg once daily, 4 patients at a dose of 27 mg and 1 patient each at a dose of 13 mg, 20 mg and 30 mg, respectively. Twelve (50.0%) patients achieved partial response (PR) and 9 patients achieved stable disease (SD). Progressive disease was found in 3 (12.5%) patients. The disease control rate was 87.5%. The median follow-up time was 17.6 months; the median progression free survival (PFS) was 10.7 (95% CI 7.0-14.4) months; and the estimated median overall survival (OS) time was 33.0 (95% CI 8.7-57.3) months. The adverse drug reactions mainly included hypertension (54.1%), hand-foot skin reactions (45.8%), diarrhea (33.3%), mucositis (29.2%), neutropenia (45.8%), thrombocytopenia (29.2%), hyperlipidemia (41.7%) and proteinuria (41.7%). The incidence rate of grades 3 and 4 adverse events was low, mainly including hypertension 12.5%, hand-foot skin reactions 4.2%, neutropenia 4.2%, thrombocytopenia 4.2%, hyperlipidemia 4.2% and proteinuria 12.5%.
CONCLUSIONSFamitinib has significant anti-tumor activity in mRCC. The common adverse reactions are generally manageable.

8.Survival analysis for laryngeal carcinoma patients with no surgery, radiotherapy or chemotherapy

Objective To investigate the survival rate and prognostic factors of laryngeal carcinoma patients with no surgery,radiotherapy or chemotherapy.Methods One hundred and sixty-seven laryngeal carcinoma cases with no surgery,radiotherapy or chemotherapy were analyzed restrospectively.Survival rates were calculated by Kaplan-Meier product-limit method.With univariate analysis,comparisons among/between groups were performed using Log-rank test.Multivariate analysis was carried out using Cox proportional hazard model.Results Overall survival time was (16.0 ± 1.4) months (-x ± s),overall 1-and 2-year survival rates were 56.4% and 26.5%,respectively.No patient survived over 5 years in these cases who had been diagnosed more than 5 years (except for those who lost).Univariate analysis showed that primary site,pathological grade,T-stage,N-stage and clinical stage were significant prognostic factors for the survival of the patients (P < 0.05=.The survival rates of laryngeal carcinoma whether with tracheotomy were no statistically significant (P>0.05).Multivariate analysis showed survival rates statistically correlated with T stage and N stage (hazard ratio were 1.812 and 1.557,P < 0.05).Conclusions The development of laryngeal carcinoma course was faster,without treatment to the tumor itself,even if palliative surgical such as tracheostomy would not improve the survival rate.In laryngeal carcinoma patients with no surgery,radiotherapy or chemotherapy,the factors affecting the survival rates include primary site,pathological grade,T-stage,N-stage and clinical stage,and of them,T-stage and Nstage are the independent prognostic factors.

Objective To investigate the effects of body mass index (BMI) on 24-hour intraocular pressure (IOP) fluctuations in adolescences.Methods Totally 90 healthy adolescences were included in the study.Measurements of 24-hour IOP were obtained by Goldmann applanation tonometer on 5:00 am,7:00 am,10 ∶ 00 am,2:00 pm,6:00 pm and 10:00 pm.BMI was calculated as weight in kilograms divided by the square of height in meters (kg · m-2).The patients were divided into three groups according to BMI,and they were Group 1 (BMI < 25 kg · m-2),Group 2 (25 ≤ BMI < 30kg · m-2),Group 3 (BMI ≥ 30 kg · m-2).Data were analyzed by statistic software SPSS 22.0.Results The mean of IOP,peak IOP,valley IOP and fluctuation value of IOP were (16.72 ±3.30)mmHg (1 kPa =7.5 mmHg),(19.26 ±3.68)mmHg,(14.29 ±3.26)mmHg and (4.98 ± 1.91)mmHg,respectively.And there was no significant differences in the fluctuation value of IOP among the three groups (all P > 0.05);the differences in mean IOP,peak IOP,valley IOP between group 1 and group 3 were significant (all P <0.05).However,the differences between group 1 and group 2,group 2 and group 3 showed no significant differences (all P > 0.05).Pearson correlation analysis showed that BMI was positively correlated with mean IOP and peak IOP (both P < 0.05).Conclusion In the adolescent population,individuals with higher BMI tend to have higher mean IOP and peak IOP.
10.Clinical features and genetic analysis of three children with β -ketothiolase deficiency

Objective:To explore the clinical features and genetic variants in three children suspected for β-ketothiolase deficiency (BKTD).Methods:Clinical manifestations, laboratory examination and genetic testing of three children suspected for BKTD at Henan Children′s Hospital between January 2018 and October 2022 were collected, and their clinical and genetic variants were retrospectively analyzed.Results:The children were all males with a age from 7 to 11 months. Their clinical manifestations have included poor spirit, shortness of breath, vomiting, convulsions after traumatic stress and/or infection. All of them had severe metabolic acidosis, elevated ketone bodies in blood and urine, hypoglycemia, with increased isoprenyl-carnitine and 3-hydroxyisovalyl-carnitine in the blood, and 2-methyl-3-hydroxybutyrate and methylprotaroyl glycine in the urine. All of them were found to harbor compound heterozygous variants of the ACAT1 gene, including c. 1183G>T and a large fragment deletion (11q22.3-11q23.1) in child 1, c. 121-3C>G and c. 826+ 5_826+ 9delGTGTT in child 2, and c. 928G>C and c. 1142T>C in child 3. The variants harbored by children 2 and 3 were known to be pathogenic or likely pathogenic. The heterozygous c. 1183G>T variant in child 1 was unreported previously and rated as a variant of unknown significance (PM2_Supporting+ PP3+ PP4) based on guidelines from the American College of Medical Genetics and Genomics. The large segment deletion in 11q22.3-11q23.1 has not been included in the DGV Database and was rated as a pathogenic copy number variation. Conclusion:The variants of the ACAT1 gene probably underlay the pathogenesis of BKTD in these three children.
