1.The value of ultrasonographic assessment in the breast dysplasia treated with the traditional Chinese medicine

ObjectiveTo investigate the value of high-frequency ultrasound in assessment of the therapeutic efifcacy of traditional Chinese medicine in patients with breast dysplasia using.MethodsFrom March to September in 2014, 50 patients with breast dysplasia in the Third Affiliated Hospital of Beijing University of Chinese Medicine were enrolled in the study. In addition, they were treated with Xiaozheng pills. Before and after treatment, these patients were examined using ultrasound. The thickness of the breast gland layer in each patient was measured. When the dilated ducts lactiferous, cysts, hypoechoic areas, or nodules of the breast were detected, the diameter or volume was measured or calculated. A grading system of the breast dysplasia was used to evaluate the disease degree, and the quadrants of the breast dysplasia were counted. The thickness of the breast gland layer, the volume of cysts and nodules, the diameter of hypoechoic areas and the dilated ducts lactiferous, the number of the breast dysplasia were compared with the pairedt-test. The grading of the breast dysplasia was compared with rank sum test.ResultsThere were significant decreases in the thickness of the grand layer, the diameters of the ducts lactiferous and the hypoechoic area after treatments [(15.92±4.73) mm vs(13.02±4.20) mm,t=2.240,P<0.01; (2.26±0.58) mm vs (1.71±0.47) mm,t=5.202,P<0.01; (21.14±5.57) mm vs(17.80±4.63) mm,t=5.529,P<0.01]. However, there were no significant differences in the volume of cysts and nodules. The grading of the breast dysplasia showed statistical difference (z=-5.667,P<0.01), and the number of the breast dysplasia quadrants was reduced markedly (5.38±1.85 vs3.28±1.97,t=7.566,P<0.01).ConclusionsUltrasound can be an effective method in assessment of patients with breast dysplasia treated with the traditional Chinese medicine. The assessment can be based on some signs, including the thickness of the grand layer, the diameters of the ducts lactiferous, and the diameters of the hypoechoic area. The grading system and breast quadrants counting in these patients may be added as supplementary criteria to make a complete therapeutic evaluation.
2.Clinical analysis of 45 cases of acute appendicitis in pregnancy

Objective To explore the clinical characteristics,diagnosis and therapeutic strategies of acute appendicitis in pregnancy.Methods The clinical data of 45 cases with acute appendicitis in pregnancy,which had been diagnosed by surgeries in our hospital since Jan 2010 to Jun 2016,were retrospectively analysed.Results All patients had exhibited abdominal pain.Among them,15 patients(33.3%)had exhibited shifting pain in right lower quadrant,and 44 patients(97.8%)had abdominal tenderness.4 cases with appendiceal gangrene and 1 case with necrosis of the great omentum had been found during the surgeries.There were 5 patients had occurred complications after the surgeries,including 2 cases with poor wound healing,1 case with septic shock,1 case with inflammatory ileus and 1 case with incompletely adhesive ileus.Threatened abortion and threatened premature delivery were found in 11 patients(during 22+3 to 36+2 weeks gestation)after the surgeries.Finally,1 patient occurred abortion(25+3 weeks gestation)and 3 patients occurred premature delivery(during 28+4 to 30+4 weeks gestation).Conclusion Abdominal pain as the main clinical manifestations of the pregnancy with acute appendicitis,prone to severe complications.
3.Determination of 5 aphrodisiacs illegally adulterated into Chinese proprietary medicine by LC/MS(n) Trap

AIM: To determine 5 different aphrodisiacs,including apomorphine,yohimbine,vardenafil,sildenafil and alprostadil,adulterated into Chinese proprietary medicine by LC/MS(n),and build a laboratory library including MS(n) spectra and retention time. METHODS: ZORBAX SB C_18 column(150 mm?4.6 mm,5 ?m)was applied.The mobile phase consisted of acetonitrile and 0.5% formic acid changing by linear gradient elution,and the flow rate was 1.0 mL/min.The ion trap mass spectrometer with positive ion mode electrospray(ESI?) was applied. RESULTS: In the mass spectrum apomorphine,yohimbine,vardenafil and sildenafil showed abundant pseudomolecular ion +,and alprostadil showed abundant pseudomolecular ion -.All the MS(n) spectra showed characteristic product ion. CONCLUSION: It is proved that the method is simple,reliable and suitable to determin aphrodisiac adulterated to health food.
4.Ex vivo expansion of regulatory T cells for clinical applications against graft-versus-host disease in allogeneic hematopoietic stem cell transplantation.

OBJECTIVETo review the characteristics of regulatory T cells (Tregs) and ex vivo expansion of Tregs for treatment of graft-versus-host disease (GVHD).
DATA SOURCESThe data used in this review were retrieved from PubMed (1970-2013). The terms "ex vivo expansion", "regulatory T cell", and "graft-versus-host disease" were used for literature search.
STUDY SELECTIONThe publications about the characteristics of Tregs, ex vivo expansion of Tregs and clinical applications of Tregs against GVHD were identified, retrieved and reviewed.
RESULTSTregs can be classified as natural Tregs (nTregs) and induced Tregs (iTregs). Both subsets share most Treg features. Given their immunosuppressive property, Tregs have been tested for their capability of preventing GVHD. The bottleneck of Treg therapy is the limited numbers of naturally existing Tregs. To solve this problem, ex vivo expansion of nTregs or iTregs has been executed. The initial data indicate Treg therapy is effective in reducing GVHD without compromising graft-versus-leukemia (GVL).
CONCLUSIONEx vivo expansion of Tregs is a reliable way to prepare sufficient number of Tregs for management of GVHD.

5.Analysis on Flocculation Characteristics of Bioflocculant-producting Bacteria TJ-3

A strain of high-efficiency bioflocculant-producting bacteria,which was named TJ-3,was screened from sewage sludge.The strain was gram-negative and rod-shaped in physiological biochemical test and was identified as Pseudomonas aeruginosa by 16S rDNA Sequencing.According to the growth curve of the TJ-3 strain,the growth stabilization period is long so that Microbial Flocculants(MBF) production is stable.The MBF produced by the TJ-3 strain is able to flocculante kaolin suspension with 98.2%.The MBF activity distribution tests show that most of the active components of the bioflocculant exist in deposition after centrifufation.When pH is 8.5,1%(quality fraction) CaCl2 Solution dosing quantity is 3.5 mL,bioflocculant dosing quantity is 1.5 mL in 100 mL kaolin suspension,the bioflocculant has an optimal flocculation.
6.Incipient Coombs' test negative autoimmune hemolytic anemia precedes non-Hodgkin's lymphoma.

The cases of lymphoma accompanied or preceded by Coombs' test positive autoimmune hemolytic anemia (AIHA) have been reported. However, Coombs' test negative AIHA prior to the diagnosis of lymphoma was rarely described. Herein, this article reports a case of non-Hodgkin's lymphoma (NHL) preceded about 1.5 years by Coombs test negative AIHA. A woman aged 69 was diagnosed with HA based on the history and laboratory tests. Further studies revealed that this patient was negative with Coombs' test for IgG, IgM, IgA and C3. After all possible causes of HA, especially malignancies were ruled out, the patient was diagnosed with Coombs' test negative AIHA and treated with prednisolone. The patient responded well initially to steroid treatment. Two recurrences of acute HA were presented at time of 10 months post steroid cessation, and immediately after an attempt to withdraw steroid, respectively, but the hemolysis was effectively controlled by reinstitution of prednisolone. At third recurrence, however, the patient was no longer responding to steroid, and was found with cervical lymphadenopathy. Coombs' test for IgG, IgM, IgA and C3 remained negative. B cell NHL was diagnosed by pathology. After receiving 6 cycles of CHOP chemotherapy, the patient was lymphoma free, but the hemolysis was not improved, however, which was effectively controlled by the following low dose-rituximab (RTX) therapy. The patient was still kept in a remission of lymphoma free of anemia. In conclusion, this report presented a very rare case of NHL with Coombs' test negative AIHA as initial major clinical manifestation.

7.Effect of Small Knife Needle on β-enorpin and Enkehalin Contents of Tansverse Process Syndrome of the Third Vertebra.

OBJECTIVETo explore the analgesic mechanism of small knife needle for treating transverse process syndrome of the third vertebra (TPSTV) by observing peripheral and central changesof β-endorphin (β-EP) and enkephalin (ENK) contents.
METHODSTotally 30 Japanese white big-ear rabbits of clean grade were divided into 5 groups according to random digit table, i.e., the normal control group, the model group, the small knife needle group, the electroacupunture (EA) group, and the small knife needle plus EA group, 6 in each group. The TPSTV model was established by inserting a piece of gelatin sponge into the left transverse process of 3rd lumbar vertebrae. Rabbits in the small knife needlegroup were intervened by small knife needle. Those in the EA group were intervened by EA at bilateralWeizhong (BL40). Those in the small knife needle plus EA group were intervened by small knife needleand EA at bilateral Weizhong (BL40). Contents of β-EP and ENK in plasma, muscle, spinal cord, and hypothalamus were determined after sample collection at day 28 after modeling.
RESULTSCompared with the normal control group, contents of β-EP and ENK in plasma and muscle increased significantly, and contents of β-EP and ENK in spinal cord and hypothalamus decreased significantly in the model group (P < 0.05, P < 0.01). Contents of β-EP and ENK approximated normal levels in the three treatment groups after respective treatment. Compared with the model group, the content of β-EP in muscle decreased, and contents of β-EP and ENK in hypothalamus increased in the three treatment groups after respective treatment (P < 0.05). There were no significant difference among the three treatment groups (P > 0.05).
CONCLUSIONSSmall knife needle treatment and EA had benign regulation on peripheral and central β-EP and ENK in TPSTV rabbits. Small knife needle treatment showed better effect than that of EA.

9.Effect of ginkgolide B on plasma levels of interleukin-12 and PAF in severe acute pancreatitis in rats.

AIMTo observe the levels of interleukin-12 (IL-12) and platelet activating factor (PAF) in severe acute pancreatitis (SAP) in rats and the efficacy of Ginkgolide B (BN52021) in treating SAP.
METHODSWistar rats were randomly divided into 3 groups: model group (SAP), treatment group (BN) and negative control group (NC). SAP was induced by retrograde infusion of 5% sodium taurocholate into the pancreatic duct in Wistar rats. NC rats only receive abdominal incision. In groups of SAP and NC rats received the femoral vein injection of isotonic Na chloride 15 minutes after induction of SAP; in BN group,rats received BN52021 instead. After operation rats were sacrificed at 1, 6 and 12 hour for plasma IL-12 and PAF determined with ELISA.
RESULTSAn increase of IL-12 in group BN was observed VS group SAP or group NC at 1 h stage (p = 0.011, P < 0.01). At 6 h or 12 h stage,an increase of IL-12 in group SAP was observed VS group NC (P < 0.01, P < 0.05). The plasma level of PAF in group SAP or group BN was increased significantly at 1 h time stage VS group NC (P < 0.001). At 6 h or 12 h stage, a decrease of PAF in group BN or group NC was observed VS group SAP (P < 0.05, P < 0.01).
CONCLUSIONIt confirmed that the plasma level of cytokine IL-12 in SAP group was decreased significantly in early stage and it witnessed a remarkable increase of cytokine PAF. The plasma level of IL-12 was increased in early stage but PAF was decreased in rats treatment by BN52021 which inhibited the development of SAP.

10.Percutaneous vertebroplasty in treatment of compression fracture of thoracic and lumbar vertebrae

Objective To discuss the value and experience of the percutaneous vertebroplasty (PVP)in the treatment of vertebral body compression fracture(VCF)in aged osteoperosis.Methods PVP was performed in 44 cases with VCF including 28 with single vertebral compressed fracture,12 with double compressed fracture and four with triple compressed fracture,with 67 vertebrae,for clinical and radiologieal evaluation.Results The mean follow-up was 15 months(4-23 months).There could be seen immediate relief of pain in 40 cases,out-of-bed activities at operation day in 19 and out-of-bed activ- ities at second day after operation in 25.Postoperative X-ray showed uniformly distributed bone cement in the vertebral,without leakage.Conclusion PVP is a recommendable method for VCF,for it has ad- vantages of pain relief,vertebrae stabilization,minimal invasion and minor complications.
