1.A Case of Rett Syndrome Observed with Video-EEG Monitoring.

Rett syndrome is progressive neurodegenerative disorder in female patients, characterized by autistic behavior, mental retardation, loss of purposeful hand skills, stereotypic hand movement, breathing dysfunction, severely impaired language, ataxia, and seizure. The diagnosis of Rett syndrome is based on its characteristic clinical manifestation and course. The electroencephalographic (EEG) findings of Rett syndrome are nonspecific, but a progressive deterioration in the EEG, characterized by a slowing of background activity and spike sharp wave discharges, may be observed. We experienced one case of Rett syndrome in a 5 year old girl having mental retardation, loss of purposeful hand skills, stereotypic hand movements (clapping, washing, hand-to-mouth), breathing dysfunction (hyperventilation/apnea). Her EEG findings on Video-EEG monitoring are excessive slowing waves during awake state and frequent spike discharges from left or centrotemporal area during sleeping. We report a case of Rett syndrome with brief review of related literatures.

7.Clinical Analysis of Diverticulosis of the Cecum and Ascending Colon.

BACKGROUND: Diverticulosis of the cecum and ascending colon is a rare disease in Western countries, it is more common in the Orient. Making the diagnosis preoperatively could be difficult, and the intraoperative assessment and decision of optimal treatments are difficult. METHODS: Therefore, we reviewed the patient's charts of diverticulosis of the cecum and ascending colon at our hospital from 1992 to 1997. RESULTS: During the past 5 years, a total of 53 cases of diverticulosis of the cecum and ascending colon had been treated at this hospital. Mean age was 41.8 years (range 20-70). The male to female ratio was 1.8 : 1. Frequent complaints were abdominal pain (46 cases, 86.8%), followed by diarrhea (5 cases, 9.3%), indigestion (4 cases, 7.5%), and bowel habits change (2 cases, 3.8%). Mean duration of illness was 3.5 days(range 24 hours~15 days). In non-surgical cases, diagnoses were established with barium enema, CT scan, or ultrasonography. The accuracy of these methods was 91%, 75%, and 25%. In surgical cases, the preoperative diagnoses were appendicitis(13 cases, 50.1%), diverticulitis (7 cases, 31.8%), and abscess (2 cases, 9.1%). Treatments of the cecum and ascending colon diverticulitis were conservative management (3 teases,58.5%) and surgical treatment (22 cases,41.5%). The operative procedures were right hemicolectomies (2 cases), ileocecal resections (2 cases), diverticulectomies (6 cases) and appendectomies only (11 cases). CONCLUSION: These results suggest that correct diagnosis of the diverticulitis of the cecum and ascending colon made before treatment is very important because correct assessment of diverticulitis intraoperatively is difacut. Liberal use of diagnostic modalities could facilitate to make correct diagnosis to set a proper plan for treatment.

8.The Diagnostic Accuracy of the 64-slice Multi-detector CT Coronary Angiography for the Assessment of Coronary Artery Stenosis in Symptomatic Patients.

PURPOSE: We evaluated the diagnostic accuracy of a 64-slice multi-detector CT (MDCT) coronary angiography against a conventional coronary angiography (CCA) for the detection of significant stenosis (> or =50% lumen diameter narrowing). MATERIALS AND METHODS: Sixty-four patients underwent a MDCT and a subsequent CCA to evaluate the presence of atypical chest pain or suspected coronary artery disease (CAD). A MDCT angiography was performed using a 64-slice MDCT-scanner (Sensation 64, slice collimation 32x0.6 mm). The coronary artery segments were classified according to a 15-segment model. The sensitivity, specificity, and diagnostic accuracy of the 64-slice MDCT for the detection or exclusion of significant CAD were calculated on a per-segment and per-patient basis. RESULTS: Fifty-nine of the 64 (92%) coronary CT angiograms were of diagnostic image quality with 93.5% (809 of 865) of the coronary segments assessable by CT angiography. One-hundred two (12.6%) segments showed significant stenosis by CCA. Stenosis of 50% or greater was detected by sensitivity, specificity, accuracy, positive predictive valve, and negative predictive value on a per segment basis (89%, 99%, 97%, 90%, and 98%, respectively) and a per-patient basis (96%, 69%, 90%, 92%, and 82%, respectively). CONCLUSION: The 64-slice MDCT coronary angiography demonstrated a high diagnostic accuracy for both the per-segment and per-patient analyses for this symptomatic patient group.

9.The Study of Distribution of HI Antibody Titers Aganist Japanese Encephalitis Virus Among Children in Seoul.

PURPOSE: Incidence of Japanese encephalitis has been markedly decreased, but this disease is known to be high and lethal in the young age group. This study was designed to find out the HI antibody titers and associated factors of Japanese encephalitis of children in Seoul, and to establish the plan of vaccination. METHODS: The survey was conducted from the end of May to the beginning of June, 1992. Serum of total 862 children aging from 4 year to 15 year in the Kangseo-ku and Songpa-ku, Seoul wre obtained. And we analyzed the HI antibody titers to Japanese encephalitis virus. RESULTS: 1) The percentage of childrens with the antibody titer higher than 1:20 was 34.5%. 2) The protective HI antibody to Japanese encephalitis virus by sex revealed that the percentage of males was 47% while that of females was 29.2% 3) The protective HI antibody titer to Japanese encephalitis was higher in the younger age than older age. 4) The comparative studies on HI antibody titer of Japanese encephalitis virus by vaccination types revealed that the percentage of children without vaccination was 0%, the percentage of children with single vaccination at 2 years ago was 9.7%, single vaccination at 1 year ago 31.6%, and single vaccination at this year 39.1%. According to serial vaccination at 1990 and 1991, 1991 and 1992, the percentage of protective HI antibody titer to Japanese encephalitis virus was 22.4%, 44.8%, respectively. 5) The factors influenced on protective antibody titer to Japanese encephalitis virus were age and vacciantion history at 1991 and 1992. CONCLUSIONS: Effective antibody titers to Japanese encephalitis virus of children in Seoul were low and the important factors to maintain protective HI antibody titers were age and recent vaccination history. Therefore regular vaccination of Japanese encephlitis virus is thought to be important in preventing the Japanese encephalitis.

10.Biliary Cystadenocarcinoma with Early Gallbladder Cancer: A Case Report.

The biliary cystadenocarcinomas are very rare malignant tumor. The etiology is unclear, this tumor may be develop from malignant transformation of begin cystadenoma. The tumor appears typically as a cystic mass which contains papillary projections or internal septations. Papillary projections are seen on the inner surface of the cyst which present numerous papillary protrusion typical to this rare tumor. Excessive mucin secretion produced by the tumor cells and its retention in the common bile duct may cause bile stasis leading to obstructive jaundice. Common presenting symptoms are abdominal fullness, pain, nausea, palpable mass and asymptomatic. The available diagnostic tools are CT and sonography, but histologic examination can be made diagnostic confirm. Surgical resection yields excellent results. We were experienced a cystadecarcinoma with early gallbladder cancer, then report with reference.
