Chronic compression or ischemia of the spinal cord in the cervical spine causes a clinical syndrome known as cervical myelopathy. Recently, a new term “degenerative cervical myelopathy (DCM)” was introduced. DCM encompasses spondylosis, intervertebral disk herniation, facet arthrosis, ligamentous hypertrophy, calcification, and ossification. The pathophysiology of DCM includes structural and functional abnormalities of the spinal cord caused by static and dynamic factors. In nonoperative patients, cervical myelopathy has a poor prognosis. Surgical treatments, such as anterior or posterior decompression accompanying arthrodesis, arthroplasty, or laminoplasty, should be considered for patients with chronic progressive cervical myelopathy. Surgical decompression can prevent the progression of myelopathy and improve the neurologic status, functional outcomes, and quality of life, irrespective of differences in medical systems and sociocultural determinants of health. The anterior surgical approach to the cervical spine has the advantage of removing or floating the intervertebral disk, osteophytes, and ossification of the posterior longitudinal ligament that compress the spinal cord directly. The posterior surgical approach to the cervical spine is mainly used for multisegment spinal cord compression in patients with cervical lordosis. In this review article, we addressed the pathophysiology, clinical manifestations, differential diagnosis, and treatment options for DCM.

STUDY DESIGN: Case report OBJECTIVES: To report a case of Brown-Séquard syndrome after blunt cervical trauma. SUMMARY OF LITERATURE REVIEW: Brown-Séquard syndrome is a rare disease characterized by hemisection of the spinal cord, and it shows the best prognosis of the various types of incomplete spinal cord injuries. MATERIALS AND METHODS: A patient with Brown-Séquard syndrome that occurred after a traffic accident was followed up for 2 years and 6 months. RESULTS: We observed normal recovery of motor strength, but sensory impairment and deep tendon hyperreflexia remained. CONCLUSIONS: Brown-Séquard syndrome is known to have a good prognosis, but in this case, the neurological abnormality did not fully recover; therefore, we report this rare case and present a review of the literature.

Artichia with papular lesions is a rare congenit,al disease characteriezed by the association of alopecia and papular lesions. We report a case of atrichia with papular lesions in 12-year-old. Alopecia of the scalp and eye brows were present at birth. Nurnerous papular lesions are presentation the trunk, buttock and inguinal area. The histopathologic iinding of a papular lesion showed ker till-filled cyst, and eruptive vellous hair cyst. The patient showed reticulat.ed hyperpigmentation on the hands and feet in addition to the characteristic feature of atriclia with papular lesions.

The effect of saponin on the Na+ - K+ ATPase of anterior capsule and epithelium of cattle lens has been investigated. The experiment also designed to determine the cation of saponin on the Na+ - K+ ATPase activity in anterior capsule and epithelium of cattle lens. The following results are observed. 1. The Na+ - K+ ATPase activity of cattle lens epithelial cell membrane is inhibited by low concentration of saponin, but increased by high concentration. The strongest activity showed at the dose of 50mg% saponin concentration. 2. The activating effect of saponin on the cattle epithelial cell membrane Na+ - K+ ATPase activity is inhibited by ouabain, but the stimulation of the Mg++ ATPase by high concentration of saponin is not inhibited by ouabain. 3. The activity ratio of Na+ - K+ ATPase by high concentration of saponin is increased by raising the sodium concentration but decreased by raising the potassium concentration. 4. The Na+ - K+ ATPase activity is increased by small amount of calcium but completely inhibited by over 5mM concentration of calcium.

A case of osteochondritis dissecans of both knee joints is developed in 15 year old boy. Since the original articles of Paget, Koning and others were reported, many theories as to etiology and pathogenesis have been proposed but exact cause has been unknown. Many Joint may show evidence of osteochondritis dissecans but lateral aspect of medial femoral condyle of knee is by far the most frequent site. Under the diagnosis of osteochondritis dissecans of both knees with clinical symptoms and X-ray finding, the one was treated with excision of framents and curettage, and the other treated with drilling of fragments. In follow up study for one year, the result was good without any complaints.

It is generally accepted that the integrity of posterior cruciate ligament plays a major role in knee stability. The rotatory movement is around a point to the back of center of joint where the posterior cruciate ligament lies, and it is a structure which is just as important as the other ligaments. Methods of reconstruction have been described, using semitendinosus tendon, gracilis tendon, popliteus tendon, patellar tendon, iliotibial band, and medial meniscus. We have successfully used the semitendinosus tendon in two patients who sustained neglected rupture of posterior cruciate ligament.

Multiple cerebral infarction developed in a 36-year-old woman with adenocarcinoma of the right lung with direct cardiac invasion. Neurological examination and brain MRI showed findings of ordinary infarction. Transesophageal echocardiography showed left atrial pedunculated mass which was the potential source of embolism. We report a rare case who had such multiple spontaneous tumor emboli large enough to result in clinically detectable cerebral infarction.
