1.Clinical observation on Nd:YAG laser for treating high-risk patients with anterior capsule contraction after cataract operation

Objective To observe the clinical effect of anterior capsule relaxing incisions with Nd:YAG laser in treating postop-erative anterior capsule contraction in high-risk cataract patients.Methods 120 high-risk cataract patients with postoperative ante-rior capsule contraction collected from December 2010 to November 2012 were divided into experimental group(performed the ante-rior capsule relaxing incisions on postoperative 3 d)and control group.The anterior capsule opening area was measured immediately after laser operation(baseline),in postoperative 1,3,6 months by using the Scheimpflug video photogeaphy system,and the descent rate of the opening area was simultaneously calculated.The decentration degree and tilt of intraocular lens(IOL),posterior capsule opacification and other complications were also assessed.Results Of the observed 120 cases,40 cases were pseudo posterior capsule stripping syndrome,40 cases were primary angle closure and 40 cases were diabetic retinopathy.There was no statistically signifi-cant difference in the baseline value of the opening area between the experimental group and the control group(P>0.05).In the ca-ses of primary angle closure 1,3,6 months after operation,the anterior capsule opening area in the experimental group was signifi-cantly increased compared with the control group(P<0.05)and the descent rate of the opening area was significantly diminished (P<0.05).In the cases of pseudo posterior capsule stripping syndrome or diabetic retinopathy,although there was no statistically significant difference in anterior capsule opening area between the experimental group and the control group(P>0.05),the descent rate of the opening area was significantly diminished(P<0.05).No significant differences were found in the aspects of IOL decen-tration and tilt,posterior capsule opacity,or other complications.Conclusion YAG laser anterior capsule relaxing incisions in the early period after cataract surgery is safe and effective in preventing anterior capsule contraction in high-risk patients.
2.Effect analysis on fiberbronchoscope management under life cycle management mode

Objective To ensure the quality of disinfection and sterilization of the fiberbronchoscope in the sterilesupply center through the implementation of whole life cycle management.Methods Using the historical comparative method,the fiberbronchoscopes managed by the departments were enrolled into a control group from February 2013 to March 2014,and the ones undergoing life-cycle management in the sterile supply center were involved in an observation group from April 2014 to May 2015.The two groups were divided according to the management subject and mode,and compared from the aspects of treatment flow,process quality control and record tracing validity.All the data were input with Excel sheet,andanalyzed statistically with SPSS 17.0.Results The observation group and the control group had the numbers of positive results in internal cavities,positive results in external surface,damaged or irrationally-placed package,invalid data tracing and non-standardized storage being(0,1.11±0.15),(0,1.09±0.20),(1.07±0.13,2.75±0.22),(0.57±0.03,1.53±0.31) and (0.13±0.07,0.95±0.21) respectively,and the differences between the two groups were significant (P<0.05).The turnover of the fiberbronchoscope was enhanced significantly in the observation group when compared with that in the control group.Conclusion Life cycle management contributes to strengthening quality control of the treatment of polluted fiberbronchoscope,and decreases the incidences for hospital infection.

OBJECTIVE:
To introduce the efficacy of three surgical options for juvenile nasopharyngeal angiofibroma (JNA) resection, and causes of operative bleeding.
METHOD:
Retrospective analysis of 36 JNAs,three surgical options were used to resect the tumor. There were 15 cases of Class I tumors , using endoscopic nasal cavity approach. Eighteen cases of class II tumors, via extended Caldwell-Luk incision, using the transantral-infratemporal fosse-nasal cavity combined approach for tumor resection. Three cases of class III tumors, the combined intracranial and extra-cranial approach was used to resect the tumor. Meanwhile, report six typical cases for reference.
RESULT:
Fifteen (15/36) cases of class I tumors, 14 cases were completely resected for the first time without recurrence, 1 recurrence case was re-resected using the same approach. Eighteen (18/36) cases of class II tumors, 13 cases were completely resected for the first time without recurrence, 5 recurrence cases were re-resected totally. Three (3/36) cases of class III were not completely removed, and underwent about 40 Gy radiotherapy with good effects.
CONCLUSION
Using these three surgical options can effectively remove different types of JNA. When necessary, the intracranial residue can use radiotherapy. Under direct vision to separate the tumor, and effective hemostasis play crucial roles for complete removal of the tumor.

9.Study on winter dormancy of Thesium chinense and its phenological phase.

In order to explore reasonable artificial cultivation pattern of Thesium chinense, the biological characteristics and nutrients change in the process of winter dormancy of T. chinense was studied. The phenological period of T. chinense was observed by using fixed-point notation and the starch grains changes were determined dynamically by PAS-vanadium iron hematoxylin staixjing method. Soluble sugar and starch content were measured by anthrone-sulfuric acid method and amylase activity was determined by DN'S method. The results showed that the normal life cycle of T. chinense was two years. T. chinense was growing by seed in the first year, but growing by the root neck bud in the second year. During the process of dormancy, starch and soluble sugar could mutual transformation in different periods. T. chinense had sufficient carbohydrate to maintain growth and also a lot of small molecules to improve their ability to fight against adversity.

10.Treatment of unstable middle 1/3 of clavicular fractures with composite external fixator in adult patients.

OBJECTIVETo explore methods and clinical effects of composite external fixtor in treating adult patients with unstable middle 1/3 of clavicular fractures.
METHODSFrom March 2008 to March 2011,36 patients with unstable middle 1/3 of clavicular fractures were treated with composite external fixtor. There were 24 males and 12 females, aged from 18 to 55 years old with an average of 43 years old. Twenty cases on the left side and 16 cases on the right side. Time from injury to operation was 2 to 6 days (averaged 3.5 days). According to Robinson classification, there were 7 cases with type 2A2, 18 cases with type 2B1, and 11 cases with type 2B2. No vessels and nerve damage occurred before opreation. The clinical effects were evaluated according to Neer scoring.
RESULTSAll cases were followed up from 6 to 12 months with an average of 8 months. The mean Neer score was 88.3 +/- 6.2, which included pain 31.6 +/- 3.2, functional score 25.7 +/- 2.2, range of motion score 21.1 +/- 1.7, and anatomy score 8.8 +/- 0.8. There were 22 cases in excellent, good in 13, fair in 1. Two cases occurred pin tract infection.
CONCLUSIONComposite external fixtor is an optional method in treating unstable middle 1/3 of clavicular fracture, and can obtain a good clinical effects.
