1.A Case of Rapid Growing Breast Cancer Associated with Pregnancy.

Pregnancy associated breast cancer is defined as breast cancer diagnosed during pregnancy or in the 12 months postpartum. The incidence is one of the most common carcinomas and approximately 1 in 3,000 pregnancies in the United States. Pregnant women are at a higher risk of presenting with more advanced disease than nonpregnant women since small lumps cannot be easily detected due to the natural tenderness and engorgement of the breasts during pregnancy and lactation. Average diagnosis delays of 5 to 7 months are reported. The prognosis is thought not to be significantly different from non-pregnancy associated breast cancer, except in cases where a delay in diagnosis is associated with more advanced disease. Therefore breast cancer is no evidence to implicate pregnancy or lactation in either the etiology and the progression. The strategies for therapeutic management of breast cancer associated pregnancy are mainly dependent on early diagnosis and the stage with gestational age. We experienced a patient who underwent a modified radical mastectomy and adjuvant chemotherapy due to rapid growing breast cancer in pregnant. We report this case with a brief review of literature.

3.Experience of Microsurgery Using Dorsalis Pedis Artery

Since the introduction of surgical microscope in microvesael surgery by Jacobson and Suarez in 1960, many surgeons have succeeded replantation, transplantation of composite segment of tissues so called free fiap-free bone graft and toe to hand transfer. McCraw & Furlow reported successfully transfered dorsal foot flap using dorsalis pedis artery in 1975 and Cobett transfered great toe to band for reconstruction of the amputated thunb. The authors experienced six cases of microsurgery using dorsalis pedis artery durig the recent two years in the department of Orthopaedic surgery of Soon Chun Hyang College and results in this paper. 1. Four cases out of six were dorsalis pedis free flap, one case was second toe to thumb and tbe other one was reconstruction of an amputated thumb in one stage using iliac bone graft and dorsalis pedis flap. 2. One case out of four cases of dorsalis pedis free flap was performed for reconstruction of contracted first web and the other cases were performed for foot. 3. Five cases out of six were successfully transfeed, one case which was toe to thumb was failed. The cause. of fail was probably due to post-operative hematoma. 4. Composite tissue using dorsalis pedis artery is one of the good donor site for composite tissue transfer for not only skin defect and scar contracture of the hand and foot but also reconstruction of the amputated fingers because it has several advantages; an acceptable thickness, a constant arterial supply, venous drainage through the saphenous system, and constant innervation through the terminal branches of the superficial and deep peroneal nerve.

4.Characteristics of Shigella sonnei Isolated in Kyungsangbuk-do in 1998: Biochemical Characteristics, Biotyping, Antibiotic Resistance Pattern, and Plasmid Profile.

Thirty nine strains and 109 strains of Shigella sonnei were isolated from the outbreaks of Youngchun and Kyungju, respectively, while 15 strains were isolated from sporadic cases of eight regions in Kyungbuk province from September to November in 1998. We investigated the relationship among the S. sonnei strains by using biochemical characteristics, biotyping, antibiotic resistance pattern, and plasmid profile. Among the isolates, seven strains of S. sonnei isolated in Youngchun showed gelatin hydrolyase positive but the others showed gelatin hydrolyase negative. One hundred and fifty two strains were a type, while eleven among thirty nine strains isolated in Youngchun were g type. Antibiotics resistance patterns of S. sonnei strains isolated in Youngchun and Kyungju were significantly different. Thirty nine strains of S. sonnei isolated in Youngchun were resistant to SM, TE, and TMP/SMX, while eighty six of S. sonnei among one hundred and nine strains isolated in Kyungju were resistant to AM, CB, K, SM, TE, and TMP/SMX. Antibiotics resistance patterns of residual twenty three isolates were similar to those of eighty six strains. The Plasmid profiles of strains of S. sonnei isolated from the Kyungju were different from those of S. sonnei strains isolated in Youngchun. The Plasmid profiles of S. sonnei strains isolated from Youngchun were identical to those of a S. sonnei strains randomly selected from the outbreak in Daegu in 1998. The Plasmid profiles of S. sonnei strains isolated from Kyungju were identical to those of two strains of S. sonnei randomly selected from the outbreaks of Kanglung and Wonju in 1998. From the above results, it is considered that the strains of S. sonnei isolated from Kyungju and Youngchun region are not identical clone.

5.Ovarian carcinoma initially presenting as enlargement of axillary lymph node.

Ovarian carcinoma is usually asymptomatic in early stage and over 70% of it initially diagnose advanced stage due to diffuse intraabdominal disease. Presenting symptoms and signs often relate to the degree of intraabdominal spread. The most common presenting symptoms and sign include abdominal pain, distension and palpable pelvic mass. The enlarged finding of isolated, distant lymph node without intraabdominal symptom and sign is vary rare. We experienced a patient who initially diagnosed enlarged axillary lymph node from ovarian carcinoma. We report this case with a brief review of literature.

6.A Clinical Analysis on the Pineal Region Tumors.

The authors analyzed 24 cases of the pineal region tumors which were treated at the Department of Neurosurgery, Yonsei University during the last eight years. Tumors of the pineal region constitutes 2.5% of all brain tumors in authors' series. Most(79.1%) of the patients were younger than 20. Of these 24 cases, 19 were male and 5 were female. Presenting symptoms were headache, vomitting and clouding of sonsciousness due to obstructive hydrocephalus, followed by eye signs, such as Parinaud's syndrome, abducens palsy, and papilledema. Other signs are ataxia and hemiplegia. Plain skull films showed abnormal calcifications and findings of increased intracranial pressure. Location and size of the tumors were most accurately visualized by the brain CT scan. Pathological types of the pineal tumors could be differentiated by the brain CT scan from the patterns of contrast enhancement and tumor density. Among the 24 patients received VP shunt surgery for hydrocephalus. Eighteen patients had the radiation therapy either after the surgery or radiation alone. Germinoma was the most frequent type of pineal region tumors in this series, accounting for half of the patients who had the surgery. There was no immediate postoperative mortality, but one patient with pineoblastoma died from multiple spinal seeding nine months after the surgery. In conclusion, it is most important to identify the pathological type of the tumors in pineal region, because the prognosis of the patient as well as the mode of treatment varies according to the pathological diagnosis. Stereotaxic biopsy of the pineal region tumors is the most accurate diagnostic methods in authors' series among such other methods as brain CT scan, CSF cytology and/or tumor marker study.

7.A Retroauricular Cholesteatoma in Soft Tissue after Tympanomastoidectomy.

As a postoperative complication, the recurrence of aural cholesteatoma is well documented. On rare occasions, a recurred cholesteatoma can penetrate the adjacent soft tissues instead of middle ear or extemal auditory canal. Recently, authors experienced a cholesteatoma in soft tissues of posteriorinferior portion of auricle, developing 8 years after an intact bridge mastoidectomy. We reviewed the reported case of cholesteatoma in soft tissues.

8.Longjintonglin Capsules for type IIIA prostatitis accompanied by abnormal semen liquefaction: A clinical observation.

OBJECTIVETo evaluate the therapeutic effect of Longjintonglin Capsules on type IIIA prostatitis accompanied by abnormal semen liquefaction.
METHODSWe selected 140 patients with type IIIA prostatitis accompanied by abnormal semen liquefaction according to the diagnostic standards of the American Institutes of Health (NIH) and treated them with Longjintonglin Capsules orally 3 capsules once tid for 12 weeks. We obtained the NIH Chronic Prostatitis Symptom Indexes (NIH-CPSI), traditional Chinese medicine (TCM) syndrome scores, leukocyte count in the expressed prostatic secretion (EPS), semen liquefaction time, and the results of semen analysis and compared these indicators before and after the treatment.
RESULTSOf the 140 cases, 132 were included in this study, excluding 8 due to their incomplete case histories. Before and after 4, 8 and 12 weeks of medication, the total NIH-CPSI scores were 24.52 ± 5.43, 21.28 ± 4.85, 18.01 ± 4.28, and 14.49 ± 3.65 (P < 0.01), the TCM syndrome scores were 35.63 ± 6.07, 26.66 ± 5.03, 17.37 ± 4.18, and 11.11 ± 3.96 (P < 0.01), and the leukocyte counts (/HP) were 27.50 ± 7.01, 22.38 ± 5.22, 16:76 ± 4.10, and 11.40 ± 4.74 (P < 0.01), respectively. After 12 weeks of treatment, 31 of the patients with type IIIA prostatitis were cured and another 72 well responded, with an overall response rate of 78.0%. Of those with abnormal semen liquefaction, 61 were cured, 39 well responded, and 32 failed to respond, with an overall effectiveness rate of 75.8%. Semen analysis showed significantly increased percentage of progressively motile sperm after 4, 8 and 12 weeks of medication as compared with the baseline (P < 0.01). No abnormal liver or renal function or other adverse reactions were observed during the treatment.
CONCLUSIONLongjintonglin Capsules, with its advantages of safety, effectiveness and no obvious adverse effects, deserve to be recommended for the treatment of type IIIA prostatitis accompanied by abnormal semen liquefaction.

9.Muscle Strength and Functional Capacity after Arthroscopic ACL Reconstruction using Patellar Tendon Autograft.

Traditionally, evaluation after ACL reconstruction has been focused on physical characteristics and measures of knee stability. Recently, however, reliance on such criteria has been refuted based on the lack of a strong relationship between these measures and both the patient s perception of knee function and return to sports activity. In present study, preoperative and postoperative physical examinations, isokinetic dynamometer, arthrometer test and functional tests were performed on 17 patients with chronic anterior cruciate ligament tears treated by arthroscopic reconstruction using autogenous bone-patellar tendon-bone graft with at least 1 year of follow-up. Muscle power, joint stability and functional recovery were checked for functional evaluation by specific methods. Lysholm score rating scale and functional tests (single leg hop test, vertical jump test, timed single jump test) were performed preoperatively and 6, 12 months postoperatively. Pivot-shift test, Lachman test, anterior drawer test, thigh circumference index and Cybex II+ isokinetic dynamometer test were done preoperatively and at 3, 6, 12 months postoperatively. KT-2000 arthrometer test was done at last follow-up. Lysholm score has showed increasing tendency at 6, 12 months postoperatively (p < 0.01). In Cybex study, deficit percentage of peak torque and total work of the quadriceps and hamstrings was same or slightly decreased at 3, 6 months, but have decreased at 1 year follow-up (p < 0.01). Functional tests and physical examinations have also showed improving tendency at 12 months (p < 0.01). Arthrometer test revealed no significant laxity of the knee joint at last follow-up. In conclusion, muscle strengh and functional capacity in ACL reconstructed knee showed significant improvement at postoperative one year compared with preoperative status. It was considered that arthroscopic reconstruction using autogenous bone-patellar tendon-bone graft is one of the useful method for chronic anterior cruciate ligament injury.
