1.Isolation of ureaplasma urealyticum from patient with genitourinary tract infection attending urologic clinic.

Since the discovery of U. urealyticum in human. the etiologic role of this microbe to urogenital tract infection has been investigated. U. urealyticum is one of the etiologic agents for such infections of human urogenital tract as nongonococcal urethritis. prostatitis. cystitis. infertility. interauterine infection, in which standard bacteriologic investigation had failed to indicate bacterial infection. We studied the occurrence of U. urealyticum in urogenital tract of persons in the district of Pusan. From March, 1990 to June. 1990, 290 heterosexual patients and 102 normal control were cultured for U. urealyticum. The results were as follows : l. Overall incidence of this organism was 19.3%, 32.2% from urethral swab and 10.5% from urine, which was lower than that of control group. 2. The incidence of this organism in Females (32.7%) was generally higher than that in males ( 16.6%). 3. This microbe was well detected in middle age person. 4. This microbe was higher detected in specimen from urethral swab (32.2%) than urine (10.5%). 5. The isolation ratio in local private clinics was lower than that in Kosin medical center. 6. Erythromycin had the best sensitivity against U. urealyticum followed by streptomycin. But tetracycline and oxytetacycline were highly resistant to this organism.

2.A Case of Papillary Type of Renal Cell Carcinoma after Renal Injury in a Child.

Renal cell carcinoma is an uncommon tumor in children and only 3-5 percent of the renal cell carcinoma occur under age of twenty. Interestingly, some of the patients had a history of trauma occurring shortly before the onset of symptom calling attention to the tumor. In histology of renal cell carcinoma, clear cell type is most common and papillary type is 10-15 percent. The papillary type of renal cell carcinoma is characterized by better prognosis than non-papillary types. We report a case of papillary type of renal cell carcinoma which was detected incidentally after trauma in 13-year-old male patient.

6.Clinical Results of the Q-Switched Nd-YAG Laser Iridotomy.

In order to evaluate the effect of Q-switched YAG laser iridotomy to the intraocular pressure, we performed the Q-switched Nd-YAG laser iridotomy in 39 eyes of 28 patients with angle closure glaucoma. Postoperative difference of the intraocular pressure was 27.5 +/- 27.0mmHg and the intraocular pressure was reduced. After 2 months, 12 eyes had needed one kind of anti-glaucomatous medication, 7 eyes had needed no medication, 6 eyes had needed two medications and 1 eye had needed three medications. Postoperative complications were iris bleeding(10 eyes), transient pressure rising(9 eyes), focal corneal opacities(2 eyes) and closure of iridotomy(2 eyes). These result shows that laser iridotomy should be considered in the management of angle closure glaucoma.

9.The Change of Corneal Astigmatism Following Varying Incision Locations and Methods for Cataract Surgery in Case of Against-the-Rule Astigmatism.

Phacoemulsification and foldable PCL implantation was done on 51 eyes with preoperative against-the-rule corneal astigmatism. Different locations and incisions were employed: 18 cases of supeiror scleral pocket incision(Group 1), 16 cases of temporal scleral pocket incison(Group 2), and 17 cases of temporal clear corneal incison(Group 3). Changes in corneal astigmatism were analyzed by vector method following operation. In group 1, the amount of against-the-rule surgically-induced-astigmatism was 0.72 D at potoperative 2 months. Minimal corneal astigmatic chagne was observed in group 2. Change of 0.83 D in the with-the rule direction was in with-the rule direction. In cases of against-the rule corneal astigmatism patients, it is better to use temporal approach considering surgically induced astigmatism, and, at postoperative 2 months, we observed that clear corneal incision induces with-the-rule surgically-induced astigmatism, and reduced total amount of corneal astigmatism.

10.A Clinical Study of the Surgical Treatment of the Spinal Injuries

The number of spinal injuries are now increasing with the high speed of automobile and industrial development. It is a general trend to treat the spinal injuries more actively in recent years, Among 135 spinal injury patients who were admitted to this hospital from August 1,1972 to August 31,1978, twenty six cases of spine fractures and fracture-dislocations who was treated with surgical measures were evaluated and analysed. Short summary of the followings observed is as follows: 1. Out of 26 patients, there were 22 male and 4 female cases. The ratio between male and female was 5.5:1. The majority (16 cases, 62%) was found in the age group of 20 to 40 years. 55.4% of the cases was caused by traffic accident. 2. The most common site of the lesion was cervical spine (9 cases, 35%) and the most common mechanism of injury was flexion and rotation (10 cases, 38%). 3. Our surgical measures were mainly the early anatomical reduction and anterior and posterior spinal fusion followed by external immobilization. 4. Among 26 cases, 34% had complete paralysis and 31%, incomplete paralysis. Complete or partial recovery was observed in 22% of the completely paralysed cases and in 87% of the imcompletely paralysed cases. 5. The initial recovery of spinal cord injury was observed from 1 to 25 days postoperatively (average 15 days).
