1.IV-gamma Globulin Therapy for Severe Abdomlnal Pain Refractory to Steroid Therapy in Henoch-Scholein Purpura.

To make the objective standard of nuclear size in grading nuclear pleomorphism of invasive ductal carcinoma of the breast, we measured maximal nuclear diameter of tumor cells on imprint cytology slides and histologic sections from 65 cases by using computer-based image analysis system (Optimas 6.0). The maximal diameter of red blood cells were also measured to evaluate the ratio of maximal nuclear diameter of tumor cells to maximal diameter of red blood cells. The mean values of maximal nuclear diameter of tumor cells on imprint cytology slides and histologic sections were 7.56microgram, 7.53microgram in nuclear grade 1, 8.92+/-0.98microgram, 9.02+/-0.74microgram in nuclear grade 2, and 12.90+/-1.47prn, 12.44+/-1.41microgram in nuclear grade 3, respectively. There were no significant differences between values of imprint cytology and histologic section. The ratio of maximal nuclear diameter of tumor cells to maximal diameter of red blood cells were 1.3-1.4:1 in nuclear grade 1, 1.6-1.7:1 in nuclear grade 2, and 2.2-2.3:1 in nuclear grade 3. Ths would be guidelines for grading nuclear pleomorphism of invasive ductal carcinoma of the breast on routine surgical pathology work.

2.The Effect of the Ranitidine (Zantac)(R) in the Treatment of Peptic Ulcer.

The therapeutie efficacy of ranitidine was evaluated itn 48 in- and out- patients with endoscopically diagnosed 18 cases of gaetric ulcer and 30 casea of ducnienal ulcer. In the open study, every pabenta was treated with ranitidine 150 mg b.i.d. for 4 weeks, and waa followed up by gastroscopy after 4 weeks of the treatment. The reeults obtained were summarized as follows; 1) 15 out of 18 cases (83. 3%) of gastric ulcers and 26 out of 30 cases (86. 7%) of duodenal ulcers had been completely healed up in 4 weeks. 2) There was a significant relationship between healing of ulcer and the relief of symptoms (x =6.12, P<0.005).3) There were no significant untoward reactions, efther subjective or objective, to the administration of the drug, except one case of severe epigastic discomfort. In conclusion ranitidine appears to be fairy effective and safe for the treatment of patient with peptic ulcr diseases.

3.Cementless Total Hip Arthroplasty in Long Term Steroid-Induced Avascular Necrosis of the Hip

We performed twenty three cementless total hip arthroplasties(THA) on seventeen patients, all of whom were diagnosed with steroid-induced avascular necrosis of the hip (AVN). The average age of the patients was 41years, mean follow-up period was 41months (24-74months). The final average Harris hip score was 94 and overall results were excellent. On radiographic evaluation, 17 femoral components met the criteria for bone ingrowth(spot weld) but it was somewhat delayed. There was no evidence of loosening of the femoral and acetabular components. The results of this study suggest that long term steriod treatment does not prevent bone ingrowth. Cementless total hip arthroplasty appears to be a reasonable therapeutic option for steroid-induced avascular necrosis of the hip.

4.The Developmental Differences of Damage in Rat Brain by Systemic Kainic Acid Injection.

Kainic acid[KA], a glutamic acid analogue, has been widely used as a excitotoxin in the study of neurotoxicity due to its ability to induce strong excitation and to increase intracellular calcium concentration of the mature central nervous system[CNS] neurons. However, it is not well known whether KA is also strongly cytotoxic to the neurons of the postnatal brain. We have injected KA into rats at different developmental stages and have investigated the changes in the expression of c-fos[transcriptional factor and a marker of neuronal activity], heat shock proetin 72[hsp 72, a neuronal injury marker], and glial fibrillary acidic protein[GFAP, a neuronal injury marker] mRNAs, which are known to be increased in KA-induced neurotoxicity, and glyceraldehyde 3-phosphate dehydrogenase [GAPDH, a house keeping gene] mRNAs with in situ hybridization histochemistry using specific riboprobes. The expression of c-fos mRNA was first identified in the CA3 area of hippocampus from 6hr after KA treatment in P7 rats. The c-fos mRNA-expressing area and the level of expression was gradually increased from P7 to adult. Hsp 72 mRNA was first expressed in the dentate gyrus and hippocampus from 6hr after KA treatment in P2l rats. In the adult rats, hsp 72 mRNA was broadly expressed in the brain at 2hr after KA treatment. The increase of GFAP mRNA expression was first identified in Pl4 rat brain from 6hrs after KA treatment, and by the development of brain it tends to appear earlier. The expression of GAPDH mRNA, however, did not show changes after KA treatment except for the adult rats showing a slight decrease at 12hr after KA treatment. These results suggest that KA may offer different level of cytotoxicity to the developing neurons by their developmental status and the difference may be correlated with the completion of synaptogenesis and increase of KA receptor.

5.A Study on the Physical Growth Status in the Korean School Girls from 7 to 10 Years of Age.

The stature, span, lower limb length, sitting height, head length and head breadth of 411 Korean elementary school girls ranging from 7 to 10 years of age were measured biometrically and the indices among all of the items were calculated to investigate the physical growth status. The following conclusion have been obtained. 1. The average statures were 117.40cm in 7, 122.72cm in 8, 126.76cm in 9 and 132.92cm in 10 years of age groups, and the annual average stature growth was 5.17cm. 2. The average spans were 113.12cm in 7, 118.70cm in 8, 124.26cm in 9, 130.46cm in 10 years of age groups, and the annual average span growth was 5.78cm. 3. The average lower limb lengths were 62.96cm in 7, 66.80cm in 8, 69.09cm in 9 and 73.13cm in 10 years of age groups, and the annual average lower length growth was 3.39cm 4. The average sitting heights were 65.82cm in 7, 68.56cm in 8, 70.18cm in 9 and 73.13cm in 10 years of age groups, and the annual average sitting height growth was 2.43cm. 5. The average body weights were 21.22kg in 7, 24.13kg in 8, 25.54kg in 9 and 29.27kg in 10 years of age groups, and the annual average body weight growth was 2.68kg. 6. The average head lengths were 16.29cm in 7, 16.38cm in 8, 16.47cm in 9 and 16.60cm in 10 yeras of age groups, and the annual average head length growth was 0.10cm. 7. The average head breadth were 14.50cm in 7, 14.65cm in 8, 14.67cm in 9 and 14.81cm in 10 years of age groups, and the annual average head breadth growth was 0.10cm. 8. While both relative span and lower limb length increased with age and the relative sitting height slightly decreased with age. 9. The relative body weight increased with age. 10. The head type depended on cephalic index was belonged to hyperbrachycephaly, and showed no differrences with age in all of the groups. 11. The relative span was over 96 in all age groups, and the value increased slightly with age. 12. The average annual growth showed remarkable high value between 9 to 10 years of age in the items of stature, span, lower limb length, sitting height and body weight.

7.Filiform Polyposis in Non-Inflammatory Bowel Disease: A clinicopathologic analysis of five cases.

Filiform polyposis(FP), an unusual form of inflammatory polyposis, has been recently designated to a lesion characterized by multiple, long slender, finger-like projections of mucosa and submucosa of the gastrointestinal tract. We present 5 cases of FP, of which two were associated with irritable bowel syndrome, two with advanced colon carcinoma, and one with active ileotyphlitis(Behcet's disease). The polyps ranged from 1.0 cm to 3 cm; they were either isolated or aggregated, and their numbers were up to 100. The common microscopic feature was filiform polyps which consisted of central submucosal core covered by intact or minimally inflamed intestinal mucosa, comparable with that in reparative processes and formation of inflammatory(redundant) pseudopolyps. We conclude that awareness of FP in non-inflammatory bowel disease may promote the endoscopists to search its underlying disease and prevent unnecessary surgical procedure. Behcets enterocolitis in the ileocecal junction seems an additional preceding disorder of a localized form of FP.

8.Brunner's Gland Hamartoma. A Clinicopathologic Analysis of Six Cases.

Pathological proliferation of Brgnners gland is rare, and its wide range of morphologieal variations have led to confusing the terminology with considerable lack of agreement. Six cases of duodenal nodular lesions which consisted of pathological proliferation of Brunners glands were examined by light microscopy. Polypectomy was made in two cases, and the remaining 4 cases were examined with endoscopic biopsy materials. Two polypectomy specimens, 2 cm and 3 cm each in great diameters, consisted of protruded ovoid mass with broad and short stalks. Microacopically, all of 6 cases revealed thin strands of fibrous connective tissue which separated the normal-looking Brunners glands in lobules. Individual lobules were composed of groups of acini formed by cuboidal cells admixed with occasional endocrine cells. Presence of glandular and ductal configuration of the Brunners gland aside from thick, randomly arranged bundles of smooth muscle were featured without distinct relation to interlobular septa. Also, one polypectomy case disclosed the nests of fat cells interspersed with the glandular acini. One mucosal biopsy case contained not only the smooth muscle bands but also the intimate mixture of both acini and ducts. We conclude that some of heterogeneous composition of duodenal nodular proliferation of Brunners gland is indicative of a hamartomatous growth.
