2.Perinatal Hypoxic-lschemic Brain Injury: MR Findings.

PURPOSE: To characterize the MR findings of hypoxic-ischemic brain injury and to assess the value of the MR imaging. MATERIALS AND METHODS: SE T1 -, T2-weighted, and IR brain MR images of 44 infants and children with the past history of perinatal hypoxic insults were reviewed. Abnormal brain MR findings of 8 patients with birth history of prematurity and 36 patients with birth history of full-term/posterm including 7 with severe anoxic insult history, were compared in regard to the location and the character of the lesions RESULTS: MRI demonstrated the followings;(1)abnormal signal intensity lesions of subcortical and/or deep cerebral white matter, cortex, and deep gray matter, (2)atrophy of the cerebral white matter, cortex and corpus callosum, with/without ventriculomegaly, and (3)delay in myelination. Periventricular and deep white matter lesions were demonstrated in the prematurity, the deep white matter lesions and/or subcortical white matter lesions in the term/post-term, and deep gray matter lesions in the 7 patients with severe anoxic insults history. CONCLUSION: MR imaging was useful in the diagnosis of the hypoxic-ischemic brain injury, and the white and gray matter lesions were correlated with the time of the injury and the severity of hypoxic insult.

5.Clinical Evaluation of Alpha-Acetyldigoxin in Patients with Congestive Heart Failure.

The results of alpha-acetyldigoxin(Dioxanin(R)) administration in 21 patients with congestive heart failure were as follows. Alpha-acetyldigoxin is a new oral preparation of cardiac glycoside derived from lanatosid C. 1. For rapid digitalization in 2-3 days, the initial dose was 1.2 to 2.6mg (average 1.96mg). For medium-fast digitalization in 4 days, the loading dose was 2.0 to 3.2mg (average 2.7mg). For slow digitalization 0.1 to 0.4mg was required for 5 to 6 days without loading dose. The maintenance dose was 0.1 to 0.4mg (average 0.33mg) daily. 2. The therapeutic effect of acetyldigoxin was excellent in 15 cases (71%), good in 4 cases (20%) and stationary in 2 cases (9%). 3. Side effects were observed in 2 cases with usual dosage regimen. One accidental case who took single dose of 4mg(20 tablests) developed supraventricular tachycardia with varying degree of atrioventricular block, ventricular bigeminy and premature beat but recovered completely after 6 days.

6.Three Cases of Oral Lichen Planus Treated with Topical Tacrolimus.

Oral lichen planus (OLP) is a painful chronic inflammatory condition, that is often resistant to many therapeutic approaches, including immunosuppressive agents. Topical tacrolimus has recently been recognized as a safe and effective remedy in the treatment of OLP. We, rerein, describe 3 patients with oral lichen planus, who showed a substantial improvement after the use of topical tacrolimus ointment (0.1%). This topical agent was well tolerated in all 3 patients and could be used an effective medicine in controlling oral lesions of lichen planus.

7.Persistent Mullerian Duct Syndrome in a Boy with Transverse Testicular Ectopia: a Case Report.

Persistent mullerian duct syndrome is characterized by the presence of mullerian structures in male subiects with normal penile and scrotal development. A deficiency of activity of a mullerian inhibiting substance during gestation is believed to be responsible for this syndrome. To date approximately 150 cases of persistent mullerian duct syndrome have been reported and the syndrome of transverse testicular ectopia with persistent mullerian duct structures is much rarer. In Korea, this is the fourth case to be reported. We report a case of persistent mullerian duct syndrome with transverse testicular ectopia in a 7 month-old-boy. Both testes were in the right side of scrotum but no epididymises were found there, which resulted in the disruption of normal relationship between the was and testis. Regarding there was no fertility, mullerian duct structures were removed.

8.Clinical Evaluation of Tibric Acid in Patients with Hyperlipidemia.

Tibric acid is a new oral hypolipidemic agent with the chemical name of 2-chloro-5(3,5-dimethyl piperidinosulfonyl) benzoic acid. The results of tibric acid administration in 22 patients with primary hyperlipidemia were as follows. 1) Serum triglyceride level was significantly decreased in 84.6% of 22 cases with an average 43.9% decrease in serum triglyceride. 2) Serum cholesterol level was decreased in 59.1% of 22 cases with an average 15.8% decrease in serum cholesterol. The decrease in serum cholesterol level was not significant. 3) It seemed that there were significant falls in the serum triglyceride of the hyperlipoproteinemia type IV and IIb patietns and in the serum cholesterol of type IIa patients 4) The side effects of tibric acid were indigestion, loose stool and anorexia. There was no side effects in 63.6% of cases.

10.A Case of Cutaneous Larva Migrans showing A Larva on Biopsy.

Cutaneous larva migrans(creeping eruption) is a rare cutaneous serpiginous erution caused by the accidental invasion of the skin by parasites. We report herein a 27 year-old female with a mild pruritic erythematous serpiginous linear skin lesion located on the patient's back for a month. Histopathologic examination revealed a larva in the dermis with dense iinfiltrates of eosinophils and lymphocytes. After treatment with oral anitiparasitics, the skin lesions resolved with post-inflammatory hyperpigmentation. This is the first case of larva migrans showing the larva on the biopsy specimen in Korea.
