1.Multifocal Reticulum Cell Sarcoma of Bone: A Case Report

A Case of multifocal reticulum cell sarcoma in a 61 year old male is reported. The patient had multiple foci of sarcomatous lesion in the olecranon of the right ulna, medial candyle of the left tibia and in both ankle joints. His initial complaints on admission were pain and swelling in his both ankles and the right elbow of three months duration. Biopsy was carried out on his right ankle joint and diagnosis of reticulum cell sarcoma was made. Surgical excision of the right olecranon and below Knee amputation of the right lower extremity were performed. Following the surgical intervention, Cobalt 60 irradiation therapy was given for 20 days with total amount of 4,000 rads. The follow-up study three months after surgery revealed much improvement and no evidence of recurrence.

4.The effect of photon energy on intensity-modulated radiation therapy (IMRT) plans for prostate cancer.

PURPOSE: To evaluate the effect of common three photon energies (6-MV, 10-MV, and 15-MV) on intensity-modulated radiation therapy (IMRT) plans to treat prostate cancer patients. MATERIALS AND METHODS: Twenty patients with prostate cancer treated locally to 81.0 Gy were retrospectively studied. 6-MV, 10-MV, and 15-MV IMRT plans for each patient were generated using suitable planning objectives, dose constraints, and 8-field setting. The plans were analyzed in terms of dose-volume histogram for the target coverage, dose conformity, organs at risk (OAR) sparing, and normal tissue integral dose. RESULTS: Regardless of the energies chosen at the plans, the target coverage, conformity, and homogeneity of the plans were similar. However, there was a significant dose increase in rectal wall and femoral heads for 6-MV compared to those for 10-MV and 15-MV. The V20 Gy of rectal wall with 6-MV, 10-MV, and 15-MV were 95.6%, 88.4%, and 89.4% while the mean dose to femoral heads were 31.7, 25.9, and 26.3 Gy, respectively. Integral doses to the normal tissues in higher energy (10-MV and 15-MV) plans were reduced by about 7%. Overall, integral doses in mid and low dose regions in 6-MV plans were increased by up to 13%. CONCLUSION: In this study, 10-MV prostate IMRT plans showed better OAR sparing and less integral doses than the 6-MV. The biological and clinical significance of this finding remains to be determined afterward, considering neutron dose contribution.

7.Cementless Total Hip Arthroplasty with Circumferentially Proximal Porous-Coated Femoral Stem -Minimum 10-Year Follow-up Results.

Purpose: The results of a more than 10-year follow-up after total hip arthroplasties with circumferential and proximal porous-coated femoral stems were evaluated. Materials and Methods: Seventy-six patients (80 hips), who were operated on between Aug. 1991 and July 1994, were followed for more than 10 years after primary total hip arthroplasties using Multilock stems. The mean age at the time of the operations was 47.3 years old and the mean duration of follow-up was 12.3 years. The clinical results and radiological findings were evaluated. Results: The mean Harris hip scores improved from 54.7 points to 88.5 points at the time of the 10-year follow up. On the last follow-up radiograph, endosteal bone formation was observed in 70 hips (90%) and all femoral stems were biologically stable. Femoral osteolysis, which was linear in Gruen zone I and VII and mostly restricted to the proximal zones, was observed in 67 hips (84%), and no hip had distal osteolysis in the femur. There was no loosening, migration, or revisions of the stems. Conclusion: Total hip arthroplasties using circumferential and proximal porous coated Multilock femoral stems demonstrated stable osseous fixation and no distal osteolysis for a minimum 10-year follow-up; therefore, all stems were reported to have satisfactory outcomes. With improvement of cup design and liner wear, circumferential and proximal porous coating designs of femoral stems can be an alternative answer to mechanical failure due to aseptic loosening.

8.An experimental study on renal infarction with ethanol

Renal infarction with ethanol was induced exprimentally in rabbits and selective renal angiography wasperformed to evaluate the effectiveness of ethanol as embolic material. The results were as follows; 1. Completeobstruction of renal artery was produced in all cases within 1 week after injection of absolute ethanol(0.5ml/kg). 2. Incomplete obstruction of renal artery was produced in majority after injection of absolute ethanol(0.2ml/kg) and changed to complete obstruction above half cases with time. 3. Incomplete obstruction of renalartery was produced in minority after injection of 60% ethanol (0.2ml/kg) and complete obstruction of renal arterywas not produced. It was considered that ethanol is an effective agent for complete renal infarction and 0.2 to0,5ml/kg of absolute ethanol is effective dose for complete renal infarction.

9.The radiological evaluation of pulmonary metastases from gastric carcinoma

Twenty cases of pulmonary metastases from pathologically proven stomach carcinoma were studies with specialreference to the clinico-radiological findings and the grades of cell-differentiation of stomach carcinoma. Theywere calssifed into five types based on the pattern of chest X-ray findings according to the reportedclassifications of metastatic pulmonary carcinoma. The radiological appearance of pulmoanry metastases formstomach carcinoma was didffferent from that of usual pulmonary metastatic cancers. The Type IV metastatic form(Type IV-Progressive increasing interstitial lung markings and then infiltration of pulmonary parenchyme with orwithout evidence of pleural effusion.) was most commonly seen in stomach carcinoma particularly in poorlydifferentiated cell type and more associates with respiratory symptoms than others.

10.Acute Renal Failure Associated with Kimura's Disease in a Patient with Chronic Renal Failure.

Kimura's disease is a granulomatous disease which develops in the skin, subcutaneous tissues and lymph nodes and is characterized histologically by the presence of lymphoid follicles, vascular proli- feration and infiltration with eosinophils. The disease shows geographical predilection to Japan, China and South East Asia. The exact etiology and pathogenesis remain uncertain. Some patients had proteinuria or nephrotic syndrome. We have recently experienced the superimposed oliguric acute renal failure associated with Kimura's disease in a male patient with chronic renal failure who had been managed conservatively. Inguinal lymph node biopsy revealed Kimura's disease. He recovered from acute renal failure after being treated with hemodialysis and prednisolone. Lymphadeno- pathy and fever subsided with steroid treatment. We report a case of Kimura's disease which was complicated by acute renal failure in the patient with chronic renal failure.
