1.Serum IgE and Peripheral Blood T-Lymphocytes in Parientis with Atopic Dermatitis.

During recent years, many investigators studied to confirm the pathogenesis of atopic dermatitis but it remains obscure. However, two major proposals, the B-padrenergic blockade hypothesis of Szentivanyi and the immunological hypothesis, have been advanced. The significant role of IgE in the pathogenesis of atopic dermatitis was considered since the discovery of elevated IgE levels in atopic dermatitis in 1969. However, recent various reports have indicated the existence of immune defects not only in humoral but also in cellular immune status of patients with atopic dermatitis. This study was undertaken to assess the humoral and cellular immune status of patients with atopic dermatitis using in vitro immune parametera The relationship between the humoral and cellular immune status of atopic subjects and the severity of disease was examined reapectively. The effect of the corticosteroid administration on the serum IgE levels of the patients with atopic dermatitis was followed. Eighty-three patients with atopic dermatitis, diagnosed and treated at the Department of Dermatology, Soon Chun Hyang University Hospital from October, 1988 to September, 1981 and twenty-one normal healthy subjects were included in this: study. Serum lgE levels were measured by Phadebas IgE (paper radio-immunosorbent technique) and peripheral blood T-lymphocytes (early and total) wer enumerated by the E-rosette technique.

3.Pyogenic Arthritis in Renal Transplant Recipients.

Acute pyogenic arthritis in renal transplant patient is one of the serious problem resulting in significant morbidity and mortality. The purpose of this paper is to analyze seven patients in whom acute pyogenic arthritis developed following renal transplantation. Pyogenic arthritis developed in seven (0.7%) out of 958 renal transplant patients whose transplanted kidney functioned well for more than two years. The age of the patients ranged from 37 to 65 years with a mean of 48 years. All patients were male. Pyogenic arthritis developed between the period of one and 12 years (mean: 4.5+/-4.2 years) following transplantation. Causative organisms were Staphylococcus aureus (three), E. coli (two), Salmonella (one) and Acinetobacter bananii (one). Involved joints were four knees, one hip, one elbow and one finger. Two patients improved on a conservative regimen of bed rest and antibiotics and five patients required open drainage. However, pyogenic arthritis recurred in three years, four years and seven years after renal transplant in one patient. Looking at the probable risk factors of age, sex, immunosuppresants, diabetics and acute graft rejection, diabetics and older age (over 40 years old) are highly associated with post renal transplant pyogenic arthritis.

4.Pronator Quadratus Pedicled Bone Graft for the Kienbock's disease: Three cases report

Pedicled bone flap raised from the volar aspect of lower end of the radius, pedicled by the pronator quadratus has been used for the treatment of scaphoid nonunion and Kienbock's disease. From July 1991 to Jan. 1992, authors performed this procedures for 3 cases of Kienbock's disease. Patients were 2 cases in the left hand, l case in the right hand. There was no critial trauma history. Age at operation were 25, 37 and 57 years. All patients were in Stage I in 2 cases and stage II-A in 1 case of Lichtman's radiologic classification. Preoperative grip powers were 11%, 38%, 63% and pinch powers were 20%, 41%, 61% of that of normal opposite hand. Follow up periods were 28, 31 and 33 months. At last follow up roentgenographic finding shows no progression of necrosis of lunate, new bone formation, decreased sclerosis, and joint space were preserved in all cases. The fuctional results at the last follow up were as follows. Pain during motion was markedly diminished in all patients. Grip power were 57%, 89% and 96% and pinch powers were 68%, 83% and 100% of that of normal opposite hand with use of calibrated Jamar dynamometer which showes significant increase than preoperative state. The ranges of motion of wrist were as follows. Volar flexion were 40, 45 and 80 degree. Dorsiflexion were 10, 20 and 45 degree. Ulnar deviation were 10, 20 and 30 degree. Radial deviation were 10, 25 and 30 degree. This viable bone graft with blood supply gives very satisfactory clinical results.

5.The Results of Open Release in Carpal Tunnel Syndrome

Forry-two symptomatic hands of 29 patients with carpal tunnel syndrome operated during the period from Marth 1986 to December 1993 were analized in clinical aspect. Among 29 patients, 3(10.3%) were male and 26(89.7%)were female. The syndrome occured on the right side in 10(34.5%) patients, on the left in 6(20.7%) patients, and on both in 13(44.8%) patients. The causes of the syndrome were idiopathic(90.5%), trauma(7.1%), mass(2.4%). Paresthesia(57.1%) was the most common chief complanit, followed by numbness(35.7%), pain(21.4%), weakness(9.5%). Thenar muscie atrophy occured in 84.8%, positive Tinel sign in 83.3% and positive Phalen sign in 90.0%. Delay of motor conduction velocity of median nerve was noted in all cases and delay of sensory nerve conduction velocity in all cases. Denervation finding such as fibrillation or sharp waves were seen in all cases. The carpal tunnel release was performed in all cases, and 31 cases were treated by carpal tunnel tunnel release with internal neurolysis. 35 cases(83.3%) were excellent or good results. The cases were long duration were poorer results. No significant difference was found between the results in cases treated by carpal tunnel release alone and those in cases treated by carpal tunnel release with internal neurolysis of median nerve.

6.Tear Pattern of Normal and Discoid Lateral Meniscus.

Discoid lateral meniscus is wider and thicker than normal lateral meniscus. The tear pattern and vulnerability to injury of discoid lateral meniscus may be different from that of normal shaped meniscus. Seven hundred and seventy one meniscal tears which were treated arthroscopically between the period of 1982 and 1995 were reviewed retrospectively. Of those cases, tear of normal shaped lateral menisci occupied 327 knees (42.4 %) and discoid lateral menisci 132 knees (17.1 %) representing the higher incidence of tear in normal shaped lateral meniscus. In normal shaped menisci there were no tear in the age of below 10 and there were 37 knees of teens. In discoid lateral menisci, there were 3 knees (2.3 %) of below 10 and 42 knees of teens (31.8 %) that represent higher incidence of younger age group (P<0.05). Tears without trauma history in 31 knees (9.5 %) of normal shaped lateral meniscus and in 70 knees (53.0 %) of discoid menisci represent vulnerability of the discoid lateral meniscus to tear without injury (P<0.05). Symptomatic menisci without tear were noted only in 16 knees (12.1 %) of the discoid lateral menisci. In normal shaped menisci, longitudinal tear within the substance was most common (119 knees, 36.4 %) whereas peripheral tear was most common (57 knees, 43.2 %) in the discoid lateral meniscus, Concealed horizontal tear (11 knees, 8.3 %) and Wrisberg type discoid lateral menisci (2 knees, 1.5 %) was noted only in the discoid lateral meniscus. In summary, discoid lateral meniscus revealed different tear pattern and vulnerability to injury and different age incidence from that of normal shaped lateral meniscus.

7.A Case Report: Surgical Treatment of Pachydermodactyly .

Pachydermodactyly is a rare distinct form of fibromatosis characterized by acquired, asymptomatic dense fibrosis and fibroblastic proliferation around one or more proximal interphalangeal joints. Intralesional triamcinolone injection may be helpful in improving the clinical appearance. A 20-year-old unemployed female patient gave a four years history of progressive swelling of the fingers affecting predominantly the proximal interphalangeal joints of index fingers of left hand and index, middle fingers of right hand. There was no pain and tenderness. Physical examination, complete blood count, antinuclear antibody, rheumatoid factor were unremarkable and radiograph of both hands showed soft tissue swelling only without bony abnormality. We excised the redundant soft tissue from the both sides of proximal interphalangeal joint with longitudinal elliptical fashion. After operation, the fingers of the patient showed marked improvement cosmetically. There were no motor or sensory changes. Pachydermodactyly was first reported in 1996 in Korea. Histologic features include an increased dermal accumulation of collagen and occasionally an increased number of fibroblast. We report for one patient with pachydermodactyly treated with longitudinal elliptical excision and obtained an improvement of clinical appearance.

8.Intestinal Total IgA and Rotavirus - specific IgA Detection in ICR Mouse Model.

In a murine model system, enhancement of the total IgA and rotavirus-specific IgA of suckling mice was measured by ELISA with the intestinal fluid following oral administration of murine rotavirus EC (EDIM-Cambridge) strain. In the EC strain-administered group, the geometric mean titers (GMT) of total IgAs were 512 and 91 at 1 and 2 week postinfection, respectively. On the other hand, the GMTs of the rotavirus-specific IgAs were 108 and 3 at the same periods, respectively. Thus increase in the total IgAs was 64 folds and that in the rotavirus-specific IgAs was 43 folds compared with the negative control group. As the maximal titers of both the total and rotavirus-specific IgAs were observed at 1 week decreasing until 2 weeks after infection, it is evident that the GMT of the total IgA implies that of rotavirus-specific IgA. In our ELISA system, whose specificity was verified by Western blot analysis, the total IgA in the administered group was determined to be 40-400 ng per 1 ml of the intestinal fluid. Therefore it is concluded that determination of the rotavirus-specific IgA in murine models can be a sensitive indication of rotavirus infection, and will be another promising tool in viral challenge experiments in vaccine development.

9.Efficacy of transthoracic echocardiography performed by emergency physician in patients with suspected acute coronary syndrome in the emergency department

Objective:
This study examined the efficacy of trans-thoracic echocardiography (TTE) performed by emergency physicians in the diagnosis and treatment of acute myocardial infarction (MI) in patients with suspected acute coronary syndrome (ACS).
Methods:
This study enrolled patients with suspected symptoms of ACS from January 1, 2017, to June 30, 2017, in the emergency department (ED). One hundred and eighteen patients, who underwent TTE (TTE group), and 384 patients, who did not undergo TTE (control group), were enrolled in this study. The rate of performed coronary angiography (CAG) and percutaneous coronary intervention (PCI) were analyzed in the TTE group. The primary outcomes included the rate of CAG and PCI performed and door-to-puncture time (DTPT).
Results:
The rates of CAG and PCI were significantly higher in the TTE group than in the control group (CAG: 38% vs. 28%, P=0.039; PCI: 31% vs. 21%, P=0.034). The mean DTPT in non-ST segment elevation was 110 minutes (110.2±38.86 minutes) in the TTE group and 151 min (151.3±108.01 minutes) in the control group (P=0.054).
Conclusion
Patients with suspected ACS in ED with TTE had a higher rate of CAG and PCI performed compared to the patients without TTE. In patients with non-ST segment elevation in the initial electrocardiogram, TTE performed by emergency physicians tended to shorten the time to diagnose MI and DTPT.
