1.Navigation-Assisted Knee Arthroplasty in Case of Extra-Articular Deformity or Retained Hardware

Total knee arthroplasty is a surgical treatment for end-stage osteoarthritis of the knee. Postoperative lower limb alignment is one of the factors determining the long-term prognosis after total knee arthroplasty. Navigation-assisted surgery can be used to achieve an accurate lower limb alignment. Particular situations, such as severe extra-articular deformity in the femur or tibia and retained hardware, are a good indication for navigation-assisted surgery. On the other hand, thorough preoperative planning and an understanding of each steps of surgery are necessary to perform total knee arthroplasty successfully in these special cases. In this review article, preoperative radiological measurements and surgical planning for extra-articular deformity correction will be elucidated. The surgical steps and necessary instruments for navigation-assisted total knee arthroplasty will be described in cases of extra-articular deformity or retained hardware. A literature review showed that the radiological and clinical results after total knee arthroplasty using navigation in osteoarthritis with extraarticular deformity were good. Therefore, it is essential to use navigation when performing total knee arthroplasty in patients with extraarticular deformity or retained hardware.
2.A Clinical Study of Serologically Diagnosed Yersinia Pseudotuberculosis Infection in Chidren.

A retrospective analysis of clinical findings in 23 patients with Y. pseudotuberculosis infection who visited Department of Pediatrics of Han Il Hospital from May. 1990 to June, 1992 was performed. 1) The most prevalent age group was 7 to 12 years (16 case: 69.6)and male-to-female ratio was 1.1:1. 2) Monthly distribution showed a high frequency in April, May and June (22 cases:95.7%) 3) The common symptoms were fever(23 cases:100%), abdominal pain (18 cases: 78.3%) vomiting(16 cases: 69.6%), diarrhea (13 cases: 56.7%), rash (13 cases: 56.5%)and acute renal failure (6 cases: 26.1%) in order of frequency, respectively. 4) The laboratory findings were anemia (17.4%), WBC>1(10E4/mm((73.9%), ESR>20mm/hr (91.3%), CRP>3+(91.3%), Positive Widal test (13.0%), BUN>20mg/dl (26.1%) and creatinine> 1.2mg/dl (26.1%). 5) The most common serotype was 4a (30.4%), followed by 4b, 5b, 2b, 2c, 5a, 1b, 1a, 6 in order of frequency. The results suggest that Yersinia pseudotuberculosis infection is one of the important causes of the important causes of fever, eruptive skin diseases and acute renal failure in Korean children and further studies including epidemiology, pathogenesis, etc, are needed.

6.Doppler Echocardiographic Findings of Mitral Valve Prolapse : Usefulness of the Apical Rotation Method of a Transducer for Assessment of Site of Prolapse.

BACKGROUND: Color Doppler echocardiography is sensitive in detecting mitral regurgitation and useful in quantitating its severity. The presence of an eccentric regurgitant jet suggests that regurgitation is caused by prolapsing or flail leaflet of mitral valve. Until recently the direction of regurgitant jet in mitral valve prolapse has been examined in a single(parasternal short axis view) or orthogonal plane using color Doppler echocardiography, and few in the apical rotation method of a transducer. The purpose of this study was to clarify the usefullness of the apical rotation method of a transducer in detection of the direction of mitral regurgitant jet and diagnosis of the sites of mitral valve prolapse. METHODS: Twenty four patients(8 men and 16 women, mean age:47.3+/-18.8 years) with mitral valve prolapse with eccentric regurgitant jet were examined by two-dimensional and color Doppler echocardiograply using conventional parasternal long and short axis views, and four apical longitudinal planes(four chamber, vertical, two chamber and transverse views) obtained by the apical retation method of a transducer. RESULTS: Thirty one regurgitant jets were detected in twenty four patients, eighteen patients had anterior, nine patints posterior, and three patients bi-leaflet(anterior and posterior) prolapse. In eighteen patients with anterior leaflet prolapse, ten had medial, eight had middle, three had lateral, and three had two portions(two, medial and middle; one, middle and lateral) prolapse. In nine patients with posterior leaflet prolapse, five had medial, three had middle, two had lateral, and one had two(medial and middle) scallop prolapse. CONCLUSION: Color Doppler echocardiography by the apical rotation method of transducer is useful in assessment of the site of prolapse in patients with mitral valve prolapse with eccentric regurgitation.

7.Comparison Study of Dipyridamole and Dobutamine Stress Echocardiography in Same Patients.

BACKGROUND: The two most commonly used drugs as a stressor during wtress echocardiography are dipyridamole and dobutamine. The purpose of this study was to compare diagnostic accuracies of dipyridamole and dobutamine stress echocardiography for fixed coronary artery disease and evaluate complications related to the two agents in the same patients. METHODS: 30(M : 5=19 : 11, age=56+/-8.8yr) consecutive patients without history of previous myocardial infarction underwent coronary angiography, dipyridamole and dobutamine stress echocardiography in random order. Dipyridamole was infused up to 0.84mg/Kg for 10 minutes during clinical, ECG and echocardiographic montioring. Dobutamine was infused in dose increments from 5 to 40microg/Kg/min under the same condition. Positive criteria for myocardial ischemia by echocardiography was now regional wall mation abnormatity or worsening of regional wall motion after stress. Significant coronary disease was defined as more than 70% stenosis by coronary angiography. RESULTS: The sensitivity and specificity of both stress echocardiography were same, 82% and 92% respectively. In a single vessel disease the sensitivity of dipyridamole echocardiography was 75% and dobutamine echocardiography was 83% without statistical difference. The correlation of ischemic free time during both stress test was 0.375. During dipyridamole infusion no test was prematurely terminated because of side effects, but 3 patients(10%) developed severe hypertension and ventricular arrytricular arrythmia during dobutamine infusion and test was terminated. CONCLUSION: Thus, by this prospective direct comparison of both stress test, dipyridamole and dobutamine stress echocardiography have similar diagnostic accuracies for the detection of coronary artery disease. But during dobutamine infusion, careful monitoring for hemodynamic changes arrythmia is required for possible serious complications.

8.Gortex Patch Sling Operation for Patient with Intrinsic Sphincter Deficiency.

PURPOSE: We evaluated the early outcomes of Gortex patch sling operation in the management of women with intrinsic sphincter deficiency(ISD). MATERIALS AND METHODS: 20 women underwent Gortex patch sling operation to treat genuine stress urinary incontinence and all patients had ISD. Patients were interviewed by telephone for incontinence, self-catheterization, and voiding symptoms at a mean of 16.6 months(range 6-35) after surgery. RESULTS: The mean age of the patients was 54.2 years and mean parity was 2. Mean operation time was 55 minutes(range 45-90) and mean duration of postoperative catheterization was 7.0 days(range 5-12). Success rate(complete dry) was 90% and de novo urge incontinence occurred in 3(15%). CONCLUSIONS: Gortex patch sling operation is a safe, simple and effective procedure for the treatment of stress incontinence due to ISD. However, further follow-up of longterm results is needed.

9.Central Neural Pathway for the Rat Tongue.

Bartha strain of pseudorabies virus[PRV-Ba] was utilized as a tracer to identify the neuronal axis of rat tongue muscles ; intrinsic muscles and extrinsic muscles, styloglossus, genioglossus, and hyoglossus muscle. After injection of 10 microliter of PRV-Ba into tongue muscles and 48-96 hours survivals, rats were perfused with 4% paraformaldehyde lysine periodate and brains were removed. PRV-Ba were localized in neural circuits by immunohistochemistry employing rabbit anti PRV-Ba as a primary antibody and ABC method. Injection of PRV-Ba into the tongue muscles resulted in uptake and retrograde transport of PRV-Ba in the rat brain. The result showed a circuit specific connection of many nerve cell groups along the time sequence : PRV-Ba immunoreactive cells appeared in hypoglossal nucleus and motor trigeminal nucleus ipsilaterally as seen with conventional tracers. Raphe nucleus, prepositus hypoglossal nucleus, spinal trigeminal nucleus, Al, A5 and facial nucleus of rhombencephalon showed immunoreactivity bilaterally. There were positive neurons in parabrachial nucleus, locus ceruleus, mesencephalic trigeminal nucleus, periaqueductal gray and A7 of mesencephalon and paraventricular nucleus, suprachiasmatic nucleus, organum vasculosum of lamina terminalis of diencephalon. Also positive reactions were showed in amygdala, insular cortex, frontal cortex and subfornical organ in telencephalon. Early immunoreactivity was appeared in hypoglossal nucleus and motor trigeminal nucleus, and there were positive neurons in the nuclei of the medulla oblongate, midbrain, pons, hypothalamus, cerebellum and medial preoptic area at middle stage. Subsequently the viral antigens were found in forebrain cell groups, paraventricular nuclei, suprachiasmatic nucleus, lateral hypothalamic area and primary motor cortex in frontal lobe bilaterally at 80-90hrs postinjection. These data demonstrate that the PRV-Ba can across synapses in the central nervous system with projection specific pattern, and this virus defines many elements of the neural network governing tongue. Therefore PRV-Ba are proved as a excellent neurotracer in the tract-tracing researches.
