LSA of the glans and prepuce of the penis is uncommon. It usually appears as whitish plaques and macules and can progress to orificial narrowing, phimosis and carcinorna. The case presented herein showed distinct papules and nodules on the glans and inner side of the prepuce instead of usual flattened patches. The patient, 24 years old man, was seen first seven months after his initial notice of the illness and has been observed by the authors four years thereafter. For the first year the papuloncdules on the glans were hard in consistence and the glans itself was being contracting, From the third year, the hard papulonodules began to resolve but resulted in greatly contracted glans penis. To observe the diagncsic histopathological finding repeated biopsies were needed.

Bowen's disease is intraepidermal squamous cell carcinoma involving skin and certain mucocutaneous junctions. The disease usually persues a slow relatively benign course but may progress to invasive squamous cell carcinoma. The present case, 67-years-old male, showed multiple dark brownish somewhat elevated horny lesions on the trunk and extremities including both palms. Some of the lesions had the appearance of small horns. Multiple biopsy specimens revealed typical histological findings of Bowen's disease or frank invasive squamous cell carcinoma. according to the each biopsy site. Authors treated all the lesions with surgical avulsion and electrodesiccation with curettage under general anestthesia and subsequent topical 5-FU ointment application.

BACKGROUND: Although the wet earwax has been known to be related with the incidence of tinea versicolor, very little has been revealed on how they are correlated with each other. The incidence of Malassezia furfur, the etiological organism of tinea versicolor, and its population densities as yearst and mycelial form in the different types of earwax, the dry and the wet, have not been studied. METHODS: The type of earwax in 65 patients with tinea versicolor were classified into 2 types, dry and wet, by its appearence and the subjective complaints of the patients. The dry and wet cerumens were obtained from 142 and 49 individulas without tinea versicolor respectively. The cerumens were stained with 10% Parker ink-KOH solution. The incidence and its population densities of yeast and mycelial M. furfur in the wet cerumens were compared with those in the dry cerumens according to a new grading method using bacterial index(BI) of lepra bacilli. RESULTS: 1. Dry and wet earwax were observed in 58.5%(38 cases) and 41.5%(27 cases) of 65 patients respectively. 2. The incidence of yeast M. furfur in the dry and wet cerumens were 100% and 63.3%(31 of 49 cases) respectively, and that of mycelial M. furfur, 28.2%(40 of 142 cases) and 32.7%(16 of 49 cases) respectively. 3. The population densities(from 3+ to 6+) of yeast M. furfur in the dry and wet cerumens were ob served in 93.0%(132 of 142 cases) and 49.0%(24 of 40 cases), and those(3+) of mycelial M. furfur, 21.1%(30 of 142 cases) and 24.5%(12 of 49 cases) respectively. CONCLUSION: The incidence(41.5%) of wet earwax in tinea versicolor patients is significantly higher than that(0.8%) in The Korean control group. The ratio between the incidence of mycelial M. furfur and that of yeast M. furfur in the wet cerumens is higher than in the dry cerumens, and the ratio of population densities in the wet cerumens is also higher. These observations suggest that the skin surface conditions of the wet earwax group may be more suitable for yeast-mycelial transformation of M. furfur than those of the dry earwax group. The comparison of chemical compositions of the skin surface between dry and wet earwax groups is needed to elucidate the correlation between tinea versicolor and the types of earwax in the future.

Eczema herpeticum occurred in a patient with pemphigus foliaceus and esophageal cancer. The patient's initial skin lesions composed of large flaccid bullaes showed subcorneal bullae with typical acantholytic cells. Subsequent grouped small vesicles appeared widely on the erythematous base were diagnosed herpes simplex virus infection by Tzanck smear and histopathological findings. The patient died one week after the development of the widespread herpes virus infection.

The fifty gastrointestinal polyps, which were removed endoscopically frorn 47 patients at the Seoul National Univeraity. Hospital from January, 1978 to September, 1983, were reviewed with regard to the clinical and pathological features. The observed data are as follows; 1) Fifty polyps were reoved from 10 patients with gastric polyps and 37 patients with colonic polyps. Out of 37 patients 26 were male and 21 female, the sex ratio being 1.2: 1. 2) The gastric polyps were located in the body and antrum and 90 per cent of the colonic polyps were found in the left colon, especially in the rectosigmoid colon. 3) Out of the 10 gastric ployps snared, 8 could be retrivevd, 2 being lost. The histological examination showed that 3 were hyperplastic polyps, 2 gastritis verrucosa, 1 inflammatory polyp and the remaining 1 revealed intramucosel carcinoma with borderline epithelium. 4) Fourty polypectomies were performed in 37 patients with colonic polyps. Histological examination revealed that 24(60%) were adenomatous polyps, 9(22.5%) retention polyp, 4 (10.0%) inflammatory polyp and 3(7. 5%) Peutz-Jegher's poIyps. Among the adenomatous polys, 4 cases were complicated with focal adenocarcinoma. 5) Except the 2 cases of post-polypectomy bleeding, no other complication was encountered. The one case of hemorrhage after gastric polypectomy was treated with transfusion of 2 pints of whole blood, and the other case after colonic polypectomy required exploratory laparotomy because of inability of bleeding control by electrocoagulation.

Since the introduction of surgical microscope in microvesael surgery by Jacobson and Suarez in 1960, many surgeons have succeeded replantation, transplantation of composite segment of tissues so called free fiap-free bone graft and toe to hand transfer. McCraw & Furlow reported successfully transfered dorsal foot flap using dorsalis pedis artery in 1975 and Cobett transfered great toe to band for reconstruction of the amputated thunb. The authors experienced six cases of microsurgery using dorsalis pedis artery durig the recent two years in the department of Orthopaedic surgery of Soon Chun Hyang College and results in this paper. 1. Four cases out of six were dorsalis pedis free flap, one case was second toe to thumb and tbe other one was reconstruction of an amputated thumb in one stage using iliac bone graft and dorsalis pedis flap. 2. One case out of four cases of dorsalis pedis free flap was performed for reconstruction of contracted first web and the other cases were performed for foot. 3. Five cases out of six were successfully transfeed, one case which was toe to thumb was failed. The cause. of fail was probably due to post-operative hematoma. 4. Composite tissue using dorsalis pedis artery is one of the good donor site for composite tissue transfer for not only skin defect and scar contracture of the hand and foot but also reconstruction of the amputated fingers because it has several advantages; an acceptable thickness, a constant arterial supply, venous drainage through the saphenous system, and constant innervation through the terminal branches of the superficial and deep peroneal nerve.

The ten cases of metallic failure after I-M nailing & plate fixation who were treated at depart tment of orthopaedic surgery, Soon Chun Hyang University during nine years from June, 1974 to May, 1983 were clinically analysed. The survey as summerized as follows: l. Of the ten cases, six cases were seen the breakage of plate, two cases were the loosening of screw, one case was the brakage of nail & one case was the angulation of nail. Of the six cases of plates, two cases were DCP, three cases were ordinary plate & one case was Thornton plate. 2. Of the six cases who metallic failure occurred as plate failure, five cases were the femur fractures, one case was the tibia fracture. All case who metallic failure occurred as nail failure were ferumr fractures. Of the two cases of screw loosening, one case was the fermur fracture. One case was the tibia fracture. 3. The average interval between operation and metallic failure. 1) The plate breakage was 6.3 months. 2) The screw loosening was 7 months. 3) The nail breakage was 6 months. 4) The nail angulation was 8 months. 4. Thirty-seven patient who had forearm bone fracture were treated by four different methods at the Department of Orthopedic Surgery, Soonchunhyang University Hospital from Jan. 1979 to March 1982. The obtained results are as follow; 1. Nineteen cases out of 37 cases of the forearm bone fracture were both forearm bones, 10 cases were ulna and 8 were radius fracture. 2. Twenty-three cases out of 37 were closed and 13 cases were open fracture, respectably. 3. The applied treatment were manual reduction with cast immobilization, intramedullary naliing, plate and screw fixation and combined. 4. Mean duration of primary bone union of the forerm bone fracture which were treated by plate and screw was 12. 4 weeks, combined fixation was 12.8 weeks, intramedullary nailing was 15.3 weeks and that of manual reduction and cast was 16 weeks in order. 5. Functional result was classified according to the rating system of Smith and Sage. The best method was plate and screw and the worst was manual reduction and cast immobilization. 6. D.C.P. fixation seems to be one of the best method in this series. Combined method, in a way of D.C.P. for radius and I–M nailing for ulna, is a method when shortening of operation time is inevitable.

Diffuse neonatal hemangiomatosis is an uncommon disease that is characteristed by a diffuse nature of the lesions, and distinguished form a single or few, or superficial or deep, capillary, cavernous, or mixed hemangiomas occuring in early or adult life. We report an autopsy case of multiple hemangiomatosis, which is associated with massive osteolysis of right humerus and arteriovenous fistulae in surrounding soft tissues. The patient is a 23 day old female infant and had a 2.0x1.5 cm sized cystic destructive bony lesion which was located in the proximal shaft of right humerus. Right arm was hypertrophied, compared to the normal looking left. There were multiple hemangiomas in right humerus, lung, cutaneous skeletal muscles and nerves. This case shares clinical characteristics of Gorham's disease.
