5.The Treatment of Infected Nonunion by the Ilizarov Technique

The goal of treatment in infected nonunion include the elimination of infection, deformity and defects as same time as obtaining union. It is very difficult to obtain union and to correct deformity, infection and shortening in infected nonunion. The purpose of this study is to review the results of treatment of infected nonunion by the Ilizarov technique. Nineteen patients with infected nonunion were treated using Ilizarov external fixator from july 1991 to September 1993. Seven patients are undergoing treatment until now, so those were excluded from this data. Twelve patients were achieved bony union. Eleven cases were united and one case was failed with Ilizarov external fixator. We analyzed twelve cases. There were eleven males and one female, eight tibias and four femurs. Bony union was achieved by direct compression in six cases. Six cases were treated by bifocal osteosynthesis, but five cases were achieved bony union and one case was failed. So, we achieved bony union with intramedullary nailing and bone graft. The results were summarized as follows; 1. Eleven cases of infected nonunion were united with Ilizarov external fixator. The mean time to union of infected nonunion site was 10. 8 months. 2. The average bone lengthening of six cases was 64mm, ranging from 15mm to 110mm. The healing index averaged l. 64 month/Cm. 3, The bone results were excellent in six cases, good in five cases, poor in one case. The functional results were excellent in four cases, good in seven cases, fair in one case. 4. In all cases, we experienced difficulties. So we concluded that Ilizarov technique for infected nonunion is a valid procedure for control of infection, union, bone defect and deformity.

7.A Case of Renal Oncocytoma.

The renal oncocytoma is a rare benign tumor arising from the proximal tubular cells and has recently become a recognized clinical and pathologic entity. The feature of this tumor includes the tendency to (1) be incidental finding, (2) be large in size, (3) have well defined borders, (4) exhibit benign clinical behaviors. Here, we report a case of renal oncocytoma which was found incidentally due to pelvic inflammatory disease of a 37-years-old woman, who was treated with the radical nephrectomy.

The vaginal stone associated with vesico-vaginal fistula is quite rare and its cause, frequency, composition and others are poorly understood. We report a case of huge vaginal stone associated with vesico-vaginal fistula in 57 years old woman. considering of the possible causes of stone formation in the vagina and its composition, prevention, treatment complication and others. A stone, 6 x 7 x 5cm in size, was removed by litholapaxy. The stone was composed of uric acid, magnesium ammonium phosphate and carbonate appetite.

10.Clinical Experience of Automated Percutaneous Lumbar Discectomy

One hundred and twenty patients with symptomatic lumbar disc herniation were treated with automated percutaneous lumbar discectomy(APLD) from June 1988 to October 1989 in Seoul National University Hospital. Among them, 95 patients were followed up for more than 6 months. Number of male patients was 58 and female 37. Age ranged from 15 to 57 years with average 28 years. L4-L5 was the most commomly involved level comprising 80%, and 2 level involvement was 9%. Average duration of procedure was 38 minutes, and average amount of tissue aspirated was 5.2 grams and average hospital stay was 2.4 days. The average follw up was 10.8 months. Eighty seven percent of patients were satisfactory and 13% unsatisfactory and there was no major complication. There was no significant disc space narrowing in all the cases and the size of herniation revealed no significant change in all the patients, even the patient had a satisfactory result. APLD was compared with chemonucleolysis performed during 1988 in terms of clinical results, disc space narrowing on plain films and herniation size change on follw up CT. Chemonucleolysis also had satisfactory result in 89% with reduction of herniation size in 41% but induced significant disc space narrowing in 84%. Authors experienced some special cases, which are generally considered poor indication for percutaneous discectomy. Those were degenerative, calcified and possibly ruptured discs. Satisfactory results were obtained in 10 of 14 these patients.
