2.Clinical and Histopathological Study of Calcinosis Cutis.

BACKGROUND: Calcinosis cutis may occur when connective tissue is abnormal (dystrophic), or where calcium or phosphate levels in the blood are high(metastatic); alternatively, there may be no obvious underlying cause(idiopathic). The exact incidence of calcinosis cutis in dermatologic patients is not well-kniown and the pathomechanism of it remains to be elucidated. OBJECTIVE: This study was performed to analyze the incidence, clinical and histopathological features of calcinosis cutis. METHODS: We studied 72 patients with calcinosis cutis who visited our department between January 1985 and December 1996. The patients were classified into 3 types (dystrophic, metastatic, and idiopathic) and were analyzed clinically and histopathologically. Results : The results were summerized as follows; 1. There were 60 cases(83.3%) and 12 cases(16.7%) of the dystrophic and idiopathic types, respectively but the metastatic type was not observed. The frequency of dystrophic calcification in individual disorders was 88.2% in pseudoxanthoma elasticum, 66.7% in trichilemmal cyst and dermatomyositis, and 64.6% in pilomatrichoma. 2. In the idiopathic type, it was common in females aged over 50 years and the mean duration of lesions was 2.8+/-2.0 years. The predilection site was the flank(5 cases) and most of the lesions(7 cases) showed skin-colored nodules 3. Histopathologically, calcified material of the idiopathic type was located predominently in the dermis(10 cases) and was present as large masses surrounded by a foreign body reaction. 4. In the dystrophic type, calcified deposits were noted predominently within the shadow cells of pilomatricoma, in the degenerated elastic fibers of pseudoxanthoma elasticum, and in the keratinized area of basal cell carcinoma, squamous cell carcinoma, trichilemmal cyst, and epidermal cyst. Foreign body reactions with giant cells and mononuclear cell infiltrations were often found around large deposits of calcium. Conclusion : The incidence of calcinosis cutis in dermatologic patients is relatively low, but a variety of disorders can be associated with cutaneous calcification. Therefore, dermatologists should be familiar with the different forms of cutaneous calcification and the dermatoses that manifest them.

3.THREE DIMENSIONAL FINITE ELEMENT ANALYSIS OF BRANEMARK NOVUM(R)IMMEDIATE IMPLANT PROSTHODONTIC PROTOCOL.

Since the treatment of edentulous patients with osseointegrated implant was first introduced more than 30 years ago, implant therapy has become one of the most important dental treatment modalities today. Based on the previous experience and knowledge. Branemark Novum(R) protocol was introduced with the concept of simplifying surgical and prosthetic technique and reducing healing time recently. This protocol recommends the installation of three 5mm wide diameter fixtures in anterior mandible and the prefabricated titanium bars for superstructure fabrication. This study was designed to analyze the stress distribution at fixture and superstructure area according to changes of fixture number, diameter and superstructure materials. Four 3-dimensional finite element models were fabricated. Model 1 - 5 standard fixtures (13mm long and 3.75mm in diameter) & superstructure consisted of type IV gold alloy and resin Model 2 - 3 wide diameter fixtures (13mm long and 5.0mm in diameter) )& superstructure consisted of type IV gold alloy and resin Model 3 - 3 wide diameter fixtures (13mm long and 5.0mm in diameter) )& superstructure consisted of titanium and resin Model 4 - 3 wide diameter fixtures (13mm long and 5.0mm in diameter)& superstructure consisted of titanium and porcelain A 150N occlusal force was applied on the 1st molar of each model in 3 directions-vertical(90degree)horizontal(0degree)and oblique (120degree). After analyzing the stress and displacements, following results were obtained. 1. There were no significant difference in stress distribution among experimental models. 2. Model 2,3,4 showed less amount of compressive stress than that of model 1. However, tensile stress was similar. 3. Veneer material with a high modulus of elasticity demonstrated less stress accumulation in the superstructure. Within the limits of this study.Branemark Novum(R) protocol demonstrated comparable biomechanical properties to conventional protocol.

4.The Prognostic Assessment of Legg-Calve-Perthes Disease

The management of LcP disease is dependent on the prognosis yet. This may be difficult to predict for the individual child, particularly at the onset of the disease when radiological sign may be minimal. But some form of conservative treatment is generally commenced before the prognosis become apparent. The purpose of this study was to determine the validity of Catterall's grouping and value of other radiological and clinical factors in predictiong prognisis and also the correlation between the clinical and radiological results at the conclusion of the pathological precess in LCP disease. Between June 1986 and July 1995, 64 childrens with LCP disease were tearted in Kyungpook University Hospital, 44 hips of 40 childrens who had adequate clinical and radiological data were reviewed. The average follow-up period was 5.4 years. The results obtained were followings: 1.The mean age at first visit or diagnosis was 6.7 years. 2. Of 40 childrens, 4 childrens had bilateral involvement. Boys were more predominat. 3. The degree of uncovering of femoral head and medial joint space at the presentation seemed to be prognostic factor for the end results. 4. Catterall & lateral pillar classification seemed to be difficult at presentation of the disease but also an important guide to determine the prognosis.

5.Characterization of Heparin:PF4 Isoantibody Interaction to Platelets in Heparin-Induced Thrombocytopenia/ Thrombosis.

BACKGROUND: Heparin-induced thrombocytopenia/ thrombosis (HITT) is recognized as the most frequent and fatal symptom complexes in patients receiving heparin therapy. The antibodies of HITT are not directly bound to heparin but bound to complexes of heparin and platelet factor 4 (PF4) derived from platelet alpha-granules. That is, HITT IgG antibody-heparin-PF4 immune complexes are bound to FcgammaRII receptor of platelets, which induced thrombocytopenia. Some researches showed the antibodies reactive to platelets could be IgM or IgA as well as IgG. So in this study, the authors tried to explain the molecular basis of heparin-PF4-isoantibody complexes . METHODS: In HITT patients who had received long-term heparin therapy, we determined HITT isoantibodies and titers using heparin:PF4 ELISA. When fifteen HITT patients with high titer antibodies (more than 1 : 100) were selected, reaction patterns of isoantibodies with the platelets were examined through serotonin release test and flow cytometry. RESULTS: All patients showed one or more isotype antibodies and the most frequent isotype was IgGl (nine patients) . In the presence of optimal concentra pion of heparin and PF4, ten patients had antibodies which activated platelets, and all of them were positive in serotonin release test. Reactive plasmas had IgGl, IgG3, IgA or IgM antibodies, and each of them except one had IgGl. These platelet activations could be blocked in vitro by anti-IV.3 antibody. Non-reactive plasmas were negative In serotonin release assay nor had TgGl. The plasmas 4hat had two or more isoantibodies showed a similar pattern of the IgG antibody by flow cytometry. CONCLUSIONS: The HITT antibodies can be all kinds of antibody isotopes, but IgA and IgM may not bind to the platelets directly. It seems to be possible only after reacting with heparin-PF4-IgG complexes.

An antrochoanal polyp is a solitary polypoid mass that arises from the maxillary sinus, and protrudes through the natural ostium into the middle meatus, and reaches the choana, The polyp does not differ histologically from an ordinary nasal polyp, which is composed of edematous, hyperplastic submucosal connective tissue stroma, and is relatively hypocellular, In this sequence of events. Its computed tomographic(CT)findings are characteristic. We retrospectively analyzed CT studies of nine cases, all of which showed similar CT findings; a solitary, homogeneous, hypodense mass without bony destruction that extends from the maxillary sinus through the widened ostium into the choana.
