1.Relation of internal echo patterns and hemodynamics by incremental dynamic CT in hepatic cavernous hemangioma.

To evaluate the correlation between the internal echo patterns and henodynamics in hepatic cavernous hemangioma, we compared the interanal echo pattern with the hemodynamic pattern examined by intravenous bolus CT(multi-phase incremental bolus dynamic CT) in 34 diagnosed hemangiomas (25 patients). We classified cavernous hemangiomas as two different hemodynamic patterns according to the findings of the early phase of intravenous bolus CT scans. Early enhancing type means that most portion of a mass is enhanced during the early hpase and late enhancing type means that only minimal peripheral portion of a mass is enhanced during the early phase. We classified ultrasonogrphic patterns as hypoechoic and hyperechoic according to main echogenicity of the mass. The late enhancing type tends toward the high echo type (20/21) and the early enhancing type tends to ward the low echo type (11/13) (p<0.01) Therefore, we believe that the early enhancing type which represents rapid contrast inflow toward mass, contains large portions of cavernous space and showes hypoechoic, but late enhancing type which represents late contrast inflow toward a mass, contains large portions of fibrous space and showes hyperechoic. In conclusion, there was significant correlation between internal hemodynamics and echo patterns in hepatic cavernous hemangioma.

6.A Case of Wiskott-Aldrich Syndrome.

Wiskott-Aldrich Syndrome is an X-linked disorder characterized by recurrent infection, thrombocypenia and eczema. Various defects in cell-mediated immunity and deficient antibody reponse to carbohydrate antigens have been described. We experienced a case of Wiskott-Aldrich Syndrome of 28 months old male patient. He has been suffered from multiple petechiae with bleeding, recurrent pyogenic infections and generalized eczema since 3 months of age. Immunological abnormalities are as follows: 1) Serum IgM was gradually decreased, while IgA and IgE were increased. 2) Antibody response against polysaccharide antigen (PRP) was not observed after 3 times of PRPT immunization. 3) CD4/CD8 ratio was reversed (0.6). 4) Proliferative response of mononuclear cells was significantly reduced, and CMI skin test also showed negative results. A brief review of literature was made.

The cutaneous metastaaes from carcinoma of breast produce four definite clinical types: inflammatory carcinoma, telangiectatic carcinoma, nodular carcinoma, and carcinoma en cuirasse. We present a case of 51-year-old woman who had purpuric, hard, sclerotic plaque with ulceration, crust and. several pea-sized hemorrhagic papulovesicular eruptions on the right chest wall for one year. Histopathological findings show metastatic ductal carcinoma but, the primary focus was not determined.

10.Comparison of Bone Mineral Density and Lipid Profiles in Pre and Postmenopausal Women.

BACKGROUND: Estrogen deficiency accelerates loss of bone mass and changes lipid profile in the postmenopausal women, so that the osteoporosis and astherosclerosis were developed. But it has not enough studies including the premenopausal women. So we have investigated about the differences of body mass index(BMI), lipid profile and bone mineral density (BMD) with pre- and postmenopausal women. METHODS: We have evaluated 201 premenopausal women and 322 postmenopausal women out of total 651 who had visited Health Screening Center in the hospital of Eul-Ji Medical College from November, 1995 to July, 1996. RESULTS: The mean age of total subjects, premenopausal women, postmenopausal women were 51.9, 43.9, 56.8 years, respectively. The mean period after menopause was 8.1 years. Significant difference was seen in BMI, lipid profile and BMD according to age and menopause(P<0.01). BMI was related to lipid profile(P<0.01), but not to BMD(P>0.1). In postmenopausal women BMI, BMD and lipid profile were related to postmenopausal period (P<0.05). In viewing their correlations BMD had strong adverse correlations with factors such as age, menopause, and postmenopausal period. Lipid profile had weak positive correlations with factors such as age, menopause, BMI(P<0.001). CONCLUSIONS: The lipid profile are related to factors such as age, BMI, menopause, and postmenopausal period. The BMD is related to above factors except BMI. Prospective study is needed to evaluate the influence of estrogen on BMD and lipid metabolism. Thus, it helps to the prevention and treatment of the osteoporosis and hyperlipidemia in the postmenopausal women.
