1.Traumatic Bilateiral Anteror and Posterior Dislocation of Hip: Report of Two Case

Two cases of traumatic bilateral anterior and posterior dislocation of hip are reported. Dislocations were developed by a traffic accident as a passenger injury in truck. In both cases, the fractued femoral heads appeared posteriorly dislocated on x-ray examination and one of them was removed through posterior approach. Primary arthroplasty for operated hip was not attempted because the patient was young and the defect seemed to be unrelated to weight bearing surface. Extreme rarity of its incidence and curious uncertain mechanism of injury urge to report these cases.

5.Clinical Study of Operative Treatment of Medical Collateral Ligamentous Injuries of the Knee Joint

Rotatory Instability of the permitting abnormal external totation of the tibia on the femur is the result af forced abduction of the flexed knee and external rotation of the tibia. The basic lesion is a tear in the medlal capsular ligament. The authors have reported clinical study in 40 patients with medial collateral ligament injuries who were treated by operative methods in the Orthopedic Department at the Taegu Presbyterian Hospital from January 1973 to Octdber 1979. of 40 patients, pes anserinus transplantation was performed on 15 patients and direct repair on 25 patients. The results are as followings: 1. All the patients were seen with a complete rupture of the medial collateral ligament and of these 45% had rupture of the femoral attachement. 2. Exellent to good result were more often obtained in recent injuries of less than 2 weeks rather than those in older Injuries. 3. Exellent to good result were more often obtained in the groups of Pes Anserinus tranplantation than the groups of direct repair. 4. In all cases of groups of pes anserinus transplantation, permanent sensory palsy af the patellar branches of the saphenous nerve was present due to the location of the surgical incision.

6.Hair Strangulation: A Case Report

A case of hair strangulation on right toe of two months old female patient is reported together with a brief review of references in this paper. This hair strangulation was on the proximal interphanlangeal joint of the right third toe. The distal portion of the hair strangulation was discolored as black, swollon, cold and no evidence of circulation. Amputationon the metatarsophalangeal joint was done.

7.A Clinical Analysis on Operative Treatment of Displaced Proximal Humeral Fracture

Most proximal humeral fractures respond satisfactory to simple conservative treatment. But operative treatment is recornmended in the cases that poor results are anticipated by prolonged immobilization, or because of the severe displacement and comminution. After Neer. in 1970, emphasized the need for operative treatment in displaced proximal humeral fractures, many papers reported better results with the operative management. Twenty five cases of displaced fractures and fracture dislocations of the proximal humerus treated at the department of orthopaedic surgery, Keimyung University with open reduction and internal fixation were analyzed clinically and radiologically. The following results were obtained. 1. The prevalent age distribution was the second snd third decades(average 42.6 years) and ratio of male and female was 2.1: 1. 2. In 10 of the 25 cases, wire loop was used for internal fixation, and the result was satisfactory in 8 cases. However, the disadvantage was difficulty in wire removal after bony union because the loop was buried in the callus or cortex. 3. Over-all results were excellent or satisfactory in 64% of 25 cases. There were 4 excellent and 1 satisfsctory in 5 cases of type Il fracture, 7 excellent, 2 satisfactory and 3 unsatisfactory in 12 cases of type III fracture, 1 excellent and 4 unsatisfactory in 5 cases of type 1V fracture, and 1 excellent, 1 unsatisfactory and 1 failure in 3 cases of fracture dislocation. 4. In 6 patients with four part lesions treated with open reduction and internal fixation and followed up for an average of 23 months(from 18 months to 3 years and 2 months), only one case of fracture dislocation revealed avascular necrosis with resorption of the head.

8.A Clinical Study of the Tibial Condylar Fractures

Fractures of the tibial condyles, involving as they do weight-bearing articular surfaces and important soft tissues such as collateral ligaments, cruciate ligaments and semilunar cartilages present a variety of problems in treatment and prognosis. Apley, Slee, and others maintained the opinion that most fractures of the tibial condyles could be managed conservatively. On the other hand, Rombold, Schatzker, and others seemed to consider closed treatment to be virtually a form of therapeutic nihilism and adviced open reduction. But recently most authors agree that the method of treatment has to be selected in each individual case, and recommend anatomical reduction of the fracture as possible and early knee motion. During 7 years period from January 1974 to July 1981, of the tibial condylar fractures treated at the department of orthopaedic surgery, Han Gang Shim Hospital, 41 cases that could be followed have been analysed according to the cause, classification, method of treatment and result. The following results were obtained from the analysis of 41 tibial condylar fractures. l. Of the 41 cases, 34 (83%) were male and 7 (17%) were female. 2. Of the 41 cases, 32 (78%) were due to traffic accident, 25 pedestrian, 7 occupant, 8 (20%) were due to fall from height. 3. Of the 41 cases, 31 (76%) were associated with other injuries. 4. Of the 41 cases, 30 were treated by conservative method with functionally acceptable result in 22 cases (73%) and 11 were treated by open reduction with functionally acceptable result in 9 cases (82%). 5. According to the Hohl and Luck's criteria, acceptable functional result was obtained in 31 (76%) cases.

9.A Clinical Study of the Tibial Condylar Fractures which had Open Reductions

The tibial plateau fracture is a fracture of the proximal end of the tibia, involving the articular surface and the supporting metaphysis of a major weight bearing joint. This fracture frequently accompanies soft tissue injury such as ligaments and menisci. So it often produces disability of the knee joint. Of the tibial plateau fractures which had been treated by open reductions at Keimyung University Hospital between May 1971 and December 1981, the 28 cases that were followed for 1 year to 3 years have been analysed according to its cause, classification, treatment and result. Open reduction is indicated in anyone with one or more of the following deformities as shown by roentgenograms: a compression greater than 6-8mm and a split greater than 5mm of the condyle. Anatomical considerations and a clinical end result study of the 28 fractures emphasize that the unacceptable results were from comminution of the fracture, associated soft tissue injury, osteoporosis, infection and other conditions that prevent early knee motion. Twenty-two cases (79 per cent) out of Twenty-eight revealed the rating of “ACCEPTABLE” according to Roberts criteria.

10.Bone Peg Fixation of the Femoral Head Fracture Associated with Posterior Dislocation of the Hip

Seven cases of traumatic fractured femoral heads with posterior dislocation of the hip were treated at the Keimyung University Hospital between November 1973 and December 1981. All cases were followed from 6 months to 8 years. Five cases were caused by traffic accidents. The other two cases were caused by a landslide. In six cases, at first a closed reduction was performed under general anesthesia and the femoral head was reduced in the acetabulum, but in 5 of the 6 cases head fragments were not reduced. So, after one or more days, open reductions were performed on the remaining 5 cases by internal fixation with iliac bone pegs (two-four). In the remaining one case of the 6, who had a head injury and an intestinal rupture, after the closed reduction, one head fragment was reduced but not anatomically. But because of his poor general condition, conservative treatment was taken. During conservative treatment redislocation and displacement of the head fragment occurred at the 4th week after the closed reduction was done. So this case was also treated by the above method. The 7th case, who has an ipsilateral femoral neck and intertrochanteric fracture, was reduced by primary open reduction. By the classification of the Pipkins, there were 4 cases of Type I and Type II, III and IV each had I case. The post operative X-ray and follow-up study showed that two were classified as excellent, four as good and the one who had the ipsilateral femoral neck and intertrochanteric fracture (Pipkin's Type III) was classified as poor.
