OBJECTIVE: The cytogenetic analysis for earlier detection of fetal chromosome aneuploidies is performed from chorionic villus using either long-term culture or direct chromosome preparation. To analyze the cause of pregnancy loss, we also attempt the cytogenetic study in product of conception(POC) using chorionic villi or fetal tissue. But the failure of analysis often occurs in direct preparation of villus cells and product of conception(POC). We studied to evaluate the clinical usefulness of FISH in uncultured chorionic villus cells of culture-failed cases. METHODS: According to the patient's indication, we performed FISH for chromosome 18, 21, X and Y in chorionic villi as well as POC and compared FISH results with their chromosomal studies. RESULTS: We found one trisomy 18 and one trisomy 21 in Chorionic Villus Sampling and one trisomy 18 and one monosomy X(45, X) in POC. The averages for accuracy of FISH were 83-91% and all cases are represented consistent results with their chromosomal studies. Among them, we could analyze using FISH only in 5 cases of culture failure including one case of monosomy X in POC. CONCLUSION: We could detect aneuploidy with uncultured chorionic villus cells in case of culture failure, using FISH, it may be the potential method to assist the cytogenetic study.

4.The timing of tooth eruption and root development of permanent canine and premolars in Korean children.

PURPOSE: The aim of this study was to investigate the timing and sequence of eruption of permanent canine and premolars, and to evaluate tooth calcification stage on emergence in Korean children. MATERIALS AND METHODS: The sample was comprised of 1,266 children (male 720, female 546) aged from 7-13 years. Tooth eruption and calcification stages were determined through oral and panoramic radiographic examination, respectively. Probit analysis was used to calculate the timing of tooth eruption and tooth calcification stage from these cross-sectional data. RESULTS: In both males and females, eruption occurred around the time when one third of tooth root or more was formed. The sequence was as follows: first premolar, canine, and second premolar in maxilla, and canine, first premolar and second premolar in mandible. Tooth eruption occurred earlier in girls compared with boys, averaging 0.63 years. CONCLUSION: Eruption sequence is identical in males and females with a trend for females to erupt earlier than males. Tooth eruption becomes earlier over the past decades in Korean children.

5.Three Dimensional Spiral CT of the Tracheobronchial Tree: Determination of the Optimal Window Settings.

PURPOSE: To determine optimal window settings for measuring the inner diameter of the trachea and both mainbronchi by spiral CT. MATERIALS AND METHODS: Chest PA radiography and spiral CT scanning were performed in ten healthy adult volunteers. Three dimensional images were reconstructed (minimal threshold value : -1000HU ; maximal threshold value : from -200 to -900HU, of 50HU intervals) to measure the inner diameter of the trachea and both main bronchi. The results of 3D spiral CT were compared with those of chest radiography. RESULTS: The inner diameters of the trachea, right main bronchus, left main bronchus-I (1cm below the tracheal carina) and left mainbronchus-II (2cm below the tracheal carina) measured by chest radiograph and 3D spiral CT were not significantly different at maximal threshold values of -400 ~ -550HU, -450 ~ -550HU, -450 ~ -600HU and -500 ~ -600HU, respectively (p>0.05). The differences in the results of the two series were statistically significant at other threshold values however (p<0.05). CONCLUSION: We determined optimal window settings for measuring the inner diameter of the trachea and both main bronchi by spiral CT. The optimal maximal threshold values were somewhat different according to measured sites of the trachea and both main bronchi.

6.Evaluation of maxillary sinus using cone-beam CT in patients scheduled for dental implant in maxillary posterior area.

OBJECTIVE: The purpose of this study is to determine the prevalence of sinus disease and abnormalities in patients scheduled for dental implant in maxillary posterior area using cone beam CT. PATIENTS AND METHOD: One hundred five maxillary sinuses in eighty-seven patients who underwent cone beam CT for dental implant in maxillary posterior area were included. Any patients who had previous history of sinus operations were not included. The sinus abnormalities were classified as follows ; normal (membrane thickness <2 mm), mucosal thickening (membrane thickness > or = 2 mm and <6 mm), partial opacification (membrane thickness >6 mm but not full), full opacification and mucous retention cyst. The relationship between the remaining bone height, sinus symptoms and maxillary sinus abnormality was statistically surveyed. RESULTS: Of 105 maxillary sinuses in 87 patients, 80 (76%) maxillary sinuses showed abnormalities ; 4 of 4 symptomatic patients and 76 of 101 asymptomatic patients. Mucosal thickening was the most common sinus abnormality. Only 3 (4%) of 80 maxillary sinus abnormalities were caused by the odontogenic origin. The prevalence of maxillary sinus abnormalities was higher in the symptomatic group than asymptomatic one (p<0.05). CONCLUSION: Maxillary sinus abnormalities were very common in the patients who were planning implantation in maxillary posterior areas. This result supports that thorough evaluation for maxillary sinus is recommended when implant treatment is planned for those areas.

8.Rapid detection of aneuploidy in uncultured fetal cord blood cells by FISH ( Fluorescence In Situ Hybridization ).

OBJECTIVE: To determine the fetal aneuploidy in fetal blood cells from cordocentesis. METHODS: We analyzed their karyotype and performed fluorescence in situ hybridization(FISH) for chromosome 18, 21, X, and Y in 14 cases of fetal blood cells from cordocentesis at Department of Obstetrics & Gynecology, College of Medicine, Seoul National University and Hamchoon Women's Clinic. RESULTS: In all cases we obtained the consistent results in both methods and were able to rapidly detect aneuploidy in uncultured fetal blood cells using FISH before karyotyping with culture for 48 hr. The averages for accuracy of FISH were from 84.6 % to 93.9%. CONCLUSION: In this study we suggest that the rapid detection in uncultured fetal blood using FISH is possible and that this diagnostic method will be clinically useful when rapid result would be demanded.

9.Squalene-Induced Lipoid Pneumonia in Rabbits: High-Resolution CT and Pathologic findings.

PURPOSE: To describe the HRCT and pathologic findings of squalene-induced lipoid pneumonia in rabbits. MATERIALS AND METHODS: Three ml of squalene was instilled into the trachea between the second and the third tracheal ring in 16 rabbits. Serial HRCT scans were obtained on day 4 (n=8), at 1 week (n=7), 2 weeks (n=1), 4 weeks(n=1), 6 weeks (n=2) and 20 weeks (n=1) after squalene instillation. With sacrifice of the rabbits pathology was reviewed at 1 week (n=3), 4 weeks (n=3), and 6 weeks (n=4) after CT scans. RESULTS: Lipoid pneumonia was induced in 8 rabbits; lesions were distributed mainly in the dependent posterior lung. On serial HRCT scans, airspace consolidation, as seen on an air-bronchogrm, and nodular opacities were early findings; these gradually diminshed and with time were replaced by nodular & linear opacities. Histologically, pulmonary fibrosis appeared one week after squalene instillation and progressed over time. Alveolar septal thickening and cuboidal change of the alveolar lining epithelium were more prominent at week 6. CONCLUSION: The early change of squalene-induced lipoidpneumonia in rabbits is the proliferation of intraalveolar macrophage, which is responsible for air-space consolidation with air-bronchograms on HRCT. Nodular and linear opacities on HRCT are due to the appearance of pulmonary fibrosis one week after squalene instillation, and its subsequent progression.

10.Detection of Down Syndrome & Edward Syndrome in uncultured amniocytes using FISH ( Fluorescence In Situ Hybridization.

FISH is suggested as a possible method to detect the numerical and structural abnormalities of chromosomes in interphase nucleus. We performed this study to discuss the clinical usefulness of FISH in uncultured amniocytes and to set up the cut-off value for further study. We collected amniotic fluid samples from patients whose chromosome studies were recommended due to screen positive for Down and Edword syndrome in triple marker test using maternal serum. The centromeric probe for chromosome 18 and the locus-specific probe for chromosome 21 were used and the results were compared to their karyotypes. We could find 2 cases of trisony 21 and 2 cases of trisony 18 and the other cases represented normal karyotypes. The accuracies were 91% for disomy 18, 89% for trisomy 18, 92% for disomy 21 and 88% for trisomy 21. Therefore FISH technique is a possible method to detect the chromosomal abnormalities in uncultured amniocytes and the use of locus-specific probe for chromosome 21 would be more useful for detecting the aneuploidy of chromosome 21 than 13/21 centromeric probe.
