4.Efficacy of hydrocolloid occlusive dressing technique in decubitus ulcer treatment: a comparative study.

The efficacy of hydrocolloid occlusive dressing technique was compared with that of the conventional wet-to-dry gauze dressing technique in decubitus ulcer of stage I and II. Forty-four patients were randomly divided into two treatment groups and each received treatment according to the two different protocols. As a result, 80.8% of the hydrocolloid occlusive dressing group (group 1) and 77.8% of the conventional wet-to-dry gauze dressing group (group 2) healed completely with no statistically significant difference between the two groups. However, the time required for complete healing was shorter in group 1 with 18.9 days compared to 24.3 days in group 2. Ulcer healing speed was also slightly faster in group 1 with 9.1 mm2/day compared to 7.9 mm2/day for group 2. Average treatment time spent by a medical staff member was significantly shorter in group 1 with 20.4 minutes/day compared to 2017 minutes/day in group 2. The hospital cost of the ulcer treatment was higher in group 2 compared to group 1 even without taking into consideration the medical personnel's labor cost. These results indicate that the hydrocolloid occlusive dressing technique offers less time consuming and less expensive method of treatment compared to the conventional technique in stage I andII decubitus ulcers.

5.Neuropsychological Characteristics of Mild Cognitive Impairment in Parkinson Disease and Subcortical Vascular Mild Cognitive Impairment.

BACKGROUND: Patients with Parkinson's disease and mild cognitive impairment (PD-MCI) and patients with subcortical vascular mild cognitive impairment (svMCI) are known to have cognitive dysfunction. The aim of this study was to determine the cognitive characteristics that can be used to differentiate these two conditions. METHODS: Twenty-two PD-MCI and 22 svMCI patients seen in our neurodegenerative disease clinic, and 22 normal controls were recruited. These three groups were matched in terms of age, gender, and education level. Mild cognitive impairment was diagnosed according to the criteria of Petersen. svMCI should meet the criteria modified from those of Erkinjuntti. RESULTS: Analysis of covariance revealed that frontal executive dysfunction was prominent in both the PD-MCI and svMCI groups after adjusting for depression. The scores in word fluency and the Stroop test were lower in the svMCI group than in the PD-MCI group. Both groups exhibited decreased performance in verbal and visuospatial memory tests. The score in the verbal recognition test was lower in the PD-MCI group than in the svMCI group, which exhibited comparatively reduced visuospatial memory. The depression level was higher in the PD-MCI group than in the svMCI group, as assessed using the Geriatric Depression Scale. CONCLUSIONS: The impairment in frontal executive function was more severe in the svMCI group than in the PD-MCI group, which displayed severe impairment in verbal memory function. These differences in cognitive function may help to differentiate PD-MCI from svMCI, and to further the understanding of their pathophysiologies. PD-MCI patients also exhibited severe depressive mood, which raises the possibility of cognitive improvement through therapy.

Li-Fraumeni syndrome (LFS) is an autosomal dominant hereditary disorder characterised by a variety of different tumor types in children and young adults. That contains with a germline mutation in the tumor suppressor gene Tumor Protein p53 (TP53). That is extremely rare. Furthermore, this is sometimes overlooked. Here, we report a case of LFS which was confirmed by mutational analysis of the p53 gene. Also, literature review is intended to improve understanding of this disease entity.

10.Loss of retinoblastoma gene and amplification of N-myc gene in retinoblastoma.

We have analyzed paired samples of genomic DNA from peripheral leukocyte and primary tumor tissue from nine patients with retinoblastoma (RB) and from two RB cell lines, WERI-Rb-1 and Y79, to detect the molecular alterations of the retinoblastoma susceptibility gene (RB-1) and N-myc gene. In Southern analysis, RB-1 deletions in tumor tissues were detected in five patients (56%), one of these revealed a total loss of RB-1. N-myc amplification was found only in one (11.1%) out of nine patients. We also observed a total loss of RB-1 in WERI-Rb-1, and a more than 100-fold amplification of N-myc in Y79. The analysis of the relationship between molecular events and clinical characteristics such as age, sex, tumor laterality did not reveal any specific correlation. These results suggest that genetic backgrounds of RB in Korean patients are quite similar to those of reported cases elsewhere. The high sensitivity of our method in detecting the RB-1 loss indicates that this method can be a useful tool for initially screening a large number of tumors.
