3.Validation of the ACS NSQIP Surgical Risk Calculator for Patients with Early Gastric Cancer Treated with Laparoscopic Gastrectomy

Purpose:
The American College of Surgeons National Surgical Quality Improvement Program (ACS NSQIP) risk calculator is useful in predicting postoperative adverse events. However, its accuracy in specific disorders is unclear. We validated the ACS NSQIP risk calculator in patients with gastric cancer undergoing curative laparoscopic surgery.
Materials and Methods:
We included 207 consecutive early gastric cancer patients who underwent laparoscopic gastrectomy between January 2018 and January 2019. The preoperative characteristics and risks of the patients were reviewed and entered into the ACS NSQIP calculator. The estimated risks of postoperative outcomes were compared with the observed outcomes using C-statistics and Brier scores.
Results:
Most of the patients underwent distal gastrectomy with Roux-en-Y reconstruction (74.4%). We did not observe any cases of mortality, venous thromboembolism, urinary tract infection, renal failure, or cardiac complications. The other outcomes assessed were complications such as pneumonia, surgical site infections, any complications requiring re-operation or hospital readmission, the rates of discharge to nursing homes/rehabilitation centers, and the length of stay. All C-statistics were <0 and the highest was for pneumonia (0.65; 95% confidence interval: 0.58–0.71). Brier scores ranged from 0.01 for pneumonia to 0.155 for other complications. Overall, the risk calculator was inconsistent in predicting the outcomes.
Conclusions
The ACS NSQIP surgical risk calculator showed low predictive ability for postoperative adverse events after laparoscopic gastrectomy for patients with early gastric cancer. Further research to adjust the risk calculator for these patients may improve its predictive ability.
4.Three Cases of Parameatal Urethral Cyst.

We herein report three cases of pararneatal urethral cyst, which developed on the male urethral meatus. These cysts occurred since birth or childhood, and did not interfere with urinary of sexual function. Histologically, these cysts were lined by pseudostratified columnar epithelium with variable thickness. No recurrence was evident after simple excision.

5.Trichofolliculoma in Korean Patients: A report of six cases.

Trichogenic tumors are a very rare group of primary cutaneous neoplasms and a few tumors have been recognized. Trichofolliculoma is a benign, papular, organoid adnexal tumor of the skin and presents as a solitary skin-colored nodule. We report six cases of trichofolliculoma occurred in Korean patients consisting of three male and three female patients. The duration of disease was variable from 3 months to 5 years. The site of tumor was limited to the face and included eyelid in two cases, nose in two cases, cheek and posterior aspect of ear in each one case. The size of the mass varied from 4 to 8 mm in diameter. Microscopically, all the cases showed large central cystic hair follicle with numerous secondary follicular buds, producing a radiating pattern. The stroma was fibrotic and well demarcated from the adjacent dermis.

7.Research on zirconium alloys with low elastic modulus and low magnetic susceptibility for improving bone resorption and susceptibility artifacts

The objective of this study was to fabricate Zr-Cu alloys with low elastic modulus and low magnetic susceptibility for the use as the dental biomaterials. Metallic implants, such as stainless steel, Co-Cr alloys, and Ti alloys, could be magnetized in the strong magnetic field of the MRI instrument and produce artifacts (magnetic susceptibility artifacts) on the image. The areas that cause the artifacts on images are closely related to the magnetic susceptibility. This susceptibility artifact is caused by metals, such as Ti, Co, and stainless steels, inside the body. Therefore, metallic implants with a low magnetic susceptibility are preferable for surgery performed with the assistance of an MRI. Recently, Zr-based alloys have been attracting interests as biomaterials due to their excellent mechanical properties and low magnetic susceptibility. The magnetic susceptibility of Zr-Nb was half that of Ti-6Al-4V. In addition, the magnetic susceptibility of Zr-Mo was reported almost one-third that of commercially pure Ti and Ti-6Al-4V alloy.However, there are limited reports concerning metallic biomaterials with low magnetic susceptibility for dental applications. The magnetic susceptibility of the Zr-Cu binary alloys was extremely low, approximately 10-7 ; this level is approximately one order less than that of pure Zr and other commercialized Ti-based metallic biomaterials. The Zr-Cu binary alloy exhibited moderate compressive strength (1261~1565 MPa), yield stress (432~595 MPa), favorable elongation (14~34%), high elastic energy (7.2~19.3 MJ/㎥ ) and low elastic modulus (20~28 GPa). Consequently, Zr-Cu binary alloys have the potential to be used as biomaterials with nullifying magnetic properties for magnetic resonance imaging diagnosis and a good combination of mechanical properties indicates them potential biomaterials for biological hard tissue materials

9.A Case of Dermatofibrosarcoma Protuberans on the Face with Various Cutaneous Lesions.

Dermatofibrosarcoma protuberans is a tumor of intermediate malignancy, characterized by its aggressive local growth and marked propensity to recur after surgical excision. Dermatofibrosarcoma protuberans is most frequently located on the trunk or proximal extremities and rarely occurs on the face. In most cases, the tumor initially occurs as an asymptomatic, indurated plaque that slowly increases in size, and then develops multiple firm nodules. We report a case of a 39-year-old female who presented with a 10-year history of asymptomatic, brownish and hypopigmented plaques on the right side of her upper forehead. They gradually extended downwards and erythematous nodules appeared on the superior border of pre-existing brownish plaques 8 months ago. Histopathological examination of a nodule showed spindle-shaped cells with a storiform pattern in the dermis, and that of the plaques showed a parallel pattern. The tumor cells were stained positive for CD 34 upon immunohistochemical staining.

10.Isolated Biliary Granulocytic Sarcoma Followed by Acute Myelogeneous Leukemia with Multilineage Dysplasia: A Case Report and Literature Review.

Granulocytic sarcoma is a rare extramedullary tumor composed of myeloid progenitor cells. Primary involvement of the biliary tract without evidence of leukemia is exceedingly rare. Here, we report an isolated biliary granulocytic sarcoma in a 30-yr-old man who presented with jaundice, fever, and chill without any evidence of leukemia. However, five months after the diagnosis, he developed acute myelogenous leukemia with multilineage dysplasia and chromosomal abnormality. A rare possibility of biliary granulocytic sarcoma should be considered as a differential diagnosis in patients with obstructive jaundice. A histologic evaluation by aggressive diagnostic intervention is important and may improve prognosis.
