Objective The aim of this study is to observe the clinical characteristics and surgical effects of macular hole retinal detachment in high myopia patients with pars plana vitrectomy (PPV)and secondary internal limiting membrane (ILM)peeling.Methods This was a retrospective study.The clinical data of 1 5 patients (1 5 eyes)with macular hole retinal detachment and high myopia,who underwent primary PPV and secondary ILM peeling,were analyzed,including disease history,refraction diopter,ocular axis length,posterior scleral staphyloma,BCVA,macular reattachment and macular hole heeling.There were 3 males (3 eyes)and 12 female (12 eyes),the average age was (60.80 ± 5.85 )years.All patients were examined by best corrected visual acuity (BCVA ), slit lamp microscopy with 90D pre-lens, indirect ophthalmoscopy,A scan and optical coherence tomography (OCT).After the first PPV and silicone oil tamponade,a shallow retinal detachment around the macular hole,especially around the scleral staphyloma was detected by OCT.During the 2nd surgery to remove the silicone oil,ILM peeling and C3 F8 tamponade were performed.Results The average refraction diopter was (- 12.6 ± 1.86)D,the average ocular axial length (29.82±0.993)mm and the average disease duration was (5.20±1.24)months.All eyes had total retinal detachment of all four quadrants,choroid detachment and macular choroidal atrophy,and type ⅡCurtin posterior scleral staphyloma. After the second surgery, all had retina attached by fundus examination.OCT examination indicated that macular hole closure in 7 eye,macular hole attached and retinal attached in 8 eyes.Their BCVA improved after both the first and second surgery (P =0.000),the BCVA after second surgery was better than that after first surgery (P =0.038).Conclusions The clinical characteristic of our series of patients were as follows:long history,with choroidal detachment and type ⅡCurtin posterior scleral staphyloma.All 1 5 eyes showed retinal attached after secondary ILM peeling.The secondary ILM peeling and C3 F8 tamponade may improve the visual outcome and retinal reattachment rate.

Objective To analyze satisfaction of the clinical practice content of nursing postgraduates with different characteristic, and to provide transformative suggestions for the construction of clinical practice scheme of nursing postgraduates. Methods Using judgment sampling and snowball sampling, the questionnaire survey was conducted among 101 nursing postgraduates, and comparing satisfaction of clinical practice content of postgraduates with different age, work experience and teaching mode. SPSS 17.0 software package was used for data description and t test. Results The total satisfaction of clinical practice content of nursing postgraduates was low, and different postgraduates′satisfaction was different. On practical operation opportunity,> 30 years old of postgraduates′satisfaction was (3.95±0.87), which was higher than those 20~30 years old, (3.35±0.90), and the difference was statistically significant, t=2.748, P<0.01. On clinical practice assessment methods and frequency, the satisfaction of postgraduates with a fixed teacher was higher than those without a fixed teacher (3.40±0.74) vs. (2.83±0.87), and the difference was statistically significant, t=3.310, P<0.01. Conclusions At present, the clinical practice content of nursing postgraduates is in shortage, and need to be further improved.

Objective To evaluate the efficacy,local control rate,survival rate and adverse reaction of intensity modulated radiation therapy (IMRT) combind with Tp concurrent chemoradiotherapy (docetaxel and cisphtin) for superior segment of esophageal cancer.Methods Seventy-eight patients with superior segment of esophageal cancer were divided randomly into IMRT combined with TP concurrent chemoradiotherapy group (chemoradiotherapy group) and single IMRT group (radiotherapy group) by double blind method with 39 cases each.The patients in chemoradiotherapy group received chemoradiotherapy,while other patients in radiotherapy group received only radiotherapy.For both groups,the same radiation technic was given with chemoradiotherapy,the total dose was 6 400 cGy/32 f.For the patients in chemoradiotherapy group were also given with TP concurrent chemotherapy weekly.The efficacy,local control rate,survival rate and adverse reaction was observed.Results Complete remission in 17 cases,partial remission in 20 cases,stable in 2 cases,the total effective rate was 94.9％ (37/39) in chemoradiotherapy group.Complete remission in 9 cases,partial remission in 19 cases,stable in 11 cases,the total effective rate was 71.8％ (28/39) in radiotherapy group.There was significant difference in the total effective rate between two groups(P ＜ 0.05).The 3-year local control rate was 69.2％ (27/39) in chemoradiotherapy group,which was higher than that in radiotherapy group[35.9％(14/39)],and there was significant difference(P ＜ 0.05).The 3-year survival rate was 74.4％(29/39) in chemoradiotherapy group,which was higher than that in radiotherapy group [43.6％(17/39)],and there was significant difference (P ＜0.05).Conclusion IMRT combined with TP concurrent chemoradiotherapy can improve the efficacy,local control me,survival rate of esophageal cancer,but can also increase the acute toxic effect.

Objective　To assess fetal cardiac hypertrophy and ventricular function in fetuses of diabetic mothers. Methods　Study group was consist of 23 fetuses of diabetic pregnancies. Control group was consist of 108 fetuses of normal pregnancies. Cardiac size was measured by ultrasound as inter-ventricular septal (IVS) thickness and ventricular diastolic and systolic (VD and VS) dimensions and perimeter. Cardiac function was expressed as ventricular shortening fraction 1 (VSF1) (ventricular diastolic perimeter -ventricular systolic perimeter/ventricular diastolic perimeter)and ventricular shortening fraction 2 (VSF2) (ventricular diastolic dimension-Ventricular systolic dimension/ventricular diastolic dimension).Fetal birth weight was recorded. Results　 IVS of study group （6.1±0.7） mm, which was significantly thicker than IVS of control group （4.4±0.7） mm. Left ventricular diastolic dimension (LVDD) and Left ventricular systolic dimension (LVSD) of study group were （3.3±0.8）cm2 and （2.2±0.6） cm2, which were greater than those of control group （2.6±0.6） cm2 and （2.0±0.5） cm2. Right ventricular diastolic dimension (RVDD) and Right ventricular systolic dimension (RVSD) of study group were （3.9±0.7） cm2　and （2.6±0.7） cm2, which were also greater than those of control group （3.5±0.5） cm2 and （2.4±0.7） cm2. Left ventricular shortening fraction 1 of left ventricular (LVSF1) of study group was 0.20±0.07, which was greater than that of control group （0.12±0.04）. LVSF2 of study group was 0.35±0.11，which was greater than that of control group too （0.23±0.10）. Right ventricular shortening fraction 1 of left ventricular (RVSF1) of study group was 0.19±0.05，which was greater than that of control group （0.13±0.11）. RVSF2 of study group was 0.33±0.09，which was greater than that of control group （0.23±0.14）. Estimated fetal birth weight of study group was （4 076±608） g, which was greater than that of control group （3 440±377） g. Conclusion　These findings showed that fetal hyperinsulinemia result in fetal weight increasing, global cardiac enlarger inter-ventricular septal thickness larger and revealed hypercontractillty of ventricle in fetuses of diabetas mothers.

Objective To investigate the effects of high glucose on the proliferation of hepatic stellate cell-T6(HSC-T6) and the expressions of transformation growth factor-?1(TGF-?1),platelet-derived growth factor(PDGF) and precollagen Ⅲ(PCⅢ).Methods Based on glucose groups of different concentration,we observed the proliferation of HSC in 30min-16h time period,and used the methods of immunohistochemistry and radioimmunoassay to measure the expressions of TGF-?1,PDGF and PCⅢ in the supernatant at 48h and 72h.Results In 2-16h time period,the proliferation of HSC was increased stepwise over time in 1500-6000mg/L group,and was more visible in 4500-6000mg/L group.The expressions of TGF-?1 and PDGF increased at 48h,and the expression of PCⅢ in the supernatant increased at 72h in 4500-6000mg/L group compared with that in glucose control and hypertonic control groups.Conclusion High glucose can promote hepatic fibrosis by stimulating the expressions of TGF-?1,PDGF and PCⅢ in HSC-T6.

Objective To construct the lentiviral RNA interference vector targeting TPX2 and to obtain the human cervical cancer HeLa cell strain stably infected by TPX2-shRNA for studying the relationship between human cervical carcinoma and TPX2 gene.Methods By targeting TPX2 gene,four double-stranded DNA hairpin structures corresponding to shRNA were designed,synthesized and connected with Pglv2-U6-Puro to construct the recombinant plasmids.Then these recombinant plasmids were transformed into DH5α competent cells.The positive clone was extracted and transfected into 293T cells for virus packages after sequenced correctly.Human cervical carcinoma HeLa cell infected by these recombinant lentiviral was screened by Puromycin,then stable cell strain was obtained.The silencing effect of TPX2 in HeLa cell was detected by RT-fluorescent quantitative PCR and Western blot.Cell cycle and cell apoptosis wer detected by Flow cytometry.Results Sequencing results confirmed that 5 lentiviral was packaged successfully.The steady cell strain transfered TPX2-shRNA was screened with 0.4 μg/mL puromycin.HeLa cells infected by recombinant lentivirus all play the gene silencing effect especially in the group of TPX2-shRNA-1.In the group of TPX2-shRNA-1,TPX2mRNA (0.21 ± 0.07) and protein (0.19 ± 0.28) rela tive expression level is lower than those in the control group (1.08±0.07) (P＜0.01) and(0.64±0.03) (P＜ 0.01)respectively;G2 and S-phase cells are higher than those in the control group (P＜0.05)and the apoptosis rate was significantly more than those in the control group (P＜0.05).Conclusions The effective TPX2 genetic interference sequence was obtained,lentiviral vectors carrying TPX2shRNA was successfully constructed,and the HeLa cell strain with TPX2 silenced was successfully screened,which lay the research foundation for the study of the role of TPX2 in cervical cancer.

Objective To explore the serum folic acid (FOL)and vitamin B12 (Vit B12)levels in gigantic young cell anemia (MA)in the diagnosis and treatment effect.Methods Select in March 2010 to March 2013,examined with clear cell mor-phology in the diagnosis of 50 patients with gigantic young cell anemia as observation group.Another selection during the same period to hospital for a medical 52 cases of healthy people as control group,observation group treated with FOL and Vit B1 2 8~1 5 d,with SIEMENS SIEMENS ADVIA Centaur CP automatic chemiluminescence immune analyzer to observe group before and after treatment and the control group was used to detect the serum FOL and Vit B1 2 levels,respectively, for all the test results were analyzed.Results Observation group before treatment serum FOL and Vit B12 levels (2.23± 0.11 ng/ml and 125.21±94.14 pg/ml)significantly lower than the control group (5.49± 3.91 ng/ml and 356.70±185.46 pg/ml),differences were statistically significant (t=5.89,P<0.01;t=7.90,P<0.01).Observation group after treatment serum FOL and Vit B12 levels (4.26±1.83 ng/ml and 307.43±139.81 pg/ml)were significantly higher than those before treatment (2.23±0.11 ng/ml and 125.21±94.14 pg/ml),differences were statistically significant (t=7.83,P<0.01;t=7.52,P<0.01).Observation group after treatment serum FOL and Vit B12 levels (4.26 ± 1.83 ng/ml and 307.43 ± 139.81 pg/ml)and control group (5.49±3.91 ng/ml and 356.70±185.46 pg/ml),there was no statistically significant difference (t=1.78,P>0.05;t=1.51,P>0.05).Conclusion The detection of serum FOL and Vit B12 levels,can be used as a gigantic young cell anemia is the effective means to the diagnosis and treatment effect monitoring.

The effect of extraction and nonextraction treatment on the soft tissue profile of the subjects borderline Angle classⅡ division 1 malocclusion is concerned mainly by orthodontists.This article reviewes the effect of extraction and nonextraction treatment on the soft tissue profile of the subjects with Angle classⅡ division 1 borderline patients and the important factors(such as differences between the types of tooth extraction and growth).The aim is to guide clinic diagnosis and treatment for borderline Angle classⅡ division 1 malocclusion.

Objective　To investigate the characteristics of fetal growth in uterus. Methods　A total of 695 term singleton infant were studied. Various ultrasound measurements (including Biparietal diameter BPD, Transverse abdomen-diameter AD, femur length FL) obtained from 26 weeks' gestation to term. 147 cases were macrosomia. 48 cases were intra-uterine growth retardation (IUGR). 500 cases were normal weight. We assessed the tendency of BPD/AD and AD/FL of fetal. Results　BPD/AD ratio of macrosomia was lowest, and BPD/AD ratio of IUGR was highest of them. The tendency of BPD/AD ratio were declined in gestation. AD/FL ratio of macrosomia were highest, and AD/FL ratio of IUGR was lowest of them. The tendency of AD/FL ratios were smooth in gestation. Conclusion　The tendency of fetal growth in uterus showed that the add of abdominal diameter contributed to gain in weight.
