Nuclear factor-E2 related factor 2 (Nrf2)can protect the normal cells from the damage of oxidants and electrophones, preventing tumorigenesis and metastasis. However, the abnormal activation of Nrf2 in tumor cells can lead to tumor development.Chemicals acting on Nrf2 pathway include activators and inhibi-tors,Nrf2 inhibitors can increase the chemosensitivity of the tumor, providing a new idea for the drug design of high-efficiency and low-toxicity.

[Objective]To measure the feasibility and efficiency of treating vertical sacral fractures with iliac hollow screws.[Method]Sixteen patients with vertical sacral fractures were treated.10 cases of Denis type Ⅰ and 6 cases of Denis type Ⅱ.[Result]The average following-up period was 16 months. All fractures united within 3 months. Two cases showed symptoms of root L5 injuries.1 case totally recovered and 1case partly recovered by the 3nd month after operation.[Conclusion]It showed simple and easy to master for clinical applying of treating vertical sacral fractures with iliac hollow screws.

Objective To investigate the multiple factors correlated to femoral head necrosis in patients over 45 years old with femoral neck fractures after closed reduction and cannulated screws fixation. Methods Three hundred patients with femoral neck fracture were admitted from May 1999 to May 2004 and treated by closed reduction and internal fixation with cannulated screws. We analyzed the multiple factors:age, gender,Garden classification,follow-up time, procedure delay, quality of reduction, time of full-weight-loading,fixation removal, and preoperative traction.Multiple correlation factors were analyzed by SPSS10.0 statistic system. Results Complete case records were documented in 99 cases who were followed up for an average of 24.5 months(range,8 to 60 months).Femoral head necrosis occurred in 15 cases(15.2%).Time of femoral head necrosis was 8 to 50 months after operation. The combination of quality of reduction, preoperative traction, age, and age?fixation re- moval was found to have the most significant influence on femoral head necrosis after internal fixation for femoral neck fractures. Conclusions Good reduction is the most important factor correlated to the outcome of a femoral neck fracture.Age is linked to the risk of necrosis but it is relatively insignificant.Preoperative traction in wrong positions can increase the incidence of necrosis.Garden classification is important in predicting femoral head necrosis and is associated with quality of reduction, but it is not a risk one in the combination of multiple correlation factors that lead to the necrosis.

The microstructure of cationic cyclopeptide (TD-34) treated Caco-2 cell membrane was observed, and we discussed the relationship between membrane structure and insulin transmembrane permeability. Atomic force microscope (AFM) was used to observe living cell membrane in air condition and tapping mode. Results showed that the surface of Caco-2 cell membrane treated with TD-34 lost its smoothness and nearly doubled its roughness. Apparent permeability coefficients (P(app)) of insulin in Caco-2 cell monolayers increased 2.5 times. In conclusion, AFM can be used to observe microstructure of cationic cyclopeptide treated cell membrane and cationic cyclopeptide enhanced insulin delivery across Caco-2 cell membrane by increasing membrane fluidity.

AIM: To determine 5 different aphrodisiacs, including apomorphine, yohimbine, vardenafil, sildenafil and alprostadil, adulterated into Chinese proprietary medicine by LC/MS(n),and build a laboratory library including MS(n) spectra and retention time. METHODS: ZORBAX SB C_18 column(150 mm?4.6 mm,5 ?m)was applied. The mobile phase consisted of acetonitrile and 0.5% formic acid changing by linear gradient elution, and the flow rate was 1.0 mL/min.The ion trap mass spectrometer with positive ion mode electrospray(ESI?) was applied. RESULTS: In the mass spectrum apomorphine, yohimbine, vardenafil and sildenafil showed abundant pseudomolecular ion +,and alprostadil showed abundant pseudomolecular ion -.All the MS(n) spectra showed characteristic product ion. CONCLUSION: It is proved that the method is simple, reliable and suitable to determin aphrodisiac adulterated to health food.

Objective To explore the clinical characteristics, diagnosis and therapeutic strategies of acute appendicitis in pregnancy. Methods The clinical data of 45 cases with acute appendicitis in pregnancy, which had been diagnosed by surgeries in our hospital since Jan 2010 to Jun 2016,were retrospectively analysed. Results All patients had exhibited abdominal pain.Among them,15 patients(33.3%)had exhibited shifting pain in right lower quadrant, and 44 patients(97.8%)had abdominal tenderness.4 cases with appendiceal gangrene and 1 case with necrosis of the great omentum had been found during the surgeries. There were 5 patients had occurred complications after the surgeries, including 2 cases with poor wound healing,1 case with septic shock,1 case with inflammatory ileus and 1 case with incompletely adhesive ileus.Threatened abortion and threatened premature delivery were found in 11 patients(during 22+3 to 36+2 weeks gestation)after the surgeries.Finally,1 patient occurred abortion(25+3 weeks gestation)and 3 patients occurred premature delivery(during 28+4 to 30+4 weeks gestation). Conclusion Abdominal pain as the main clinical manifestations of the pregnancy with acute appendicitis, prone to severe complications.

Objective To detect the levels of 25-hydroxyvitamin D3 and analyse the related clinical factors in patients with chronic kidney disease (CKD).Methods Patients with CKD in our department from March 1st to July 1st were enrolled continuously. The level of 25-hydroxyvitamin D3 and intact parathyroid hormone(iPTH) were detected by electrochemiluminescence and immunochemiluminescent respectively.Serum calcium, phosphorus and albumin were measured by automatic biochemical instrument. Results 127 patients were selected and the average age was (60.9?15.3).The mean level of 25-hydroxyvitamin D3 was (12.06?6.41)?g/L.82.6% patients had vitamin D deficiency and 96.9% patients had vitamin D insufficiency. The level of 25-hydroxyvitamin had no statistics difference D3 between stage 1,2 and 3 CKD patients but was much hihger than that of stage 4 and non-haemodialysis stage 5 patients. The level of 25-hydroxyvitamin D3 was not related to serum calcium, phosphorus and iPTH,while positively related to albumin and eGFR and negatively related to serum creatinine and total cholesterol. Conclusion Vitamin D deficiency and insufficiency are frequent in CKD patients and deteriorate with the progress of kidney function impairment. The level of 25-hydroxyvitamin D3 is not related to the traditional CKD-MBD index such as serum calcium, phosphorus and iPTH.

Objective To explore the influence of continuous veno-venous hemofiltration (CVVH) therapy on cardiac index (CD, global end-diastolic volume index (GEDI) and extravascular lung water index (ELWI) monitored by Pulse-indicated continuous cardiac output (PiCCO). Methods The 12 critically ill patients with acute renal failure were selected consecutively from department of intensive care unit (ICU) in Beijing Hospital. The patients who received CVVH therapy were monitored by PiCCO plus device. The hemodynamic measurements were performed during the process and interruption of CVVH. Pared t-test was used to analyze the values of CI, GEDI and ELWI. Results A total of 48 groups of data from 12 patients were analyzed. There were no significant differences in CI [(4.75±0.93) L ? min(-1) · m(-2)vs. (4.69±0.89) L · min(-1) · m(-2)], GEDI [(780.60±109.30) ml/m2 vs. (784.75± 106.20) ml/m2] and ELWI CC11.61±3.45) ml/kg vs. (11.54±3.56) ml/kg] between the interruption and process of CVVH, as obtained by the pared t-test (all P>0.05). Conclusions The CVVH therapy has no effect on the accuracy and reliability of PiCCO measurement of CI, GEDI and ELWI.
