1.MR imaging of intracranial tuberculosis: Emphasis on development of lesions and therapeutic effect.

We retrospecively analyzed MRI findings of 14 patients with intracranial tuberculosis according to the disease process before anti-tuberculous medication. We also analyzed the correlativity between these findings and the clinical outcome after anti-tuberculous medication in 8 patients who had taken follow-up MR imaging. MR imaging was performed with either a 0.5T(1 patients) or 1.0T(13 patients) superconducting system with spinecho pulse sequences. Abnormalities on MRI were seen in 87%(12/14) of the patients. MRI findings consisted of meningeal enhancement(n=10), parenchymal tuberculoma(n=6), hydrocephalus(n=5), and infarction(n=5). Most of them were found within 3 months from symptom-onset. Parenchymal tuberculomas were found after 1 month of symptom-onset. All cases of hydrocephalus and infarction were associated with meningeal enhancement. On follow-up MRI after anti-tuberculous medication, changes of MR imaging did not correlate with clinical outocome in 4(50%) of 8 patients. Improvement of MRI findings were observed in patient treated for more than 3 weeks. In conclusion, MRI is a good modality in the diagnosis of intracranial tuberculosis, however, the MRI findings are not always correlated with clinical outcome after, anti-tuberculous medication.

4.Two Cases of Allopurinol Hypersensitivity Syndrome in Patients Receiving Thiazide Therapy due to Hypertension.

Allopurinol hypersensitiviy syndrome (AHS) is a rare immunologic response to allopurinol, characterized by multiple findings such as skin rash, fever, hepatic dysfunction, decreased renal function, leukocytosis and eosinophilia. The risk developing AHS tends to increase in patients receiving thiazide therapy or in those who have a pre-existing renal disease. We report two cases of AHS in patients who were taking thiazide medication due to hypertension and underlying renal disease. They developed an erythematous skin rash, fever, renal dysfunction, and eosinophilia after allopurionol therapy.

5.Microneedle Transdermal Drug Delivery Systems for Allergen-Specific Immunotherapy, Skin Disease Treatment, and Vaccine Development

Transdermal drug delivery systems (TDDSs) overcome the hurdle of an intact skin barrier by penetrating the skin to allow molecules through. These systems reduce side effects associated with conventional hypodermic needles. Here, we introduce novel microneedle (MN) TDDSs that enhance drug delivery by creating micron-sized pores across the skin. Many MN TDDSs designed to deliver a diverse array of therapeutics, including allergen-specific immunotherapy, skin disease treatments, and vaccines, are under pre-clinical and clinical trials. Although epicutaneous approaches are emerging as new options for treating food allergy in many clinical trials, MN TDDSs could provide a more efficient and convenient route to deliver macromolecules. Furthermore, MN TDDSs may allow for safe vaccine delivery without permanent scars. MN TDDSs are a major emerging strategy for delivering novel vaccines and treatments for diseases, including skin diseases, allergic diseases, and so on.
6.A Statistical Survey on 1448 Skin Tumors for the Recent 10 Years.

BACKGROUND: Many studies focused on the distribution of skin tumors in Korea, but none of them included whole data refered from deparments other than dermatology and primary physicians. OBJECTIVE: This study was to investigate the incidence and distribution of skin tumors among Korean people. METHODS: A study was made on 1448 cases of skin tumors which were obtained mainly from surgical pathology specimen during the period of 10 years from 1992 to 2001 at Department of Dermatology in Keimyung University Dongsan Medical center. RESULTS: 1. Of the 1448 cases of skin tumors, 1171 cases(80.9%) were benign tumor, 277 cases (19.1%) were malignant tumor and male to female ratio was 1: 1.1. 2. Among the 1171 cases of benign tumors, 174 cases of melanocytic nevi, 173 cases of epidermal cyst, 109 cases of seborrheic keratosis, 62 cases of granuloma pyogenicum were noted. 3. Among 277 cases of malignant tumors, 107 cases of basal cell carcinoma, 58 cases of squamous cell carcinoma, 27 cases of metastatic cutaneous carcinoma, 24 cases of Bowen disease were noted. 4. The predilection sites of Intradermal nevi were face(38.4%), trunk(17.4%), scalp(16.3%), and those of epidermal cyst were face(41.0%), trunk(29.5%), neck(8.1%). The predilection site of basal cell carcinoma, squamous cell carcinoma was face(80.4%, 58.6%). CONCLUSION: The 1448 cases of skin tumors, which were confirmed histologically from surgical pathology specimen at Department of Dermatology in the Keimyung University Dongsan Medical Center during the period of 10 years from 1992 to 2001, were reviewed.

7.Risk factors of stoma re-creation after closure of diverting ileostomy in patients with rectal cancer who underwent low anterior resection or intersphincteric resection with loop ileostomy.

PURPOSE: The aim of this study was to identify the risk factors of stoma re-creation after closure of diverting ileostomy in patients with rectal cancer who underwent low anterior resection (LAR) or intersphincteric resection (ISR) with loop ileostomy. METHODS: We retrospectively reviewed 520 consecutive patients with rectal cancer who underwent LAR or ISR with loop ileostomy from January 2005 to December 2014 at Chonnam National University Hwasun Hospital. Risk factors for stoma re-creation after ileostomy closure were evaluated. RESULTS: Among 520 patients with rectal cancer who underwent LAR or ISR with loop ileostomy, 458 patients underwent stoma closure. Among these patients, 45 (9.8%) underwent stoma re-creation. The median period between primary surgery and stoma closure was 5.5 months (range, 0.5–78.3 months), and the median period between closure and re-creation was 6.8 months (range, 0–71.5 months). Stoma re-creation was performed because of anastomosis-related complications (26, 57.8%), local recurrence (15, 33.3%), and anal sphincter dysfunction (3, 6.7%). Multivariate analysis showed that independent risk factors for stoma re-creation were anastomotic leakage (odds ratio [OR], 4.258; 95% confidence interval [CI], 1.814–9.993), postoperative radiotherapy (OR, 3.947; 95% CI, 1.624–9.594), and ISR (OR, 3.293; 95% CI, 1.462–7.417). CONCLUSION: Anastomotic leakage, postoperative radiotherapy, and ISR were independent risk factors for stoma re-creation after closure of ileostomy in patients with rectal cancer.

8.A Case of Cutaneous Metastasis from Carcinoma of the Hypopharynx.

Distant metastases from squamous cell carcinoma of the head and neck occur at many sites, most commonly lung and bone. Metastases to the skin are unusual, and account for fewer than 10% of all distant metastatic lesions. A 61-year-old man developed cutaneous metastasis two years later after the diagnosis of squamous cell carcinoma of the hypopharynx had been made. He was presented with asymptomatic multiple erythematous hard nodules with telangiectasia on the right shoulder. This lesion was diagnosed histologically as poorly differentiated squamous cell carcinoma. The skin metastases improved with radiotherapy and palliative chemotherapy, including cisplantin, 5-fluorouracil.

10.A Case of Perforating Calcific Elastosis in a Multiparous Women after Laparoscopic Surgery.

Perforating calcific elastosis is a localized cutaneous acquired disorder most frequently in obese, multiparous, middle-aged women. We report a case of perforating calcific elastosis in a 66-year-old multiparous woman who had asymptomatic yellowish cobblestoned plaque with keratotic papules scattered on its surface in the periumbilical region. She have been treated to laparoscopic surgery 5 years ago. The biopsy specimen showed basophilic calcified elastic fibers in the dermis that undergo transepidermal elimination. The von Kossa stain showed dense deposition of calcium in this area.
