1.A Study on Assessment of CAPs (Client Assessment Protocols) using MDS-HC 2.0 on City Elderly .

BACKGROUND: This study used MDS-HC 2.0 (Minimum Data Set-Home Care) to analyze the health and the state of function of the traveling health objects. This study was intended to make use of it with the basic materials for providing them with traveling health service suited for the requirement on the health of the traveling health objects. METHODS: The subjects of this study were 1160 people (over 65 years) living under management control of local Health Center from September 7th to October 3th, 2006. This study was analyzed with inter RAI program & SPSS/WIN 10.0, chi-square -test, t-test, and ANOVA. RESULTS: The result showed that 8.97 CAPs per an elderly person was identified and the subjects over 60% had an injury problem from a fall, health prevention service, vision, IADL, pain, and cognition. The number of CAPs in general was high in higher age and the less educated, and those without a job. But, in subjects that had a life partner and a spouse, the number of CAPs was low. In CAPs by the distinction of sex, CAPs which was much more in man than women in statistics were the improvement of health, the abuse of alcohol and drinking wine, bedsore, and the weak supply system. CAPs which women had much more were the function of the heart and the lungs, pain, the performance of the doctor's advice, health prevention service, and incontinence of urine and insertion of catheter. According to the results comparing CAPs by the level of the ADL, the number of CAPs was shown that the group of ADL 2 was higher than group ADL 1. The matter in which the traveling health service had to be applied in all both ADL1 and ADL2 was injury from a fall, health prevention service, and vision. Conculsion: MDS-HC is applicable to decide the care needs for health and social service supplies. The results can be further applicable for careplan, and referral criteria in continuum of care service over long-term care spectrums.

2.Unipolar Versub Bipolar Hip Hemiarthroplasty for Fractures of Femoral Neck in the Dlderly.

Among the 85 cases of prosthetic replacement of femoral neck fracture which had been operated at Department of Orthopaedic Surgery, Korea Veterans Hospital from January, 1982 to May, 1990, 32 cases (15 unipolar hemiarthroplasty and 17 bipolar hemiarthroplasty) were examined clinically and radiologically at an average 7.7 years postoperatively. And the results were as follows; 1. The group I (Unipolar hip hemiarthroplasty without cement) had Harris hip score - average 79.4. The group II (Unipolar hip hemiarthroplasty with cement) had Harris hip score average 87.3. The group III (Bipolar hip hemiarthroplasty without cement) had Harris hip score average 80.5. The group IV (Bipolar hip hemiarthroplasty with cement) had Harris hip score average 86. 2. There were 2 cases of stem loosening (13%) among 15 cases in unipolar hip hemiarthroplasty and 1 case (6%) among 17 cases in bipolar hip hemiarthroplasty. 3. The movement of two components in bipolar hip hemiarthroplasty was noted in all cases and the movement of the outer bearing joint was more active than that of the inner. And the movement of the two components was not correlated with the use of cement. But Harris hip score was superior cement stem group to noncement stem group. 4. Unipolar hip hemiarthroplasty showed the similar clinical and radiological results with bipolar hip hemiarthroplasty of femoral neck fracture in the elderly. As a result, unipolar and bipolar hip hemiarthroplasty were considered the proper treatment method of the femoral neck fracture in the elderly.

3.Surgical Decompression for the Impingement Syndrome of the Shoulder

The impingement syndrome has an anatomical character that occur against the anterior edge and the undersurface of anterior third of the acromion, coracoacromial ligament and acromioclavicular joint. The treatment consists of conservative and surgical things. We consider the surgical intervention only in cases fail to control pain by conservative treatment over 3 months(from Feb. 1990 to Jan. 1994). The author experienced 15 cases (14 pts) in positive impingement sign & test and evaluated by shoulder arthrography in all cases. 1. The mean age was 60 years ranged from 48 to 68 years and the average symptom duration was 23 months. 2. There were all positive impingement sign & test clinically. 3. There were partial tear of rotator cuff in 3 cases and complete tear in 12 cases radiologically. 4. The anterior acromioplasty with division of coracoacromial ligament was performed in all cases, additional rotator cuff repair in 3 cases and bicipital tenodesis in 2 cases. 5. At the follow up based on UCLA shoulder rating scale, results were excellent & good in 13(86%) and unsatisfactory in 2(14%) cases.

4.Spinal Stenosis in Paget's Disease: 1 Case Report

In the paget's disease involving the vertebra, the symptom and sign of compression come from encroachment on the spinal cord due to the enlarging vertebral bodies, pedicles and laminae. The cord compression appears to be preceded by a long insidious period of noncharacteristic clinical symptoms with minor neurologic dysfunction. The lumbar syndrome of Paget's disease is classified according to its severity, clinical findings, biochemical factors and radiologic patterns. The severity of neurologic complication in Paget's disease involving the vertebra varies according to the level involved. The outcome depends on the suddeness of symptom onset and the effectiveness of decompression. We have experienced a case of Paget's disease involving long bones and lumbar spine in 56 year old male patient, in which surgical decompression produced improvement. We report a case of spinal stenosis in paget's disease with brief review of literature.

5.Successful Electroconvulsive Therapy for a 74-year-old Female with Major Depressive Disorder and Tardive Tremor: A Case Report and Literature Review

Tardive tremor is an infrequently form of tardive syndrome that is developed from prolonged treatment with dopamine receptor blocking agents. This condition presents as a prominent tremor that may cause significant distress but currently lacks effective treatment. Electroconvulsive therapy (ECT) has been applied to treat tardive syndrome. In this study, we report a 74-year-old female patient with major depressive disorder, whose tardive tremor and depressive symptoms showed remarkable improvement after receiving 10 sessions of ECT treatment.
6.Occipital lobe seizure due to hypertensive encephalopathy in youth.

BACKGROUND & OBJECTIVES: Hypertensive encephalopathy is an acute neurologic syndrome characterized by abrupt and marked elevation of blood pressure, headache, vomiting, seizure, visual disturbance, and altered mental status. This syndrome may occur as a complication of toxemia, renal artery stenosis and acute glomerulonephritis. We report 4 young patients with occipital lobe seizure, as a presenting sign of hypertensive encephatopathy, whose brain MRI and perfusion scans showed lesions on bilateral occipital lobes. Case : Four young patients experienced moderate to severe headache, visual illusion, generalized seizure, and loss of sight for a few days. Their systolic blood pressure was 150-170mmHg and diastolic blood pressure 100-120mmHg. Three patients had proteinuria and hematuria as the results of acute glomerulonephritis. CSF findings were within normal limits. EEG showed intermittent generali,ed or bilateral temporoparietooccipital slowings in all patients and continuous parietooccipital slowings in one patient. Brain MRI man showed hyperintensity signal in T2WI and hypointensity signal in TlWl on bilateral occipital lobes and SPECT scan showed normal perfusion three patients and increased perfusion on occipital areas in one patient. They were treated with antihypertemsive agents and anticonvulsants (phenobarbital or valporic acid) for 2-10 months. Seizure was well controlled and didn't recur though discontinuing anticonvulsant. CONCLUSION: In young age, acute hypertension may cause clinically occipital lobe seizure and radiologically bilateral occipital lobe lesions.

7.Research on zirconium alloys with low elastic modulus and low magnetic susceptibility for improving bone resorption and susceptibility artifacts

The objective of this study was to fabricate Zr-Cu alloys with low elastic modulus and low magnetic susceptibility for the use as the dental biomaterials. Metallic implants, such as stainless steel, Co-Cr alloys, and Ti alloys, could be magnetized in the strong magnetic field of the MRI instrument and produce artifacts (magnetic susceptibility artifacts) on the image. The areas that cause the artifacts on images are closely related to the magnetic susceptibility. This susceptibility artifact is caused by metals, such as Ti, Co, and stainless steels, inside the body. Therefore, metallic implants with a low magnetic susceptibility are preferable for surgery performed with the assistance of an MRI. Recently, Zr-based alloys have been attracting interests as biomaterials due to their excellent mechanical properties and low magnetic susceptibility. The magnetic susceptibility of Zr-Nb was half that of Ti-6Al-4V. In addition, the magnetic susceptibility of Zr-Mo was reported almost one-third that of commercially pure Ti and Ti-6Al-4V alloy.However, there are limited reports concerning metallic biomaterials with low magnetic susceptibility for dental applications. The magnetic susceptibility of the Zr-Cu binary alloys was extremely low, approximately 10-7 ; this level is approximately one order less than that of pure Zr and other commercialized Ti-based metallic biomaterials. The Zr-Cu binary alloy exhibited moderate compressive strength (1261~1565 MPa), yield stress (432~595 MPa), favorable elongation (14~34%), high elastic energy (7.2~19.3 MJ/㎥ ) and low elastic modulus (20~28 GPa). Consequently, Zr-Cu binary alloys have the potential to be used as biomaterials with nullifying magnetic properties for magnetic resonance imaging diagnosis and a good combination of mechanical properties indicates them potential biomaterials for biological hard tissue materials

9.Effect of Surface Roughness on the Mechanical Properties of Ceramic-Polymer Blocks for CAD/CAM Dental Restoration

The effect of surface roughness of ceramic-polymer CAD/CAM blocks on the mechanical properties was investigated in this study. Commercially available Polyglass (Vericom, Korea) and Enamic (Vita, Germany) were selected for this purpose. They were cut into either (4.0×2.1×17.0) mm and (3.0×4.0×17.0) mm, followed by grinding, and polished sequentially with 6 µm and 1 µm diamond paste. Flexural strength, fracture toughness, and Weibull analysis were determined according to ISO 6872 Dentistry-Ceramic materials. The elastic moduli were calculated from a stress-strain curves under flexural loading. The statistical significances of the mechanical properties between the products and surface roughness were analyzed with ANOVA and pared t-test at a significance level of 0.05. After grinding with 6 µm diamond paste after cutting by observing with an atomic force microscope, the arithmetic average roughness decreased to 47~49% and the maximum roughness decreased to 68~69%. When polishing with 1 µm diamond paste, The average roughness decreased to 13~22% and the maximum roughness decreased to 16~19%. When the flexural load was applied, stress increased linearly and fractured without plastic deformation both Polyglass and Enamic. As the surface roughness decreased, the mechanical properties were increased both Polyglass and Enamic. However, the mechanical properties of Polyglass increased up to P3, while Enamic showed almost maximal values at E2, after that there was no significant differences between E2 and E3. It could be due to the different microstructure between two blocks used in this experiment.

10.Ankle Arthrodesis by Chevron Osteotomy: Report of 16 cases

The Marcus ankle arthrodesis was carried out in 16 patients at department of orthopaedic surgery of Korea Veterans Hospital from January 1989 to December 1993. All of patients were reviewed and evaluated with subjective and functional rating scales according to the Mazur scoring system. The average follow up was 23 months, ranging from 12 months to 60 months. The Marcus ankle arthrodesis involves a chevron osteotomy, tibial onlay graft medially, internal fixation with staples and screws. According to the Mazur score, the results were 6 cases of Excellent, 7 cases of Good. The average Mazur score was 75.1. There were 4 cases of complication, a non-union, a superficial wound infection, a bursitis, and a tendon injury. In summary the Marcus ankle arthrodesis by chevron osteotomy seems to be a good procedure, which gives solid bony union with short duration, relief of pain, normal looking ankle configuration without leg length discrepancy and good function with few complications.
