3.Quadricepsplasty and Arthrolysis in Stiff Knee

Many people who sustain injuries to the femur or muscles of thigh are handicapped for life by varing degree of limitation of motion of the knee joint flexion such as squatting or kneeling. Many surgical procedures were reported by Thompson (1944), Judet (1959), van Nes (1962), Nicoll (1963), Hesketh (1963), Jahng (1965), Han (1979, 1986), and Daoud (1982). During the period from March 1987 to August 1989, 12 cases of stiff knee joint were treated surgically in our clinics. l. 12 cases of stiff knee treated by modified Judet technique of quadricepsplasty and arthrolysis. 2. The stiff knee was corrected successfully without release of rectus femoris muscle in our cases. 3. The string tie was very useful in postoperative care. String tie was applied with the knee in a full flexed position and when it was discarded, knee was extended and quadriceps setting exercise was begun. 4. In 10 cases, plate removal was added, in two eases, combined bone surgery and quadricepsplasty for malunion or nonunion of the femur, which showed good results. 5. The average flexion achieved by operation was 119.6, the average flexion gain 70.8, while the average extension gain 8.8. 6. By Judet criteria, the result of 11 cases were excellent and one case showed fair result.

4.Revitalization of Idiopathic Osteonecrosis of the Femoral Head by Vascular Pedicled Iliac Bone Grafting

Osteonecrosis of the femoral head seems to be due to circulatory disturbance of the femoral head, usually idiopathic in origin, but also associated with chronic alcoholism, gout, prolonged treatment with corticosteroids, sickle cell disease, Caisson's disease and surface and degenerative changes in the hip joint. Aims of this procedure are to decompress the femoral head hence allowing better circulation, to revitalize the dead head by insertion of vascularized pedicle bone and to give a mechanical support preventing further collapse of the femoral head. Authors reviewed 19 cases of the vessel pedicled iliac bone graft performed for the idiopathic osteonecrosis of femoral head from January, 1984 to April, 1988 with follow-up periods from 23 months to 66 months, average 37.5 months. The purpose of this follow-up note is to report the long term follow-up result and to show the critical points of this technique.

7.Characteristics of Shigella sonnei Isolated in Kyungsangbuk-do in 1998: Biochemical Characteristics, Biotyping, Antibiotic Resistance Pattern, and Plasmid Profile.

Thirty nine strains and 109 strains of Shigella sonnei were isolated from the outbreaks of Youngchun and Kyungju, respectively, while 15 strains were isolated from sporadic cases of eight regions in Kyungbuk province from September to November in 1998. We investigated the relationship among the S. sonnei strains by using biochemical characteristics, biotyping, antibiotic resistance pattern, and plasmid profile. Among the isolates, seven strains of S. sonnei isolated in Youngchun showed gelatin hydrolyase positive but the others showed gelatin hydrolyase negative. One hundred and fifty two strains were a type, while eleven among thirty nine strains isolated in Youngchun were g type. Antibiotics resistance patterns of S. sonnei strains isolated in Youngchun and Kyungju were significantly different. Thirty nine strains of S. sonnei isolated in Youngchun were resistant to SM, TE, and TMP/SMX, while eighty six of S. sonnei among one hundred and nine strains isolated in Kyungju were resistant to AM, CB, K, SM, TE, and TMP/SMX. Antibiotics resistance patterns of residual twenty three isolates were similar to those of eighty six strains. The Plasmid profiles of strains of S. sonnei isolated from the Kyungju were different from those of S. sonnei strains isolated in Youngchun. The Plasmid profiles of S. sonnei strains isolated from Youngchun were identical to those of a S. sonnei strains randomly selected from the outbreak in Daegu in 1998. The Plasmid profiles of S. sonnei strains isolated from Kyungju were identical to those of two strains of S. sonnei randomly selected from the outbreaks of Kanglung and Wonju in 1998. From the above results, it is considered that the strains of S. sonnei isolated from Kyungju and Youngchun region are not identical clone.

8.Reclassification of the Medullary Carcinoma and It's Clinical Characteristics.

PURPOSE: Reclassfication of the medullary carcinoma using a strict histologic criteria and analysis of the clinical and pathological characteristics of the medullary carcinoma. MATERIAL & METHODS: Thirty-seven cases of the breast carcinoma originally diagnosed as medullary carcinoma were reviewed. One to ten microscopic slides of each case were reexamined and reclassified using the strictly defined histologic criteria defined by Ridolfi et al. Tumors were excluded from the category of the typical medullary carcinoma (TMC) on the basis of presence of glandular features, focal marginal infiltrations, or sparse mononuclear infiltrations. Tumor with two or more atypical features, or extensive marginal infiltrations, no mononuclear cell infiltration and/or less than 75% syncytial growth were classified as infiltrating ductal carcinoma with medullary feature (IDC). A predominantly syncytial growth pattern (75% or more) was requisite for inclusion in both TMC and atypical medullary carcinomas (AMC). RESULTS: Twenty-two tumors (60%) fulfilled the criteria for TMC, and 12 tumors (32%) were AMC and three tumors (8%) were IDC. TMC occupied 3.1% of breast cancer. The mean age of patients with TMC was 45.4+/-11.2 years and the average size of the tumor in TMC was slightly larger than that of breast cancer in general, although not statistically significant. The frequency of lymph node metastasis in TMC was similar to breast cancer in general. Five year survival of patients with TMC was 95.5% which was significantly better than breast cancer in general. CONCLUSION: The TMC occupied 3.1% of breast cancer. The mean age of patient, tumor size and lymphnode metastasis were not different from that of breast cancer but 5 years survival of patient with TMC was significantly better than breast cancer in general.

9.A Case of Familial Hypercholesterolemia with Diabetes Mellitus.

Familial Hypercholesterolemia is the most common hyperlipoproteinemia during the childhood, which occurs from the mutation of genes that regulates low-density lipoproteins (LDL), and is classified into two types, the homozygote presented abnormal genes from both parents and the heterozygote presented an abnormal gene from each parent. Type IIa familial hypercholesterolemia is characterized by, especially, increased level of LDL which is common type and normal level of high-density lipoproteins. The clinical signs are arterosclerosis, xanthoma of the Achilles tendons and the arcus cornea. The treatments are dietary and drug therapy. This report is a case of familial hypercholesterolemia diagnosed as type IIa hyperlipoproteinnemia with Insulin Dependent Diabetes Mellitus.

10.A Case of Multiple Bleeding Pseudoaneurysms Complicating Acute Pancreatitis.

Arterial pseudoaneurysm is rare but potentially a catastrophic complication of pancreatitis because it can cause massive gastrointestinal bleeding. Since surgical treatment of arterial pseudoaneurysm has a high mortality, percutaneous angiographic embolization of bleeding artery has recently been advocated as the alternative therapy. Acute hemorrhage into the peritoneal cavity or pseudocyst complicating chronic pancreatitis was frequently reported. However, multiple pseudoaneurysmal hemorrhage without pseudocyst complicating acute pancreatitis are extremely rare. Here, we present a case of multiple pseudoaneurysmal hemorrhage secondary to acute pancreatitis, which were managed successfully by percutaneous angiographic embolization.
