4.The karyotype of Paramphistomum explanatum (Creplin, 1849) obtained from Korean cattle.

As a series of systematic classification of paramphistomes, in the first step, paramphistomes in the rumen and reticulum were collected on 170 Korean cattles (2-3 years age, male) slaughtered at Jeonju abattoir from July 1984 to September 1985 and were classified by means of morphology of the worms. Afterwards, the karyotype of Paramphistomum explanatum (Creplin, 1849) which is the common in Korean cattle was detected by means of modified air-drying method from testis cells of the worm. The following is a brief summary of the leading facts gained through the experiment. Most of the cattle slaughtered at the abattoir were infected with paramphistomes. The 5 species of the worms were detected on 170 Korean cattle and the worm burden per head was from 2 to 784 (on the average 170) worms, 120(70.59 percent) heads out of them involving 2-100 worms. In 28,900 individuals of paramphistomes obtained on 170 Korean cattle, appearance rates of various worms were as follows : 49.74 percent in P. explanatum, 48.08 percent in P. cervi, 0.98 percent in Orthocoelium orthocoelium, 0.89 percent in Fischoederius cobboldi and 0.14 percent in Cotylophoron cotylophorum. The chromosome number of 620 P. explanatum in the haploid and diploid cells was n=9 and 2n=18, and abundant cells in meiotic division were observed; 1,420 haploid and 38 diploid cells were reliable. Nine pairs of mitotic chromosomes were homologous and the chromosomes were composed of five medium-sized metacentrics (m), subtelocentrics (st) or submetacentrics (sm)and four small-sized subtelocentrics (st) or submetacentrics (sm), while meiotic metaphase chromosomes were composed of five medium and four small-sized. The haploid of the testis cells showed C-band in the centromeric region from 8 of them, whereas the remaining chromosome No. 5 included heterochromatin on the tip region, and chromosomes No. 3 and No. 7 showed a remarkable C-band distinguished from other chromosomes.

6.Mechanism and management of burr entrapment:A nightmare of interven-tional cardiologists

Entrapment of the burr within calcified lesion is an uncommon, but serious complication during rotational atherectomy and usually needs surgical retrieval. We report a case series of this complication and also review the possible mechanisms, such as kokesi phenomenon or insufficient pecking motion with decreased rotational speed. We also review the potential techniques ever proposed to rescue this complica-tion percutaneously, including simple manual traction, balloon dilation to release the trap, snaring the burr as distal as possible for forceful local traction and wedging the burr with a child catheter to facilitate retrieval. Gentle pecking motion of the burr for sufficient ablation and shortening the run less than 15 s may avoid such complications. Interventional cardiologists using the rotablator should be familiar with the tips and tricks to avoid and rescue this complication.

8.Work Stress and Mental Health of the Industrial Workers.

Information on work stress and mental health status of industrial workers was required for optimum staffing and health care management. This study dealt with the relationship between mental health status examined by PSI(psychiatric symptom index) and socio-demographic variables. Especially this study attempted to find relevant determinants of the stress and mental health through multiple stepwise regression analysis based on data obtained from 687 occupational workers. The findings of this study were as follows; Mean scores of symptom dimension were higher in youth, female, more educated, unmarried and divorced, and workers in the noisy condition and short duration in job. And it showed high scores in long duration in job for anxiety; college educated for anger; unmarried for cognitive disturbance. The factors affecting the mean scores of psychiatric symptom index were varied according to the types of symptom; age, sex and duration in job for anxiety; age, sex, living with parent, education and marital status for anger; sex and marital status for depression; noisy condition and sex for cognitive disorder.

9.Flexible Intramedullary Nailing for Femoral Fractures

Various methods of internal fixation have been used for the treatment of femoral fractures. In 1970, Ender and Simon-Weidner described their method of flexible intramedullary nailing for the treatment interterochanteric and subtrochteric fractures. This method was widely used and obtained good result but Pankovich described that the intertrochsnteric fraeture, particullary unstable, is the worst indication due to many complications and difficulties in the procedure, We used flexible intramedullary nailing fot 115 cases of the femoral fractures from Jan. 1985 to Mar, 1988 at the Department of Orthopedir Surgery, Soonchunhyang University Hopsital with following results. 1. The intertrochanteric, subtrochanteric and shaft fractures were healed within 12.6 weeks, 15.8 weeks and 17.3 weeks respectively, however, unstable or comminuted fractures took more time to be healed. 2. The most frequent complication was shortening (20 cases), flollowed by nail migration, broken nail, delayed and non-union in orders. 3. Nonunion and delayed union were frequently occured at the distal shaft fractures and developed broken nail. the healing time is longer than those of other sites, So, the flexible intramedullary nailing on the distal shaft fractures of femur may be inappropriate. 4. In 8 cases of unstable or comminuted fractures of subtrochanteric and shaft, open reduction and cerclage wiring were done before nail insertion. We could prevent shortening and start early weight bearing and obtain the good result of bone union. 5. In 8 cases of locking with screw through the nail eyes, there was no nail migration and knee joint pain was almost none. 6. Among 4 cases of secondary Ender nailing and bone graft, one case was infected and the others were united after 11.7 weeks. In nonunion, this method may be useful. 7. Varization and shortening were developed in the unstable intertrochanteric fractures but clinically they were not significant. We recommed ender nailing for the unstable intertrochanteric fractures for experienced surgeons.

10.The Use of Inset Patella Prosthesis in Total Knee Arthroplasty

The types of patellar component of total knee replacement prostheses are dome-shape, metalbacked, anatomically shaped, and press fit condylar type, etc. The fixation of inset patella type is achieved with bone cement supplemented by a central peg and a small extended flange around the circumference, providing resistance to torsional and shear loads. From October 1990 to June 1992, 53 cases in 40 patients had total knee arthroplasties. Ortholoc II implant was used in all patients. Cementless femoral and tibial component were used. But, cement was used in patellar component. The average follow up was 29 months(range 21-40 months). There were 37 women and 3 men and the average age was 51 years(range 22-71 years). The diagnosis of 40 patients was osteoarthritis in 25, rheumatoid arthritis in 9, infection sequelae in 6. For alignment of the patellar and congruency of the patellofemoral joint, additional retinacular release was done in 7 cases. The average preoperative Hospital for Special Surgery(H.S.S) knee rating score was 50.7 point, which improved to 89.4 point at last follow up. Postoperative patellar pain was 4 cases. In the roentgenorgraphic follow up, ratellar subluxation was 2 cases and lateral tilting of the patella was 5.9 degrees in 10 cases. The average patellar depth was 20.8mm preoperatively and 21.6mm postoperatively. In the Merchant's view, the preoperative lateral patellofemoral angle of Laurine was average 14.3 degree, which changed to average 16.4 degree at last follow up, and the preoperative Merchant's congruence angle was average 14 degrees, which decreased to average
