1.Vitamin D Resistant Rickets

The discovery and synthesis of vitamin D and the elucidation of the role of sunlight in the activation of the vitamin D precursors changed the syndromes of rickets from a therapeutic enigma to a socioeconomic and public health problem. Since Albright in 1937 first described vitamin D resistsnt rickets, which did not respond to treatment with the usual dose of vitamin D, it has progressively become a common form of rickets in practice. In addition, as the result of increasing understanding of renal physiology and careful investigation, a spectrum of renal tubular abnormalities have been identified which cause clinical rickets and which in many cases are insensitive to even large doses of vitamin D. We have reported a case of an unusual form of vitamin D resistant rickets which did not easily respond to treatment with high doses of vitamin D and was associated with hypocalcemia in multiple pathologic fractures in the lower extremities of the patient.

3.Preliminary Study on the Establishment of Proper Portion Using Consumed Size and Food Preference of Frequently Served Meals in the Elementary School Lunch Program in Inchon: II. A Study on the Consumption Size of Frequently Served Meals in the Elementary .

The purpose of this study is setting proper portion by consumed size of food and food preference and then reducing plate wasts in school lunch. The survey was conducted from Oct. 21 to Nov. 22, 1996 through the practical measurement and questionnaires. The subjects were 43 students of 1 elementary school. The results of this survey was as follows : 1. Average consumption size of cooked rice was 115g. Average consumption size of one course dishes was 209g. The soup & pot stewes showed lower consumption rate than the other kind of foods. Average consumption size of fresh & boiled salad was 23.4g Average consumption size of stir fries was 55g. Average consumption size of steams & stewes was 42g. Most grill & fried foods showed high consumption rate compared to most of the other foods. Average consumption size of kimchies was 10.6g. The most of given desserts except apple were all consumed. 2. There was significant correlation between food preference and consumption rate for total food items. 3. Response of satisfaction about serving size of most of given foods except fresh & boiled salads, soup & pot stewes and kimchies were "moderate". 4. The purpose of this study was to evaluate the preference trend of frequently served meals in the elementary school lunch program. Therefore, this evaluation was surveyed on frequently served meals in the elementary school lunch program in Inchon. Also, this study was surveyed on food preference using questionnaire including frequently served food items. Statistical analysis of data was completed using SAS program. The results of this survey was as follows: 1. Freqrenctly served fooods were 56 food items including 5 boiled rice, 3 one course dishes, 10 soup & pot stewes, 10 fresh & boiled salads, 8 stirred fries, 7 stewes, 6 grill & fries, 3 kimchies and 4 desserts. 2. Students liked barley bab more than the other kinds of cooked rice. One course dishes were preferred most by the subjects. Among soup & pot stewes, fermented soybean paste stew showed lower preference than the other kind of stewes. Among fresh & boiled salads, fruits salad was the most favore, whereas, root of bellflower salad was the most hated. Among side-dishes by different preparation method, stir fries, stewes, grill & fries, kimchies, dessert were prefered most by the subjects. There were significant difference between male and female students for most given foods except soup & pot stewes. Female students showed higher preference for most given foods except fresh & boiled salads, kimchies, dessert than male students.

6.The Relationship between Chondromalacia Patellae and Extensor Mechanism of the Knee Joint

Among previously proposed etiological factors of chondromalacia patellae, abnormality in the extensor mechanism of the knee joint has been attributed as a cause of chondromalacia patellae by many authors Malalignment in the extensor mechanism of the knee joint is supposed to cause the positional change of patella. This positional change of patella can be measured by Q angle and P/PT ratio. Hereby we intened to classify the relationahip between the above measurement lenoting the positional change of patella and chondromalacia patellae. These values were obtained from 60 patients found to have chondromalacia patellae in Han Gang Sacred Heart Hospital, from Jan. 1977 to Sep. 1978. Values were also obtained from 60 normal persons as a control group. Thus a statistical analysis was made for the possible relationship between extensor mechanism of the knee joint and chondromalacia patellae. The following conclusion were made, 1. The sex ratio was 1:1.3 with female preponderance. There was 26 male and 34 female patients, common age group of chondromalacia patellae was age 20-40 years. 2. The average value of Q angle was 11 in control group, while 18 in chondromalacia patellae, having the difference of 7. 3. The average values of Q angle among different sexes were reviewed. The male control group had 9 of average value of Q angle, while male chondromalacia patellae had 16. The female control group had 12 of average value of Q angle, while female chondromalacia patellae had 20. 4. The average value of P/PT ratio was 1.03 in control group, while 0.9 in chondromalacia patellae, having the difference 0.13. For P/PT ratio in different sexes, the average value was 1.01 in male control group. while 0.9 in male chondromalacia patellae. The average value of P/PT ratio was 1.04 in female control group, while 0.89 in female chondromalacia patellae, 5. For statistical survey for these differences, P value was examined by t-test methods. The statistical diffrences of Q angle in male control group and chondromalacia patellae was 3.4 and it was 2.06 in he female. The difference between control group and the chondromalacia patellae in the overall seves was 2.61, thus having statistical significance.

7.Proximal Migration of the Intramedullary Nail in Fracture of the Femur

Various devices have been introduced in the field of orthopaedic surgery for the treatment of fractures and dislocations. Widely used among these are metallic nails and pins. A rare complication is the migration of such intermal fixation devices from their original sites of insertion. Examples of migration have been shown in cases for which devices like Kirschner wire and Steinmann pin were used. Acromio-clavicular joint dislocation gives us another good example. Kuntscher nail has beenin worldwide use for the treatment of femoral shaft fractures since the original work of Kuntscher in 1940. Yet few cases were reported, for migration & Kuntscher nail. We, recently had an experience of proximal migration of a Kuntscher nail in a femoral shaft fractnre which was markedly comminuted. The case is presented here with references.

The pedicled fillet flap concept has been successfully applied in both the upper and lower extremities for the treatment of difficult wound. In cases of inevitable extremity amputation, the transfer of pedicled flap from the amputed part is possible. In such substance, it is possible to obtain the pedicled fillet flap from the amputed limb to provide stump coverage. We experienced two cases of below-knee amputation due to severe flame burn which the stump was covered with island pedicled sole-fillet flap.

9.Reconstruction of ankle using free vascularized flap from amputated hand: A case report.

It is indispensable to cover the skin defect when bone or tendon is exposed. In case of inevitable amputation of an extremity, it is possible to harvest a free flap from the amputed limb for providing coverage of the other wound. This technique allows immediate wound coverage without the morbidity of an additional donor site. We experienced an electrical burn case with inevitahle wrist disarticulation and successfully treated soft tissue defect of ankle using free vascularized flap.

10.Statistical Analysis of Serologic test for Syphilis Durinf five Years.

This paper is studied for statistical anaIysis of 1181 cascs of positive V.D.R.L test among 24414 cases who were visited for annual Physical examination and in or outpatients in the Han Gang Sung Shim Hospital during 5 years and 7 months from January 1972 to July 1977. Clinical survey was carried out with 182 syphilitic patients in the Department of Dermatology, Han Gang Sung Shim Hospital during 19 months from January l976 to July 1977. The results were as follows; 1. Positive rate for V.D.R.L test was 4.8%, and there were statistically significant differences in annual incidence(P<0.01). Biologic false positive rate for V.D.R.L test was 26. 7%, and there were statistically aignificant differences in annual incidence(P<0.01). 2. Departmental analysis such as Dermatology, Urology, Physical Examination and others including Obstetrics and Gynecology, Internal Medicine, Pediatrics etc. were carried out, the results showed that there were statistically significant departmental differences in V.D.R.L positive rate, but no in biologic false positive rate. 3. Biologic false positive rate was high inversely proportional to dilution titer, and that was a statistically significant difference(P<0.01). 4. 182 had syphilis, of whom 106(58. 2%) were latent syphilis and 74(40. 7%) were primary and secondary syphilis. 5. 119(65. 4%) among 182 syphilitic patients were male, most of patients(87.8%) occured in age from 21 to 40 years in the both sex. But there no statistically significant differences in age distribution between males and females. 6. The most predilection site of chancre was coronary sulcus(54.5%). 7. The lesions of secondary syphilis were composed of 18 cases(25%) of condyloma latum, 17 cases(23. 6%) of roseolar syphilid, 17 cases of papulo-squamous syphilid, 13 cases of alopecia syphilitica, 1 case of pustular syphilid, and 6 cases of others. 8. Married patients were nearly two times than single. In women, married. patients were much more(80. 3%) than single, and there were statistically significatnt differences(P<0.01). 9. There were no significant differences in S.T.S positive rate of spouse between infectious and latent syphilis. 10. Occupation distribution of male sexual partners were composed of 35 cases(29. 4%) of waitress, 28 cases(23. 5%) of call-girl, 20 cases(16. 8%) of office-girl, 17 cases of housewife, 12 cases of out of work, 7 cases of student. 1I. For patients with primary, secondary, and latent syphilis, treatment consiste of benzathine penicillin 2. 4 million units intramuscularly on the first visit and repeated three penicillin preparations at 7 days intervals in most of patients.
