2.Giant cell tumor in long bones: The significance of marginal sclerosis for the differential diagnosis.

Plain radrographs of thirty nine patients with giant cell tumor of long bone and CT scans of twenty patients among the thirty patients were reviewed retrospectively to evaluate the frequency and significance of sclerosis of the tumor margin. The sclerosis of the tumor margin was observed on plain radiographs in thirteen patients(33.3%) and they were located either on epiphyseal or on both epiphyseal or metaphyseal portion of the tumor. The authors concluded that the giant cell tumor should not be excluded from the differential entities eventhough the tumor has the marginal sclerosis.

3.Metastatic Omental Hepatocellular Carcinoma: Two Cases Report.

We report metastatic omental hepatocellular carcinoma in two patients with post-lobectomy hepatocellular carcinoma who have had previous abdominal surgery or the rupture of hepatocellular carcinoma. Ometal metastatic masses were similar to primary masses of the liver in pathologic findings as well as in radiologic findings.

4.Clinical Features of Cutaneous Polyarteritis Nodosa.

BACKGROUND: Polyarteritis nodosa(PAN) is a disease of necrotzing vasculitis which has a clinical spectrum encompassing those cases of multisystem involveme it and skin-limited variant. The clinical course of cutaneous PAN has been considered to be a benign one, however there is sorne controversy regarding its nosological entity. OBJECTIVE: To characten the clinical course of the patients with cutaneous PAN and determine whether or not it is a benign disease or something more sever. METHODS: Clinical and laboratory findings in 8 patients with Logy-proven cutaneous PAN but without any detectable visceral involvement were observed periochelly during a follow-up period over 18 months. Meticulous physical and pertinent laboratory earinations were performed each time during their visits. RESULTS: Livedo reticulari, on the lower legs was the mostorenon cutaneous findings with these patients. During the follow-up periods there were no suspected abnormal physical and laboratory findings to have other organ involvement in all 8 patients Serologic examinations for antineutrophil cytoplasmic antibody and hepatitis-B surface antigen revealed negative in all patients tested. CONCLUSION: The follow-up periods of 2 years or so does not seem to be long enough in characterizing the clinical features of PAN. However, the cutaneous: on of PAN can be regarded as a benign cutaneous variant of the disease having no visceral lesion of arteritis, even though mild degrees of constitutional symptoms and a few laboratory abnormlites could be seen.

5.Social support and phychosocial distress among white-collar workers.

The effects and roles of social support and stressors (stressful life events and chronic strains) on psychosocial distress were examined in a model with data from a sample of the white-collar workers (N = 416) in Korea. The findings of this study present that mean levels of psychosocial distress were higher' in the female, youth, unmarried and divorced, and low economic status (monthly income). The analysis also shows that, as expected, stressful life and chronic strains as stressors are positively related to occurrence of psychosocial distress, and social support is negatively related to psychosocial distress. Further, the contribution of social support to predicting psychosocial distress is greater in magnitude than that of stressful events, chronic strains, and sociodemographic variables. There is no evidence that social support buffers the impacts of stressors on psychosocial distress, rather social support takes a role of main or direct effects on psychosocial distress. Therefore, the strength of the social support is an important and meaningful strategy to prevent from stress and mental, physical illness.

6.Radiologic analysis of the spinal tuberculosis

Tuberculosis remains high incidental disease in Korea with an estimated incidence of 2.5% in generalpopulation. Among the tuberculosis of bone, spinal tuberculosis is high incidence and curable disease, but earlytreatment demands early diagnosis. Authors reviewed clinical aspects of 376 cases, and reviewed conventional filmsof 74 cases and CT of 8 cases, confirmed histopathologically as spinal tuberculosis from Jan. 1976 to May 1985 atYonsei medical center, Yonsei University. The results were as follows: 1. The frequent site of involvement werelower thoracic and lumbar vertebra, 4th lumbar vertebra was the most common lesion site among them. 2. Theseperated lesions were 10.2% among spinal lesion. 3. The most common type and pattern of bone density wasintervertebral type and mixed pattern each other. 4. Paravertebral abscess, kyphosis and narrowing ofintervertebral disc space were discovered more than 80% in reviewed conventional films. 5. In children, there isno predilection site. 6. Spinal CT was more accurate diagnostic method than conventional study in the evaluationof following aspects: 1) extent of lesion 2) degree of spinal canal involvement 3) changes of surrounding organ.

In Bureger's angiographic findings are not pathognomonic, but in most cases they are diagnostic. We reviewedperipheral angiography of 36 cases of Buerger's disease who were suspected clinically during the period from Jan.1979 to Dec. 1984. The results were as follows: 1. The sex distribution of the cases were 34 males and 2females(17:1) and most common in 4th decade. Eight-six percents of the patients were smokers. 2. Chief complainson admission were ulceration, pain at rest, coldness, and discoloration. 3. The anterior tibial artery wasaffected most commonly (88%) and posterior tibial(76%), peroneal(68%), popliteal(22%), and femoral arteruy(15%) inorder. 4. The angiographic findings were 1) In all cases, there was abscence of arteriosclerotic lesions andshowed smooth vessel walls in non-affected arteries. 2) Direct corkscrew type collaterals were found in 2% of thelesions, and indirect type collaterals in 81%. The tree-root or spider's leg type collaterals were also found in74%. 3) Standing-wave patterns were found in 30% which affected above the popliteal artery. 4) Early venousdrainage was found in 16% of the lesions.

8.Occurrence of Sword bean Scab Caused by Cladosporium cucumerinum in Korea.

A black scab disease occurred on sword bean (Canavalia gladiata) in plastic film houses around Chinju area during the spring season of 1999. The disease started from flower bud, then moved to flower stalk, pod, petiole, cirrus, stem and leaves. The lesions started with small dark brown spots then were gradually expanded. Severely infected plants reached 37.4% of whole plant covered with scab. Numerous conidia were produced on the diseased flower disk, pod, floral axis, stem and leaves. Most of the conidia were appeared to be readily dispersed in the air, but the mycelia were not suggested causing of sooty mold by ectoparasitism. A fungus was isolated from the diseased stem, and inoculated to healthy plants to satisfy the Koch's postulates and proved the fungus was the causal agent of the disease. The isolated fungus grew on potato dextrose agar, forming greenish black to pale brown colonies. Conidia were ellipsoidal, fusiform or subspherical, mostly one-celled but occasionally septated. The conidia were 3.9~34.1x2.7~5.1microm in size and formed in long branched chains on the erected conidiophores which were pale olivaceous brown and variable in length between 7.2~210.7microm in size. Ramoconidia were 7.6~29.2x3.2~14.4microm in size. The fungus was identified as Cladosporium cucumerinum based on the above morphological characteristics. The optimum temperature for mycelial growth and conidial formation was about 15 to 25degrees C. Cladosporium scab of sword bean caused by the fungi has not been reported in Korea previously.

9.Occurrence of Rhizopus Soft Rot on Peach (Prunus persica var. vulgaris) Caused by Rhizopus nigricans in Korea.

A soft rot of fruits caused by Rhizopus nigricans occurred on peach (Prunus persica var. vulgaris ) in The Chinju City Agricultural Products Wholesale Marke during in summer season of 2000. The disease infection usually started from wounding after harvest fruits, and then moved to outside. At first, the lesions started with water soaked and rapidly softened and diseased area gradually expanded. In severely infected film house, the rate of infected fruits reached 65.2%. Numerous sporangiospores were produced on the diseased fruits. Most of the sporangiospores were appeared to be readily dispersed in the air. The mycelia grew surface of fruits and produced stolons. Colonies on potato dextrose agar at 25~30degrees C white cottony at first becoming heavily speckled by the presence of sporangia and the browinish black at maturity, spreading rapidly by means of stolons fired at various points to the substrate by rhizoids. Sporangia were 85.3~243.5x53.4~219.2microm in size and were globose or sub-globose with. somewhat flattened base. The color of sporangia was white at first and then turned black with many spores, and never over-hanging. Sporangiophores were 8.9~36.6microm in width, smooth-walled, non-septate, light brown, simple, long, arising in groups of 3~5 from stolons opposite rhizoids. Sporangiospores was 9.7~24.8x5.9~15.8microm, irregular, round, oval, elongate, angular, and browinish-black streaked. Columella was 70.2x149.7microm. brownish gray, and umberella-shaped when dehisced. The causal organism was identified as. Rhizopus nigricans Lind on the basis of the morphiogical characteristics of the fungus. Rhizopus soft rot on peach (Prunus persica ) caused by the fungi has not been reported in Korea. This is the first report of rhizopus soft rot on peach caused by Rhizopus nigricans in Korea.

10.Diagnostic Value of p53 Expression in the Evaluation of Effusions.

The diagnostic accuracy of routine cytological preparations from effusions ranges from 60% to 70%. Immunohistochemical markers, especially tumor-associated antigens, have been successfully employed to increase diagnostic sensitivity in effusion cytology. However, more than two different antibodies in diagnosis of effusions are needed. In the view of prevalence of abnormalities of p53 gene in human malignancies, we investigated the diagnostic usefulness of demonstration of p53 protein immunoreactivity in distinguishing benign changes versus malignant processes in effusions. p53 protein expression was studied immunohistochemically in 76 effusions(28 malignant and 48 benign) using anti-human p53 antibody. p53 immunoreactivity was identified in 19 of 28(67.9%) malignant effusions. In contrast, no p53 immunoreactivity was!, observed in all benign effusions. A specificity of 100% and a sensitivity of 67.9% Were observed. These results suggest that immunohistochemical detection of p53 protein seems to be helpful in distinguishing benign changes versus malignant processes in effusions, although its principal limitation is-its relatively low sensitivity.
