5.Maternal and perinatal outcomes in triplet versus twin pregnancies with the result of Assisted Reproductive Technology.

The present study was performed to compare maternal and perinatal outcomes in triplet and twin pregnancies with the result of ART(Assisted Reproductive Technology). Each pregnancy of 18 triplet pregnancies with 18 weeks or more was matched for maternal age, EDC(estimated date of confinement), parity, history of preterm delivery, indication of ART with two sets of twin pregnancies. Triplet pregnancies had a significantly shorter gestational age at delivery than twin pregnancies(30.4 versus 34.6 weeks), and a significantly lower mean birth weight(1,514 versus 2,286g). The mean hospital stay was significantly longer in triplets(22.3 versus 10.1 days). The incidences of 5 min Apgar score less than 7, neonatal deaths were significantly more often in triplets than twins; 18(33.3%) vs 6(8.3%), 15(27.8%) vs 7(9.72%), respectively. There were significant differences in the incidence of neonatal complications such as respiratory distress syndrome; 14(25.9%) vs 7(9.72%), ventilatory support; 18(33.3%) vs 5(6.9%), neonatal seizure; 9(16.7%) vs 0, btween the two groups. However, there were no significant differences between the groups in maternal complications or neonatal morbidity such as congenital malformations or hyperbilirubinemia, intraventricular hemorrhage, or bronchopulmonary dysplasia. We suggest that counseling patient regarding the anticipated perinatal outcomes of triplet pregnancies with the result of ART should be conducted with our data.

7.Reproductive Outcomes after Microsurgical Reversal of Tubal Sterilization in Women 36 Years Age or Older.

OBJECTIVE: To determine the reproductive outcomes of women undergoing microsurgical reversal of tubal sterilization at age 36 years or older. MATERIALS AND METHODS: A series of 133 patients who received microsurgical reversal of the previously sterilized fallopian tubes at Seoul National University Hospital from July, 1980 to January, 1992 was reviewed and evaluated for clinical characteristics, pregnancy rates, and factors influencing the outcome of tubal reversal. RESULTS: Of 133 patients, 78 (58.6%) had been sterilized by laparoscopic cautery. Loss of children was a leading cause for tubal reversal. The mean interval from tubal sterilization to tubal reversal was 65.0 months. The overall pregnancy rate was 52.6% (70/133), and the mean interval was 9.4 months from tubal reversal to pregnancy. Excluding 7 patients who were lost to follow-up, 76 pregnancies were confirmed in 63 patients with the delivery rate per patient of 66.7% (42/63). There were no significant differences in age, duration of tubal sterilization, postoperative tubal length between pregnant and non-pregnant groups. CONCLUSIONS: Microsurgical reversal of tubal sterilization could be a justifiable method in women 36 years age or older.

8.Congenital Lobar Emphysema.

We had experienced a case of congenital lobar emphysema in a 3 months old male infant. Chief symptoms included tachypenea, respiratory difficulty, cyanosis, Chest X-ray or chest CT scan revealed extensive emphysematous changes of the right upper and middle lobes, compression of the right lower lobe and shifted of mediastinum to the left side. This condition was appeared in the absence of infection and foreign body in the bronchus and its failure to respond to conservative treatment. This patient was treated by the right upper and right middle lobes pneumonectomy. A brief review of literature was made.

10.Surgical Treatment of Cancer of the Orbit with Lymph Node Metastasis of Parotid Gland.

BACKGROUND AND OBJECTIVES: Malignant orbital tumors represent a diverse group of neoplasms such as malignant salivary tumors, squamous cell carcinoma, sarcoma, melanoma etc. A number of orbital tumors have a potential for lymphatic metastasis, but only a little information about the lymphatic drainage and treatment for malignant orbital tumors has been published. The purpose of this study was to review lymphatic spread of malignant orbital tumors and its surgical treatment of orbital cancer when metastasized to the parotid gland. MATERIALS AND METHODS: Five orbital cancer patients with lymph node metastasis of parotid gland were evaluated retrospectively. They had been treated by resection of primary tumor, parotidectomy, neck dissection and postoperative radiotherapy (except one case) from 1994 to 2000. Histopathology revealed squamous cell carcinoma in three patients, malignant melanoma in one and rhabdomyosarcoma in one. RESULTS: There were no complications postoperatively. Four cases had metastasis to intraglandular lymph nodes in the parotid gland and only one case had metastasis to preauricular lymph node. Two cases had metastasis to preglandular lymph nodes and prevascular lymph nodes in the level I lymph node. Three were NED (no evidence of disease), rhabdomyosarcoma case was DOD (died of disease) due to recurrence in submental area and breast and malignant melanoma case was AWD (alive with disease). CONCLUSION: Most orbital cancers with lymph nodes of the parotid gland have metastasis to intraglandular lymph nodes, but some of them does to extraglandular lymph nodes also. The examination of the parotid gland and the cervical lymph node must be considered in orbital cancers that have a potential for lymphatic metastasis. The results of this study indicates the need to include deep lobe and extraglandular lymph node of parotid gland for surgical removal of disease.
