1.Statistical Analysis and Comparative Study of Breast Volume and Measurements in Korean Females.

A study of the volume and linear measurements of the breast to determine new nipple positions for 142 subjects was undertaken. Volume determination was done by using the Grossman-Roudner device. The size and shape of the female breast may differ significantly, and such size and linear measurements have to be measured accurately prior to breast aesthetic surgery. Statistical analysis of these results are closely related to physical constitution(height, weight, chest circumference). There have been increasing changes in linear measurement and decreasing changes in breast volume during the last 10 years, The results were as follows: 1) the mean distance from the midclavicular point to the nipple was 19.5cm, the distance from the sternal notch to the nipple was 19.4cm, the distance between nipples was 18.6cm, and the areolar diameter was 3.3cm, 2) the regression equation for the midclavicular point to the nipple was 7.236+ 0.00077x height(cm)+ 0.233 x weight(kg), for the sternal notch to the nipple was 8.845-0.0039 x height(cm)+ 0.214x weight(kg), for the distance between nipples was 11.192 - 0.0033xheight(cm)+ 0.151x weight(kg), and for the areolar diameter was 4.031+0.02xheight(cm)-0.048 x weight(kg),3) the mean value of breast volume of the left side was 209.6cc, and that of the right side was 211.8cc, 4) the regression equation for the mean breast volume was -137.673 + 6.668 x weight(kg), which was equal to -528.414 + 9.115x chest circumference under the axilla(cm) and -468.420 + 8.298 x chest circumference across the nipple (cm). In conclusion, these data could be useful for numerous aspects of breast surgery, including augmentation mammoplasty, mastopexy, reduction mammoplasty, and correction of asymmetrical breasts as the standard for Korean females, if we know the body constituents (weight, chest circumference) before surgery.

2.Comparison of Central Corneal Epithelial Healing Rates Between Different Wound Conditions.

The role of limbal epithelial cells on the initial re-surfacing of the central epithelial defect was evaluated. Central or peripheral corneal wounds were produced by using n-heptanol in New Zealand white rabbit, and the epithelial healing rates were evaluated. Central corneal wounds made by applying a filter paper soaked in 0.2 N NaOH with vs without limbal damage were produced for the paralleled set of experiment. A 5.5 mm round filter paper for central wound and a 2 mm filter paper strip for limbal damage were used for the initial wounding. Epithelial defect areas were photodocumented after fluorescein stainmg at 0.6, 18, 30, 42, 48 hours following initial damage. Epithelial healing rates were measured by linear regression analysis, The epithelial healing rates of central and peripheral heptatlol wounds were 0.71 +/- 0.14 mm2/h, and 0.79 +/- 0.10 mm2/h(p>0.05), respectively, NaOH central wound without limbal damage and with limbal damage were 0.77 +/- 0.11 mm2/h, and 0.76 +/- 0.11 mm2/h, respectively(p>0.05). These data suggest that the limbal epithelium has no influence on the initial re-surfacing of the central epithelial defect.

7.A Clinical Study of 40 Patients with Tsutsugamushi Disease in Chungcheung Province.

Forty patients(9 male and 31 female) with tsutsugamushi disease were evaluated clinically. The diagnosis of tsutsugamushi disease was confirmed by indirect immunofluoresent test or isolation of causative agent. More than 77% of patients were over 40 years of age. After the incubation period(5 to 13 days, average 8 days) symptoms developed suddenly. The frequent symtoms were fever(100%), chill(100%), headache(100%), rnalaise(97%), and myalgia(95%). On physical exammination the rash, eschar, and lymphadenopathy were found in 97%, 82%, and 67% respectively. Eschar was observed in the trunk, especially axillary, abdominal and inguinal area. Serum transarninase(SGOT snd SGPT) was elevated(89 and 87%). Urinalysis revealed proteinuria in 35%(12/34) and hematuria in 19%(7/36). Chest roentgram and ECG demonstrated abnormalities in 39%(14/36) and 73%(26/36) respectively, The treatment with doxycycline was very effective in all cases.

8.Clinical Features and ECG Findings of Supraventricular Tachycardia in Pediatric Patients.

PURPOSE: Supraventricular tachycardia(SVT) is the most frequent symptomatic arrhythmia in children. We performed this study to disover the SVT mechanisms, age at SVT onset, symptoms and ECG findings of SVT and effect of adenosine on SVT. METHODS: We studied 57 patients(male : 30, female : 27, age : 1 day-15.8 years) who had been admitted or transferred due to SVT from January, 1997 to March, 2001. Retrospectively we reviewed their medical records, ECG and electrophysiologic study findings. RESULTS: Of the total 57 patients, the mechanisms of SVT(including atrial flutter) were atrioventricular reentrant tachycardia(AVRT) in 36(63%) patients, atrioventricular node reentrant tachycardia(AVNRT) in 9(16%) patients, ectopic atrial tachycardia(EAT) in 6(11%) patients, multifocal atrial tachycardia(MAT) in 3(5%) patients and atrial flutter(AF) in 3(5%) patients. Of the 12 primary atrial tachycardias(including EAT, MAT and AF), 11 tachycardias began before 1 year of age. The SVT occured before 1 year of age in 21(37%) patients, at 1 to 5 years of age in 7(12 %) patients, at 5 to 10 years of age in 19(33%) patients and after 10 years of age in 10(18%) patients. In symptomatic SVT, the frequent symptoms were palpitation, chest discomfort and chest pain, gastrointestinal symptoms including nausea and abdominal pain, dizziness and dyspnea in decreasing frequency. P wave was discernible in 31(86%) of the 36 AVRT patients and 5(56%) of the 9 AVNRT patients. Adenosine was effective in 19(91%) of 21 AVRT or AVNRT patients. No significant side effect resulted from was occured by rapid bolus intravenous injection of adenosine. CONCLUSION: AVRT was most frequent in pediatric SVT patients. SVT frequently occured before 1 year of age and at 5 to 10 years of age. Primary atrial tachycardia mainly occured before 1 year of age.

10.A Study on the Role of Bacteria in the Pathogenesis of Confluent and Reticulated Papillomatosis.

BACKGROUND: The pathogenesis of confluent and reticulated papillomatosis(CRP) is still unknown, although many theories have been suggested. It is stressed that abnormal host response to colonization of pityrosporum orbiculare might play a role in the pathogenesis of CRP, but is not completely understood. Frequently, we have observed bacterial colonies in the stratum corneum and, especially, within the hair follicles from biopsy specimens of patients with CRP. In addition, successful treatment for CRP with minocycline, a derivative of tetracyclines, has been reported continuously. OBJECTIVE: The purpose of this study was to document the possible association of bacterial factors in the pathogenesis of CRP and to elucidate the effectiveness of minocycline. METHODS: We performed Brown and Brenn gram staining in 12 biopsy specimens, bacterial cultures, identification of microorganisms and antibiotics sensitivity testing including minocycline. We administered oral minocycline with an initial dose of 100mg per day for 1 to 3 months and a maintenance dose 50mg per day for I to 2 months, then evaluated the response of treatment. RESULTS: 1. On Brown and Brenn staining, the gram positive bacterial colonies that stained dark bluish or nearly black were observed within the infundibulum of hair follicles in 10 out of 12 biopsy specimens(83.3%) and on the keratotic invagination of stratum corneum in 11 out of 12 biopsy specimens(91.7%). 2. Histological features of regions where bacterial colonies were observed showed, hyperkeratosis and keratotic invagination on the stratum corneum in all cases. Hyperkeratosis(66.7%), parakeratosis(16.7%), inflammatory cell infiltration(25.0%), perifollicular fibrosis(33.3%), and abnormal keratin in sebaceous ducts(50.0%) were shown in hair follicles. 3. In anaerobic cultures, no bacteria was grown. In aerobic cultures, staphylococcus species were identified in 6 cases, streptococcus viridans in one case, sternotrophomonas maltophilia in one case. In the susceptibiliity test, minocycline was sensitive in 7 from 8 cases. 4. The therapeutic response to minocycline was observed within one month in all cases, and the time to clear the lesions was 1/2 month in 2 cases, 1 month in 4 cases, 2 months in 5 cases, 3 months in one case, respectively. CONCLUSION: Form these results, we suggest that bacterial factors may play an important role in the pathogenesis of CRP and that the action mechanism of minocycline in the treatment of CRP may be due to antibacterial effects.
