1.Isolated Musculocutaneous Nerve Palsy after the Reverse Total Shoulder Arthroplasty.

Reverse total shoulder arthroplasty has been performed with promising results in rotator cuff tear arthropathy. However, the global complication of the reverse total shoulder arthroplasty is relatively higher than that of the conventional total shoulder arthroplasty. Neurologic complications after reverse total shoulder arthroplasty are rare but there are sometimes remaining sequelae. The cause of the neurologic complication is multifactorial, including arm traction, position and the design of the implant. Most cases of neurologic palsy following reverse total shoulder arthroplasty occur in the axillary nerve and the radial nerve. The authors report on a case of a 71-year-old man with isolated musculocutaneous nerve palsy after reveres total shoulder arthroplasty with related literature.

5.Production of IL - 6 and IL - 8 in Human Fibroblasts Stimulated with Mycoplasma Lysates and Bacterial Toxins.

Cytokines are hormone-like proteins which mediate and regulate inflammatory and immune responses. The purpose of this study was to investigate the effect of lipopolysaccharide (LPS), Staphylococcus enterotoxin B (SEB), and Mycoplasma lysates on regulation of IL-6 and IL-8 production by human nasal fibroblasts. Primary cultured cells were incubated with LPS (1.0 ug/ml) from E.coli, SEB (1.0 ug/ml) from S.aureus, or Mycoplasma lysates (M.pneumoniae, Mp; M. fermentans, Mf; M. hominis, Mh, each 1.0 ug/ml). The culture supernatants were collected at 2, 6, and 24 hr and assessed for IL-6 and IL-8 production by enzyme-linked immunosorbent assay. The production of IL-6 in the culture supematant was downregulated by LPS, SEB, or Mycoplasma lysates. But IL-6 was upregulated by mixed exposure with Mp+LPS (2 hr), Mp+LPS+SEB (24 hr), Mf+LPS (24 hr), Mf+LPS+SEB (2 hr), Mh+LPS (24 hr), Mh+SEB (24 hr), or Mh+LPS+ SEB (24 hr). The production of IL-8 in the culture supematant was similar to that of IL-6 by same stimulants. But IL-8 was upregulated by mixed exposure with Mf+LPS+SEB (2 hr), Mh+LPS (24 hr), Mh+ SEB (24 hr), or Mh+LPS+SEB (24 hr). These studies show that costimulation of LPS or SEB with Mycoplasma whole cell lysates upregulates the production of IL-6 and IL-8.

6.Prevention of Heterotopic Bone Formation after Total Hip Arthroplasty in High Risk Patient with Radiation Therapy

Heterotopic ossification(HO) is a frequent complication of total hip arthroplasty(THA), If it is sevre enough, it can convert a successful result of the operation into a poor one. Among several strategies that researches in recent years have tested, radiation therapy have been accepted to have efficacy for prevention of heterotopic ossification by many authors. While postoperative irradiation has been shown to lower the incidence of HO effectively in high risk patients following THA, patients in multiple dose protocols continue to develop some heterotopic ossifiction despite therapy and have some problems. Ninteen patients received single dose exposure to 600 rad delivered within 48 hours of THA with shielding of prosthesis region. The patients were considered high risk group for developing HO because of hypertropic osteoarthritis, post-traumatic osteoarthritis or the presence of previous formed ectopic bone. 7.Clinical Studies of Brain Abscess in Children.

Clinical investigation of 20 cases of brain abscess has been done at the dept of Pediatric Yonsei Medical Center, who had been treated during the period of 8 years from January 1977 through Jun 1985. Following results has been obtained: 1. Of 20 patients, 14 patient was male and the rest 6 was female rating M:F ratio 2.3:1 and the youngest patient was undler 1 year, the oldest one was 15 years old. 2. The chief complaints at admission were headache, fever, vomiting in order of frequency, while neurologic signs were neck stiffiness, hemiparesis, papilledema etc. 3. The preceeding causative diseases of brain abscess were diseases involving ENT 15%, congenital heart disease 20%, head trauma 35% and the rest 20% the etiology of brain abscess was not determined. The abscess tended to occure at temporoparietal area in the cases associated with otitis media while abscess originating from congenital heart diseases tended to occure multiple area. 4. 10 cases out of 20 had lumbar puncture under the suspision of meningitis, 2 cases manifested normal CSF finding. Patients who has focal neurologic finding associated with OMPC or congenital heart disease should suspect for brain abscess, which can be easily conform by brain C-T scan as well as patient who manifested unusual clinical course. 5. Factor relating prognosis were mental state and duriation between onset of clinical symptom and time starting treatment; clear mental state and short duration reflect better prognosis. 6. The motality rate was 33%, and the neurologic sequelas were hemiparesis (36.4%), mental deficit (18.2), dysarthria, epilepsy, hydrocephalus and blindness.

A 34 year-old woman was hospitalized with anterior chest pain and indigestion. Chest radiograph and computed tomogram revealed a sewing needle in the cardiac cavity. She had no histories of surgical intervention, drug abuse, or acupuncture. We removed the needle from the right ventricle under cardiopulmonary bypass.
