1.Subcapital Stress Fracture of the Femur after Internal Fixation of Intertrochanteric Fracture: A case report.

Fracture of the femoral neck occurred after internal fixation of intertrochanteric fracture of the femur is very rare and have been described previously in terms of stress fracture, stress-riser fracture, Youngs modulus fracture or iatrogenic fracture in the literature. This fracture documented about 20 cases in the English literature and usually occurred in elderly patients with osteoporosis and it always occur in the subcapital region. We report a case of subcapital stress fracture of the femur occurred after internal fixation with compression hip screw of intertrochanteric femur fracture.

2.Limited Two Incision Technique in Carpal Tunnel Release.

Open carpal tunnel release surgery has heen the gold standard method of treatment for who did not respond to conservative treatment and whose neurologic symptoms were progressive. However, open carpal tunnel release using a standard incision frequently associated with delayed return of hand function, residual scar tenderness and pillar pain. So, two new alternative methods such as endoscopic carpal tunnel release and limited incision technique have developed. To define the role of the limited two incision technique, we anaiyzed the postoperative functional results of 40 cases in 33 patients. The patients were divided in two groups. Group A was standard incision group (23 cases in 19 patients) and Group B was limited two incision group (l7 cases in 14 patients). The postoperative functional results were analyzed statistically using chi-square test at postoperative 3, 6, 9 and l2 weeks. There were no significant differences in reliet of numbness and paresthesias in both groups. There was a statistically significant difference in relief of scar tenderness and pillar pain at postoperative 3 and 6 weeks in Group B. There was no significant complication in both groups. We can conclude that the limited two incision technique of carpal tunnel release is a safe procedure which allows rapid return of hand functions with reduced incidence of scar tenderness in the early postoperative stages.

3.Correlation of Endoscopic Redness with Histological Findings in Superficial Gastritis.

Superficial gastritis has been classified as a type of chronic gastritis, since a report of Schindler in GASTRLTIS, 1947. Howev~er, Benedict reported that superficial gastritis is only acute gastritis or shows normal mucosa histologically. The com mon endopical findings of chronic superficial gastritis were adherent mucus, edema, redness. The common redness which were encounterd are patchy redness and comb-like redness(Kammrotung). We studied the relationship between the redness of superficial gastritis and acute inflammatory changes histologically. Each case of superfieial gastritis was biopsied to redening and non-redening mucosa respectively. We collected 24 cases with 48 biopsy specimens. The results are as follows: Acute inflammatory changes were found only one case in redness and none in non-redness groups. There was no difference in acute inflammatory changes in two groups. The degree of mucosal atrophy were 62.5%, 66.7% in normal mucosa, 12.5%, 16.7~% in mild atrophy, 25%, 12.5% in moderate atrophy, 0%, 4.l% in severe atrophy with respect to redness and non-redness mucosa respectively. There was no difference in degree of mucosal atrophy in two groups. Mucosal atrophies were higher in older ages above 41 years old than below 40 and with increasing age, there was increasing tendency of mucosal atrophy. In conclusion, There was no relationships between mucosal redness and acute in flammatory changes histologically and also between mucosal redness and degree of atrophy.

4.The Treatment of Displaced Ankle Fractures with Early Joint Motion and Weight Bearing

The ankle, which at times bears up to five times the body weight, is the most congruous joint of the lower extremity. Recently, importance of early joint motion and weight bearing after anatomical reduction & rigid internal fixation of the displaced ankle fracture was emphasized. Therefore, we studied prospectively, fifty-two patients with displaced ankle fractures admitted to the Department of Orthopedic Surgery, Severance hospital during a period of 4 years from January 1985 to December 1988. In fifty-two patients, group I (24 cases)were treated with early joint motion and weight bearing without cast immobilization and group II (28 cases) were treated with cast immobilization for long duration after open reduction and rigid internal fixation. The result of an average 15 months follow up of these patients were as follows. l. In displaced ankle fracture, early recovry of joint motion was obtained in early joint motion and weight bearing group after accurate anatomical reduction and rigid internal fixation. 2. In clincal evaluation, 95.8% resulted to satisfactory in group I, whereas 85.8% in group II. 3. In radiologic evaluation, 95.8% resulted to satisfactory in group I, whereas 82.1% in group II. 4. In lateral malleolar fracture, accurate reduction and fixation with plate and screws was important. 5. Syndesmotic ligament should be repaired and weight bearing was allowed without removal of transfixing screw. 6. Reduction of lateral malleolus made reduction and fixation of medial malleolus easy, and thus maintaining talar tilt angle. 7. Anatomical repair of deltoid ligament was needed for stability of ankle joint and prevention of local degenerative change.

5.Clinical Analysis and Treatment of Cervical Spine Injury.

This study analyzed 88 patients who sustained a cervical spine injury during the past 4 years(Jan, 1993-May, 1996) in whom had 33 anterior, 21 posterior interventions were underwent and 34 remaining patients recieved conservative treatment with halovest. In 45 cases of upper cervical injuries, 16 operations were done. Among these, anterior approach was used in 3 patients and posterior approach in 13 patients. In 43 cases of lower cervical injuries, 39 operations were done. The anterior approach was used in 30 patients, posterior approach in 9 patients, and bilateral approached in remaing 4 cases. For patients with a predominent posterior ligamentous or osteoligamentous lesion, we selected anterior approach, when closed reduction was possible. Whenever the facet joint remained interlocked, a posterior approach was chosen. This report does not mentioned priority of anterior procedure at any case. Although clinical experience does not support the experimental data, we examined the reliability of anterior approach with use of internal fixation.

6.De Ritis Ratio (Aspartate Transaminase/Alanine Transaminase) as a Significant Prognostic Factor With Upper Urinary Tract Carcinoma Who Underwent Radical Nephroureterectomy and Adjuvant Chemotherapy

Purpose:
To investigate the correlation between preoperative De Ritis ratio (aspartate transaminase [AST]/alanine transaminase [ALT]) and postoperative clinical outcome in patients with upper urinary tract carcinoma (UTUC) who underwent radical nephroureterectomy (RNU) and adjuvant chemotherapy (ACH).
Materials and Methods:
We respectively analyzed the clinical and pathological data of 102 patients who underwent RNU and ACH for UTUC. Patients were divided into 2 groups, according to the optimal value of AST/ALT ratio. The effect of the AST/ALT ratio was analyzed by the Kaplan-Meier method and Cox regression hazard models for patients’ cancer-specific survival (CSS) and overall survival (OS).
Results:
Mean survival time was 50.5±41.2 months. Mean age was 61.4±9.7years. Forty-one of the patients (46.5%) were in the high AST/ALT group. According to receiver operating characteristic analysis, the optimal AST/ALT ratio was 1.2. In Kaplan-Meier analyses, the high AST/ALT group showed worse outcomes in OS (p=0.007) and CSS (p=0.011). Using Cox regression models of clinical and pathological parameters to predict OS, high AST/ALT ratio (hazard ratio [HR], 5.428; 95% confidence interval [CI]; 1.803–16.334; p=0.002), pathological T3 (pT3) or higher (HR, 1.464; 95% CI; 1.156-1.857; p=0.002), and to predict CSS, high AST/ALT ratio (HR, 4.417; 95% CI; 1.545–12.632; p=0.005), and pT3 or higher (HR, 1.475; 95% CI; 1.172–1.904; p=0.002) were determined as independent prognostic factors.
Conclusions
Pretreatment AST/ALT ratio is a significant independent predictor of CSS and OS in advanced UTUC patients receiving systemic ACH after RNU.

Kallmann's syndrome is characterized by hypogonadotrophic hypogonadism resulting from insufficient release of GnRH and associated with anosmia or hyposmia, which has been related to agenesis of olfactory bulbs. We experienced a case of 17 year-old Kallmann's syndrome woman and a case of successful pregnancy of 29 year-old Kallmann's syndrome woman with hMG and hCG treatment and present two cases with a review of literatures.

8.Surgical Treatment of Spondylolisthesis Using Transpedicular Fixation System

We have analyzed clinical results of forty-five patients who had spondylolisthesis which were operated using transpedicular screw fixation devices (CD or Steffee VSP system) between June, 1987 and March, 1992 at the department of Orthopedic Surgery of Kwang-ju Christian Hospital. Simultaneously we compared the postoperative results between two groups. The results were as follows: 1. The ratio of man and woman was 1: 2.2 and mean age was 44.3 years (44.8 years in degenerative type and 42.5 years in isthmic type). 2. The most common level was L4 on L5 and two-third of all cases were isthmic type. 3. The clinical results were satisfactory in 91% (41 cases) according to anthors modified evaluation system. There was no significant difference between degenerative and isthmic type. 4. The mean % of slip was improved from 23.3% preoperatively to 7.4% postoperatively and the mean slip angle was improved from 3.5 degrees preoperatively to-5.4 degrees postoperatively. 5. The devices which were used for operation were C-D instrument in 21 cases and Steffee VSP system in 23 cases. There was no difference in radiographic examinaton and clinical result between two groups. 6. The anterior fusion was done for 9 active young patients. The results were significantly satisfactory. 7. We consider the transpedicular fixation system is the most recommandable method of treatment for spondylolisthesis in providing excellent reduction of slippage and maintenance of reduction with rigid fixation device.

10.A Clinical and Statistical Study on Pregnancy of Elderly Primipara.

OBJECTIVE: Recently, the incidence of elderly primipara is increasing. Thus, impact of old age on pregnancy outcomes became important. Our purpose was to assess the effect of maternal age on pregnancy outcomes. METHODS: 73 cases of the elderly primipara over 35 years at the department of obstetrics and gynecology, Chon-ju presbyterian medical center, from Jan. 1, 1995 to Dec. 31, 1998, were compared with 73 cases randomly chosen young primiparas during same period. RESULTS: Elderly primipara had higher rate of infertility and spontaneous abortion history, antenatal complications, cesarean section, poorer neonatal outcome than young primipara. CONCLUSION: The pregnancy outcome of elderly primipara is considered to be less favorable than those of young primipara. Careful antenatal and intrapartal care should be performed for the elderly primipara. We think that many clinical study of congenital anomaly in elderly primipara will be necessary.
