PURPOSE: Endovenous laser treatment for incompetent great saphenous vein (GSV) has been recently proposed as an effective means of treatment. The aim of this study is to identify the complications and safety of EVLT as the main treatment for primary varicose vein incompetence when used concomitantly with miniphlebectomy and sclerotherapy. METHODS: We reviewed the records of 153 patients who underwent EVLT. Venous duplex scanning was performed 3 months after surgery in all the patients. The clinical outcomes were evaluated using questionnaires at the outpatient clinic or through phone calls. RESULTS: Considering the 97% satisfaction rate of EVLT, EVLT was an effective treatment for primary varicose vein. We observed complications such as paresthesia (20.9%), pain lasting more than 2 weeks (11.8%), ecchymosis or bruising (7.2%) that subsided spontaneously, superficial burn (5.2%) and phlebitis (2%) that was controlled by conservative treatment. The GSV recanalization rate 3 months after the procedure was 3.3%, resulting in a 96.7% success rate. CONCLUSION: The early results indicate that EVLT is an effective and safe procedure to eliminate the SFJ (sapheno-femoral junction) and obliteration of the GSV. However, long-term follow-up is necessary in our study. Better treatment outcomes are possible with accumulated experience and the establishment of strict indications for EVLT.

In order to provide some basic data for the control of air water pollution in Korea, the suthors have estimated the amount of air and water pollutant emitted from industries which are employed over 20 employes. This study have done from July 1, 1972 to the end March 1973. The results are as follows: 1. Total number of establishments tith over 20 employees is 5,197 in Korea and the largest gorup establishments was the manufacturing of texttiles with 1,363 establishments(26.2%). 2. By order of number of employees it was observed that there 2,800 industries with 20-59(53.9%) employees, 1,101 with 50-99(21.2%), 571 with 100-199(11.0%), 501 with 200-499(9.6%) and 225with over 500(4.3%) respectively. 3. By order of regional distribution, it was observed that there were 2,257 industries in Seoul (43.3%) and 736 industries in Pusan(14.2%). 4. Industrial coal consumption was 596,154 M/T in 1972, but it'11 be 315,000 M/T in 1980, Fuel consumption was 4,972,000 K1 in 1972, and estimated volume will be 19,3700,000 K1 in 1980. 5. Ttotal amounts of air polutants emitted from industris by fuel combustion were sulfur oxides 79,459 tons, carbon monoxide 33,908 tons, particulate 31,304 tons and hydrocarbon 30,280 tons in 1972 but in 1990 there will be sulfur oxides 1,010,474 tons, nitrogen oxides 204,575 tons, carbon monoxide 68,014 tons, particulate 64,820 tons and hydrocarbon 67,622 tons, respectively. 6. Annual emitted air pollutants through the working processes were sulfur oxides 91,250 tons and nitrogen oxides 32,485 tons in 1972, but sulfur oxieds 118,625 tons and nitrogen oxides 42,555 tons will be present in 1980, respectively. 7. Annual emitted air pollutants by national unit area amounted to 0.77 tons/km2/year in 1965 and 14.7 ton/km2/year in 1980. 8. Total industrial wastes from all industries in Korea were estimated at 810,360 tons/day in 1972; manufacturing of chemicals and plastic products showed the highest amount of wastes at 470,000 tons/day. 9. The amounts of water pollutants due to industrial wastes were the .B.O.D., 471.5 tons/day,suspended solid 331.5 tons/day, CN, 2.3 tons/day, and Cr. 3. 4 tons/day in 1972, but it might be evident of a B.O.D of 3.388 tons/day, suspended solid 2,544 tons/day, CN 20.1 tons/day, and 26.5 tons/day in 1990. 10. Total population equivalent of B.O.D. was 943,000 in 1972, and the estimated value in 1990 will be6, 780, 000.

PURPOSE: Supraventricular tachycardia(SVT) is the most frequent symptomatic arrhythmia in children. We performed this study to disover the SVT mechanisms, age at SVT onset, symptoms and ECG findings of SVT and effect of adenosine on SVT. METHODS: We studied 57 patients(male : 30, female : 27, age : 1 day-15.8 years) who had been admitted or transferred due to SVT from January, 1997 to March, 2001. Retrospectively we reviewed their medical records, ECG and electrophysiologic study findings. RESULTS: Of the total 57 patients, the mechanisms of SVT(including atrial flutter) were atrioventricular reentrant tachycardia(AVRT) in 36(63%) patients, atrioventricular node reentrant tachycardia(AVNRT) in 9(16%) patients, ectopic atrial tachycardia(EAT) in 6(11%) patients, multifocal atrial tachycardia(MAT) in 3(5%) patients and atrial flutter(AF) in 3(5%) patients. Of the 12 primary atrial tachycardias(including EAT, MAT and AF), 11 tachycardias began before 1 year of age. The SVT occured before 1 year of age in 21(37%) patients, at 1 to 5 years of age in 7(12 %) patients, at 5 to 10 years of age in 19(33%) patients and after 10 years of age in 10(18%) patients. In symptomatic SVT, the frequent symptoms were palpitation, chest discomfort and chest pain, gastrointestinal symptoms including nausea and abdominal pain, dizziness and dyspnea in decreasing frequency. P wave was discernible in 31(86%) of the 36 AVRT patients and 5(56%) of the 9 AVNRT patients. Adenosine was effective in 19(91%) of 21 AVRT or AVNRT patients. No significant side effect resulted from was occured by rapid bolus intravenous injection of adenosine. CONCLUSION: AVRT was most frequent in pediatric SVT patients. SVT frequently occured before 1 year of age and at 5 to 10 years of age. Primary atrial tachycardia mainly occured before 1 year of age.

PURPOSE: It is well known that a high proportion of patients with cerebral palsy have neurogenic bladder. However, cystometry performed to determine the presence and type of neurogenic bladder is an invasive procedure. We evaluated the validity of uroflowmetry and postvoid residual urine volume as a screening tool for neurogenic bladder in children with cerebral palsy. MATERIALS AND METHODS: 34 children with cerebral palsy (range 6-13 years) were randomly selected for this study. Uroflowmetry and residual urine volume were deter mined at least twice in 22 children. Uroflow curve pattern was classified into normal, tower, plateau and staccato type. All patients underwent cystometry with sphincter elec tromyography. We analyzed relationship between the results of uroflowmetry and cystometry. RESULTS: Of the 34 patients, 23 (67.6%) children displayed upper motor neuron lesion of bladder. 11 (32.4%) had uninhibited contraction, 9 (26.5%) small capacity and con comitant detrusor-external sphincter dyssnergia was found in 3 (8.8%). Of 22 children who performed uroflowmetry, 13 showed abnormality. Staccato, tower, and plateau type of pattern was seen 11 (84.6%), 1 (7.7%), and 1 (7.7%) patient, respectively. The re maining 9 (40.9%) children were normal. Eleven of the 13 children with abnormal uroflow curve on uroflowmetry showed upper motor neuron lesion of bladder. Among 13 children with abnormal cystometric results, 11 reveal abnormal uroflow curves. 2 children (22.2%) displayed upper motor neuron lesion of bladder among the 9 children with normal uroflow curve. So, uroflowmetry reveal sensitivity 84.6%, specificity 77.8% for neurogenic bladder in cerebral palsied children. CONCLUSIONS: These results show a high rate of neurogenic bladder in children with cerebral palsy as in previous reports. Abnormalities on uroflow curve correlated well with abnormal cystometry findings. We believe that uroflowmetry is a viable noninvasive screening tool for the detection of neurogenic bladder in children with cerebral palsy.
