A 34 year-old woman was hospitalized with anterior chest pain and indigestion. Chest radiograph and computed tomogram revealed a sewing needle in the cardiac cavity. She had no histories of surgical intervention, drug abuse, or acupuncture. We removed the needle from the right ventricle under cardiopulmonary bypass.

3.Shortening of the Femoral Neck During Healing Period of the Femoral Neck Fracture

Since osseous healing in the fracture of the femoral neck is carried out by endosteal bone formation, the size of contact area of the fracture fragments and impaction are of prime importance. It is not uncommon to be faced with the problem of back out of the internal fixation devices during healing period. It seems unnecessary to emphasize the benefit of sliding to close the gap between the fracture fragments. We report a retrospective study of 25 cases of the femoral neck fractures treated by closed reduction and fixation with multiple Knowles pins, paying particular attention to the amount and duration of sliding back out of the fixation devices and relating these to the final results of fracture union. Our clinical study led us to the following conclusions; 1. The shortoning of the femoral neck over 3mm occurred in 16 cases(64%) out of 25 cases. The average amount of back out of Knowles pin is 6mm. 2. Most of the shortening occurred during the first 8 weeks after surgery and extrusion of the nail is frequent findings in unstable fractures and osteoporoses. 3. Backing out over 12 weeks after surgery was associated frequently with delayed union and non-union. 4. Collapse or shortening is nceessary to promote a bone to bone contact and bone healing.

4.Chemoradiotherapy in squamous cell carcinoma of the anal canal: a single institution experience.

PURPOSE: We reviewed the treatment outcomes and prognostic factors for patients with anal canal carcinoma who were treated with curative intent chemoradiotherapy (CRT) at Severance Hospital from 2005 to 2011. MATERIALS AND METHODS: Data for 38 eligible patients treated during this period were reviewed. All patients were treated with curative intent using radiotherapy (RT) with (n = 35) or without concomitant chemotherapy (n = 3). Among 35 patients who received CRT, most of the chemotherapeutic regimens were either 5-fluorouracil (5-FU) plus mitomycin C (23 patients) or 5-FU plus cisplatin (10 patients). Recurrence-free survival (RFS), colostomy-free survival (CFS), overall survival (OS), and locoregional control (LRC) rates were calculated using the Kaplan-Meier method and survival between subgroups were compared using the log-rank test. Cox's proportional hazard model was used for multivariate analysis. RESULTS: Over a median follow-up period of 44 months (range, 11 to 96 months), 3-year RFS, CFS, OS, and LRC were 80%, 79%, 85%, and 92%, respectively. In multivariate analysis, tumor size >4 cm was an independent predicting factor for poorer RFS (hazard ratio [HR], 6.35; 95% confidence interval [CI], 1.42 to 28.5; p = 0.006) and CFS (HR, 6.25; 95% CI, 1.39-28.0; p = 0.017), while the presence of external iliac lymph node metastasis was an independent prognosticator for poorer OS (HR, 9.32; 95% CI, 1.24 to 70.3; p = 0.030). No treatment-related colostomies or deaths occurred during or after treatment. CONCLUSION: Curative intent CRT resulted in excellent outcomes that were comparable to outcomes in previous randomized trials. No severe treatment-related toxicities were observed.

6.A Case of Malignant Angioendothelioma.

Malignant angioendothelioma is an uncommon malignant vascular tumor featuring endothelial cell, and it occurs most frequently in sixth and seventh decade. Most lesions occur over the face and scalp, and may be solitary or multiple, The lesion usally appears as a localized dark red to purplish nodules or plapue with small satellite nodules. Histopatholgic finding shows a nodule eomposed of irregular vascular spaces lined by plump endothelial cells stroma. We present a case of malignant angioendo thelioma on a 42 year-old woman. The patient had purplish nodule with satellite lesions on the right cheek. This case was confirmed by clinical picture, histologic findings and electron microscopic examination.

8.Study of Calcifying Epithelioma in 9 Cases.

The diagnosis of calcifying epithelioma is more easily made on the basis of the histopathological findings than the clinical manifestations. Calcifying epidermal cyst is only one that must be differentiated from calcifying epitheIioma because of its similarity in histopathological findings. 9 cases of calcifying epithelioma which have been collected at Seoul National University Hospital for the past ten years, frorn October 1960 to September 1970, are reported. Summary of the clinical and histopathological findings are as follows; 1) Range of age: one to sixty years old. 2) Ratio of sex: nearly equal in both sexes (5 male, 4 female.) 3) The final diagnosis of all of the cases was made not on the clinical basis but on the histopathological findings. 4) Only one case out of nine showed multiple skin lesions.

9.The Treatment of Tibial Shaft Fractures by Interlocking Nailing.

The treatment of tibial shaft fracture has become one of the most controversial subjects in orthopedic surgery. Comminuted, segmental and rotationally unstable fractures or bone defect at fraction site have problems of the fixation. The interlocking nail solve these problems. We have experienced 8 cases of the tibial shaft fractures treated with interlocking nail from 1986 to 1988. Authors analyzed these cases and out own clinical study. The results were as follows. 1. The average bone union rate was about 15 weeks. 2. The interval between operation and crutch walking was 4.3 weeks. 3. The merits of this operation were the short hospitalization and early adaptation of social activity. 4. The advantage is be able to do early ambulation without following muscular atrophy or joint stiffness. 5. The results were assessed on clinical examination and radiographic appearance by Hamza et al. An excellent results were 7 cases and good result was 1 case.

10.Clinical Evaluation on Management of Urethral Stricture.

A clinical evaluation was made on patients with urethral strictures, who had been admitted to the Department of Urology, Kyungpook National University Hospital during 7 years periods from January, 1977 to December, 1983. The results were as follows: 1. Out of total 1979 inpatients, 105 were male patients with urethral strictures, giving rate of 5.4% 2. The most frequently involved age group was the 4th. decade being 24.7% of total and 64.6% was seen in age group of 20 to 49 years. 3. Trauma was the most frequent cause of urethral strictures(81%), followed in order of frequency by iatrogenic(11.4 %), inflammatory and post-operative cause. 4. Out of 121 patients with urethral trauma during the study period, urethral strictures were developed in 85 (70.2 %). According to the initial managements of urethral trauma, the incidence of stricture was 74.5% in the suprapubic cystostomy, 78.6% in the primary realignment and 55.9% in the Foley`s catheterization. 5. The treatment modality was surgical intervention in 54 patients and conservative management in 51. Success rates were 90.7% in surgical intervention and 84.3% in conservative managements. Success rate according to the permanent urethroplasty was 93.7% in the perineal end to end anastomosis (30 out of 32 patients), 70% in the transpubic urethroplasty (7 out of 10) and 100% in the Badenoch pull through procedure(4) and realignment(5). 6. As to the late complications, restructure was observed in 7 patients, impotence in 4, false tract in 3 and incontinence in 1.
