5.Microsurgical Training in the Rat for the Arteriovenous Fistula of the Chronic Renal Failure Patients.

For the chronic renal failure(CRF) patients arteriovenous fistula(AVF) is performed on the upper extremity. The primary site is usually the wrist including snuffbox where diameter of the vessels is small, usually one to three millimeters. The trainee of general surgery has been lack of experience of small vessel surgery. Here on the basis of experimental microsurgery, novel animal practice model of AVF was developed for the clinical application. Among several types of vascular anastomosis of AVF, side-to-side anastomosis with distal vein ligation were favored in this study. In brief, rats were anesthetized with intraperitoneal urethane injection and midline abdominal incision was made. Aorta and IVC below the left renal vein was exposed and seperated and mobilized carefully. Side-to-side and end-to-end anastomosis can be performed microsurgically. Loupe(x3.5-5) and experimental microscope may help to carry out vascular anastomosis confidently. The key-point of the procedure is that both corners of stomas were sutured for staying and from the one-end continuous sutures were done with total open-loop method.

6.The Effect of Magnesium Depletion on the Electrocardiogram and Plasma Electrolytes in Liver Cirrhosis.

Plsama and urinary Mg were measured in relation to EKG change in 24 patients with liver cirrhosis, and summarized as below. 1. Plasma albumin and Ca were below normal, however Na, K, and Mg level were in the lower limit of normal. 2. Abnormal EKG findings were QT prolongation, ST elevation, peaked T, low T, ST depression and U wave in the order of frequency. 3. Coefficient of correlation between plasma Mg and QT prolongation was -0.5 compared with that of Ca-0.28. 4. Mg depletion was moderately correlated with plasma albumin (r=0.59). 5. Marked urinary Mg excretion in the presence of normal plasma Mg suggest that plasma Mg level poorly reflect total Mg depletion.

8.A Rat Model of Heterotopic Partial Liver Transplantation with Mesocaval Shunt.

Heterotopic partial liver transplantation(HLT) in the rat is relatively simple method to orthotopic liver transplantation. Addition of mesocaval shunt which diverts almost intestinal blood to systemic circulation provides only splenopancreaticoduodenal blood for the graft. The usefulness of our novel model is first, evaluating the pure effect of pancreaticoduodenal blood to liver regeneration, second, evaluating the contribution of splanchnic viscera to liver reperfusion injury. In the first group (conventional HLT, C-HLT), the thirty percent graft liver was transplanted just below the host liver with whole portal blood input. In the second group(mesocaval shunt added HLT, M-HLT), the superior mesenteric vein was diverted to systemic circulation and portal blood from the spleen-pancreas-duodenum supplied the graft. The graft weight at 2 posttransplant weeks was significantly increased in the C-HLT group compared with the M-HLT group, which suggests pancreatic blood alone is not sufficient to regenerate the partial liver grafts. There was no significant difference in the graft survival between two groups, which implies the influence of intestine to postreperfusion injury is negligible.

9.Fixation for Reattachment of Trochanteric Fragment in Pertrochanteric Fracture Treated with Bipolar Hemiarthroplasty.

Purpose: To retrospectively compare the fixation methods for reattaching a trochanteric fragment in a pertrochanteric fracture treated with bipolar hemiarthroplasty. Materials and methods: Forty cases of an unstable femur pertrochanteric fracture were analyzed and followed up for average of 19(6~40) months. There were 18 cases fixed with tension band wiring (group 1), 7 cases treated using modified tension band wiring with K-wires (group 2) and 15 cases treated with the GTRD (Greater Trochanteric Reattachment Device) (group 3). Result: Group 1 was treated with simple a surgical procedure and the results were good. The second group had firm fixation postoperatively but required additional surgery in two cases to remove the K-wires because of wire migration after ambulation. Group 3 had relatively good results but required more dissection and a longer operating time. Conclusion: Tension band wiring or GTRD are good fixation methods for reattaching trochanteric fragments in pertrochanteric fractures. Moreover, tension band wiring is recommended for old osteoporotic patients due to a simple procedure and firm fixation. The use of modified tension band wiring using K-wire or a Steinmann-pin should not be used due to the possibility of distant migration.

10.Effects of Taxol on the Synthesis of Nitric Oxide in Murine Microglial Cells.

Taxol, an anticancer drug, blocks cell division by stabilizing microtubules. However, taxol has distinct cell-cycle-independent effects. For example, taxol and bacterial LPS induce strikingly similar responses in murine microglial cells. Here, we report that taxol, like LPS, provides a ""second"" signal for murine microglial cell activation to induce tumoricidal activity. Tumoricidal activity determined by MTT assay appeared that taxol or LPS alone weakly activated microglial cells to kill P815 mastocytoma cells, whereas combinations of taxol or LPS with IFN-r synergized to activate macrophages to lyse tumor cells in a dose dependent manner. Secretion of nitric oxide (NO) correlated with tumor cell killing, and the activated microglial cells failed to kill tumor cell targets in the presence of N'-monomethyl-L-arginine (N'MMA), a competitive inhibitor of NO synthase (NOS). Treatment of the cells with anti-TNF-a neutralizing antibodies clearly blocked taxol plus IFN-r induced tumoricidal activity as well as NO production. Collectively, the data illustrate the potential for taxol to activate microglial cell mediated-antitumor mechanisms in addition to its better characterized role as an anti-mitotic agent.
