3.A Case of Allergic Contact Dermatitis to 'Aroma Oils' in an Aroma Therapist.

Allergic contact dermatitis from aroma oils has long been recognized and with increasing frequency. Contact dermatitis of the allergic type of fragrances used for aromatherapy has been reported rarely, and only in patients exposed due to their occupation. We report herein a case of allergic contact dermatitis to aroma oils in an aroma therapist. A 25-year-old woman, an aroma therapist, had been treated with topical steroids under the impression of contact dermatitis. She had had pruritic erythematous maculopapules on the arms extending to the hands for the previous 6 months. Patch tests showed strong reactions to aroma oils(3% fennel and 3% juniper). The skin lesions were cleared after she stopped her job. However, re-exposure to aroma oils caused eczematous lesions repeatedly at the sites previously involved. We predict that increasing popularity of aromatherapy will lead to increasing number of patients developing allergic contact dermatitis from aroma oils. This has to be taken into consideration when testing patients with suspected contact dermatitis.

4.Intramuscular Baker's Cyst in Plantaris: A Case Report.

Baker's cyst is found as most frequent cystic mass around the knee occurring between medial head of gastrocnemius muscle and semimembranosus muscle. The proximal or posterolateral extension of the cyst had been rarely reported and the cyst into the surrounding muscular tissue extremely rare. Intramuscular Baker's cyst that we report was found between lateral head of gastrocnemius muscle and popliteus tendon, and then extended into the plantaris muscle. With review of the literature, we here report a very rare case of plantaris muscular extension of Baker's cyst.

5.Microsurgical Treatment of Cerebral Arteriovenous Malformation.

Five cases of cerebral arteriovenous malformation(AVM) are presented, which underwent total extirpation using the operating microscope. The following are considerations which favor the total extirpation of AVM and the advantages of using the operating microscope, including the rationale for non-use of hypotensive anesthesia or mannitolization which is commonly used in intracranial aneurysm surgery: 1) Relatively small sized AVMs are found to show a high rate of large intracerebral hematoma, including that the degree of bleeding from the malformed vessels is not related to the size of the AVM. 2) The operating microscope facilitates the following procedures: a) determination of the extent of the malformed vessels. b) clipping feeders. c) identification of red vein and laminar flow. d) preservation of normal brain tissue.

9.The Experimental Study for the Reaction of the Hemostatic Agents to the Surrounding Brain Tissue.

In a series of 20 young rats, three cortical lesions were made in each hemisphere. Two of these lesions were filled with avitene and gelfoam, while the third was left empty as a control. The animals were killed successively on weeks 1, 2, 4 and 8 after the operation. The results were as follows : 1) Although there was no difference in the type of tissue reactions, avitene was more rapid and profound than gelfoam in the process and degree of inflammatory reaction at the same periods. 2) Avitene biodegradaded more rapidly than gelfoam. 3) The extent of fibrosis and adhesion to the surrounding brain tissues were not grossly different between these two agents but more profound histopathologically in avitene at the same periods. With these results, we could conclude that the avitene was superior to the gelfoam as the hemostatic agent in neurosurgical area.

10.Spontaneous Spinal Subdural Hematoma: Treatment with Lumbar Drainage.

We report a rare case of spontaneous spinal subdural hematoma(SSDH). A 63-year-old man presented with radicular pain and paraparesis on both legs for several months. On magnetic resonance images, SSDH was found in lumbar region. Electrodiagnostic report showed bilateral lumbosacral polyradiculopathy, such as cauda equina syndrome. SSDH was drained with lumbar drainage at L4-5 level without direct exploration. The patient improved after drainage of the hematoma and then he was able to walk independently.
